patients to remain in their homes. Wiltshire Health and Care are actively
supporting this through acute admission avoidance, urgent crisis response, 24
hour community care, and enhanced care for patients living with long term
conditions. As more and more patients are treated outside of a traditional
healthcare setting, the demand for community nurses in the UK is growing
rapidly to ensure a high standard of care to everyone who needs it.

Alongside your clinical role in your patients lives, you will also
uniquely serve as a teacher and educator to the patient. You will be informing
them of future ways in which they can supplement and administer their own care
or instruct their family on how best to care for them. This will ensure that
the patient is receiving the best possible treatment in any scenario.
Main duties of the job
We have an opportunity within Warminster Westbury Community Team to employ
a Band 5 Nurse. Our nurses are expected to work seamlessly within the
multi-professional community teams, working closely with primary and secondary
care, and members of the voluntary sector We are integrated co-located
multi-disciplinary teams which create a wrap around approach in the delivery of
excellent patient care to our diverse population. Within our team we have
District Nurses, Physiotherapists, Occupational Therapists, Care Co-ordinators,
Health Care Support Workers, and Home First Rehabilitation Support Workers.
We actively encourage development of all our staff,
offering regular in-service training and professional development including the
chance to become a Nurse with Special Interest.
We value staff members who embrace change and are
keen to help develop and shape future services. The successful candidate will
be able to provide clear evidence of up to date, relevant clinical practice and
education. Applicants need to have well developed communication skills, good IT
knowledge, and evidence of basic clinical skills. We are looking for an
enthusiastic, motivated and hardworking individual who can work flexibly and
efficiently. The team operates various shift patterns from 07.00 until 22.00, 7
days a week.
