Job summary

Our Urgent Care Service provides a two-hour response to patients in crisis at home. Our aim is to preventunnecessary hospital admission and facilitate early discharge from the acute hospitals. We also provideunscheduled care out-of-hours for patients who may be at the end of life, or experiencing an urgent need forhealthcare.We carry out detailed holistic assessments, developing personalised plans of care in collaboration withpatients and their carers. Our patients are complex, and require proactive risk management, in order to bemaintained safely at home. We maintain clinical oversight, and ensure all actions are completed appropriately.Nurses use a wide clinical skills base to provide hospital-level care at home. This includes IV therapy, end oflife symptom control and management via syringe driver, catheterisation skills, phlebotomy, woundmanagement, risk assessment, as well as general observations and fundamental nursing care.This service is providing nursing within patients' homes and covering West Kent. The ability to drive isessential to this post.

Main duties of the job

Registered Healthcare Professionals who work in all these services require a range of core skills as follows:Assessment, planning, implementation and evaluation of holistic care linked to packages of prescribed careThe development of integrated care programmes in partnership with the patientTo support in the supervision and development of all junior staff including Health Care Support Workers andStudentsTo actively work as a member of the multi-professional team to provide high quality care to patientsTo undertake a range of competent clinical interventions to support the outcomes of both individual and grouptherapyManage a caseload of patients on a daily basis consisting of patients with complex needs associated within aspecific speciality.Post holder needs a valid driving licence

Qualifications

Essential

  • Registered Healthcare Professional with relevant Professional body (1st Level Nurse/ NMC Degree in a health relevant subject or willingness to complete a degree (within 3 years of appointment)

Desirable

  • Mentoring qualification equivalent

EXPERIENCE

Essential

  • Experience of recent community/acute healthcare gained through education or work experience placement or previous employment

KNOWLEDGE

Essential

  • Awareness of professional issues and recent developments in the NHS Knowledge of up to date clinical practice

SKILLS & ABILITIES
