In this varied and rewarding role, you will:

  • Assess and treat patients with clear, person-centred therapy plans, delivering effective interventions and reviewing progress.

  • Lead, manage and supervise Support Workers, alongside overseeing key administrative tasks within the team.
  • Work alongside MDT colleagues and other partner services to support high quality interventions and co-ordinated care.
  • Actively contribute to MDT meetings and family conferences, ensuring joined-up care and confident communication with patients and families.
  • Make independent, evidence-based clinical decisions that influence practice and improve outcomes.
  • Use your knowledge of HR policies to manage staff appropriately, fairly, and consistently.
  • Take ownership of your professional development, maintaining and strengthening clinical skills through courses, conferences and workshops, and staying up to date with national and local policies--building deep expertise in chronic disease processes and their management.
  • Work in line with Trust policies, taking responsibility for the health, safety and welfare of yourself and others.
