Shift Patterns: 20 hours p/w between Monday to Friday

Main duties of the job

• Assist the Specialist Dietitians for Adults with Learning Disabilities in the monitoring and follow up of patients requiring home visits across Berkshire.
• Perform initial assessments for designated and triaged patients requiring healthy eating and weight reduction, devising step wise goals, care plans and delivering healthy eating information. To inform support agencies, GPs and referrers of outcomes of review and forward targets.
• Support dietitians with ordering, updating and making amendments to ancillaries and equipment using online software for patients who are home enterally fed.
• Be responsible for delivery and evaluation of Healthy Eating Group Programme for support staff looking after adults with a Learning Disability where appropriate.
• Provide support in the monitoring of the Nutrition Support, Home Enteral Feeding caseloads, residential care home caseloads, under the guidance of the Specialist Dietitian in residential care homes.
