If you're looking for a rewarding role where you can develop your skills, contribute new ideas to a welcoming service undergoing transformation, and make a meaningful difference, we'd love to hear from you.
Closing date for applications is 23-11-2026, with interviews being held in person on 03-12-2026.

Main duties of the job
You will be responsible for managing a varied caseload, providing assessment, treatment and rehabilitation for adults with a wide range of health conditions within community settings. This will include working with individuals presenting with musculoskeletal, neurological, respiratory, orthopaedic and frailty-related conditions.
You will undertake comprehensive assessments, develop and implement treatment plans, and evaluate outcomes to support recovery, rehabilitation and independence. The role requires effective clinical reasoning, risk assessment and decision-making, adapting interventions to meet the needs of individuals in their own homes and community environments.
Working as part of an integrated neighbourhood team, you will contribute to joint assessments and coordinated interventions, ensuring seamless care and effective communication between services.
The post offers opportunities to further develop your clinical skills while contributing to service improvement and transformation. You will be supported by experienced clinicians and will have access to regular supervision, in-service training and continuing professional development opportunities.
The role requires frequent travel between Livewell Southwest sites and community locations across Plymouth. The post holder will therefore need to travel independently and be able to transport equipment required to deliver patient care.
This role may not be eligible for sponsorship under the Skilled Worker route.
