JOB TITLE: ADVANCED NURSE PRACTITIONER REPORTS TO: GP PARTNERS (Clinical) PRACTICE MANAGER (Administration) HOURS: PT-negotiable Job summary: The nurse practitioner is an experienced nurse who is able to provide autonomous care for registered patients of the practice, within their professional boundaries. The nurse practitioner will need to prioritise and triage the needs of patients accordingly, seeking GP input as appropriate where the problem lies outside their professional capability. They will demonstrate safe clinical decision making and a high level of independent care including assessment, diagnostic and prescribing skills. They will attend patients in a number of settings including urgent medical problems, chronic disease management and nursing input.

Main duties of the job

The nurse practitioner will work as part of the Ravensworth Surgery team, alongside practice nurses, GPs and administerial staff to support the delivery of practice policies and procedures. The nurse practitioner would be expected to work at both Jarrow and South Shields sites, travelling independently between the practices. Please see job and person spec

Job responsibilities: Clinical:Triage and treat patients wishing to see a health care professional who present with undifferentiated and undiagnosed acute illness or with long term conditions (including adults, children, mental and physical health) making any necessary referrals to other members of the primary health care team / secondary care and ordering diagnostic tests where appropriate. To undertake selected home visits for acute illness and long term condition management as needed. To undertake telephone triage and telephone consultations To answer queries relating to ongoing patient care. Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations. To adhere to and work to professional code of conduct as regulated by the NMC. Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary To prioritise health problems and intervene appropriately to assist patients with complex health needs, and in urgent/emergency situations. To provide safe and evidence based, individualized patient care adhering to practice policies and procedures, with consistency of care. To prescribe where appropriate, adhering to local and national guidance, integrating both drug and non-drug based treatments into a management plan. To provide information and advice to the patient/carer regarding the treatment plan, including risks/benefits and safety netting. To provide health education and counselling. To record a clear, accurate and contemporaneous account of the consultations within the patient notes. To contribute to the practice achieving local and nationally administered quality and prescribing targets in order to sustain high standards of patient care and service delivery. To provide opportunistic care including smoking cessation and sexual health signposting/advice. To provide chronic disease management and reviews. To provide practice nurse duties e.g. cervical screening, new patient medicals, immunisations etc. Administrative: Participate in the administrative and professional responsibilities of the practice team. Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes Ensure appropriate items of service claims are made accurately, reporting any problems to the practice manager Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit Attend and participate in practice meetings as required Restocking and maintenance of clinical areas and consulting rooms Training and personal development Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments. Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate. Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development. If it is necessary to expand the role to include additional responsibilities, full training will be given. Develop and maintain a Personal Learning Plan, taking responsibility for identifying training needs (including personal and mandatory re quirements). Participate in clinical supervision and peer review. To participate in the appraisal of nursing staff. Manage, supervise, co-ordinate and guide the nursing team To assist with the management of the nursing service in participation with other members of the practice team, ensuring relevant information is disseminated to and from the nursing staff Assist with the development and maintenance of practice policies and protocols for all nursing activity Ensure the practice manager is aware of any matters arising or problems / potential problems.
