Main duties of the job
- Report
to the Clinical Site Team, attending 08:30 site huddle, to obtain an overview
of site pressures, and names of any definite discharges that can be pulled to
the Discharge Lounge (DCL)
- Take
accurate handovers- assessing the suitability of the patient for the DCL.
- Access
EDN, pharmacy and transport systems to ascertain any outstanding tasks for the
patients safe discharge.
- Excellent
communication with patients, relatives and all internal and external staff-
aiming to build relationships and promote the use of the DCL.
