Main duties of the job
<p style="margin:0px;font-size:inherit;padding:5px 0px;">As the <strong style="font-weight:700;">Consultant Geriatrician for our Hospital at Home Team</strong>, you will provide senior clinical leadership and specialist frailty expertise to support the delivery and development of innovative hospital-level care in patient\'s own homes across BSW. Working closely with multidisciplinary teams, operational leaders and system partners, you will help shape clinical pathways, support complex decision-making, champion best practice in frailty care, and drive service improvement. You will play a key role in strengthening partnerships across acute, community, primary care and urgent care services, ensuring integrated, patient centered care that supports admission avoidance and home first approaches. The role also includes leading clinical governance, quality improvement, workforce development and strategic service transformation to ensure Hospital at Home continues to deliver safe, effective and sustainable care for older people.</p><p style="margin:0px;font-size:inherit;padding:5px 0px;">Please see the attached for the full job description for a full list of job responsibilities.</p>
