You will work closely with GPs, social care teams, acute hospitals, care homes, voluntary organisations and other system partners to ensure seamless, integrated support for patients across the community. This role requires excellent clinical reasoning, strong organisational skills and the ability to work autonomously while contributing positively to team leadership and service development. If you are passionate about integrated care, supporting safe hospital discharge and helping people live independently within their communities, we would love to hear from you.

  • Provide advice and support relating to safeguarding, medication and long-term care needs.
  • Participate in multidisciplinary team meetings, care home MDTs and system partner forums.
