Bank Band 6 Unplanned Care Registered Nurse - Hertfordshire

CLCH NHS Trust provides a wide range of community healthcare services, and the Hertfordshire Division is looking for experienced and motivated Bank Band 6 Unplanned Care Registered Nurses to support the delivery of high-quality, responsive care to adults in the community.

This is an exciting opportunity to join the Unplanned Care Service, supporting patients who require urgent assessment, treatment and ongoing clinical management within a community setting.

Working as part of a multidisciplinary team, you will work autonomously within your professional scope of practice while collaborating closely with Unplanned Care Advanced Nurses, paramedics, locality teams, other healthcare professionals, statutory services and voluntary sector organisations.

Under appropriate clinical guidance, you will undertake skilled nursing assessments and interventions, contribute to clinical decision-making and help deliver safe, effective and person-centred care. You will also play an important role in supporting patients to remain safely within the community and preventing unnecessary hospital admissions where clinically appropriate.

If you are an experienced Registered Nurse looking for flexible Bank opportunities within a supportive community service, we would welcome your application.

Main duties of the job

  • Undertake comprehensive nursing assessments of patients presenting with urgent or unplanned healthcare needs within the community.
  • Deliver safe, effective and evidence-based nursing care in accordance with Trust policies, procedures and professional standards.
  • Recognise and respond appropriately to changes in patients' clinical conditions, escalating concerns promptly to the appropriate clinician or emergency services.
  • Support the assessment, treatment and ongoing management of patients with acute, complex or deteriorating health needs.
  • Develop, implement and review individualised care plans in collaboration with patients, carers and the multidisciplinary team.
  • Administer medications and undertake clinical procedures within the limits of competence, authorisation and local policy.
