Main duties of the job
This post will be part of a team who care for children and young people with Diabetes. The post holder will work closely with hospital, community and primary care services.
The key purpose of the role is to deliver high-quality targeted family support to families of children diagnosed with diabetes. The post holder's responsibilities will include- supporting families with adjustment to diagnosis, providing parenting support, supporting clinics, and forging working relationships with local schools and services. They will also be responsible for delivering group interventions for parents and young people.
The support will be provided in the home, community and hospital settings. The post holder will work collaboratively with the family, diabetes team and any professional or voluntary services supporting the family and will be required to develop their knowledge about diabetes and how it is managed in children, and in keeping with treatment and technology advances.
The post holder will act as a role model in the delivery of patient care and will provide support to the team in the effective management of the service.
The post holder will provide effective liaison with the multi-disciplinary team and other professional colleagues and agencies involved within the child's care.
The post holder will be required to act in a manner consistent with the NMC Code of Professional Conduct carrying out their duties in accordance with Trust policies, guidelines and procedures.

Job summary
An exciting opportunity has arisen to join the Central Booking Service, and be a part of our Support Service and administration team as part of the Whittington Community Patients Access Service . The Service provides referral management and appointment booking services to Whittington Adult Community Services.
This is the chance to be part of something transformative and innovative and have a positive impact on how we improve patient pathways.
Post holder will also register, source and upload documents for new patients, book and reschedule appointments for all patients and process the outcomes from appointments.
Post holder will be competent in the use of the computer software RIO, Medway .
The post holder will be expected to always provide excellent customer care using Trust Customer Care Standards.
Depending on the requirements of the team, the post holder may be required to work a shift pattern between the hours of 8am to 8pm , Monday to Sunday including Bank Holidays . The post holder will also be expected to travel across Whittington Health sites and work in any of the Health Centres in Haringey or Islington as may required .

Main duties of the job
- To line manage junior staff
- To provide a high quality, helpful and courteous response for members of the public, service users and representatives of other departments or agencies by telephone, showing sensitivity to their feelings and respecting their wishes wherever possible.
- To communicate effectively with callers, managing expectations and ensuring patients are happy with the responses they receive.
- To gather key information from referrers and patients etc. who are calling the service.
- To carry out an initial triage of patients
- To manage appointments for patients, and to provide advice and information to callers about community clinical services, including criteria for eligibility and other service specific details.
- To register new patients onto the electronic patient record system such as RiO, Medway and other relevant patient systems
- To administer the processes for establishing efficient clinics; booking interpreters and transport for patients who require them, ringing to confirm attendances with patients, organising appointment letter print runs and sending out letters, processing outcome and discharge forms, cancelling and rescheduling clinics
- Support the band 4 Administrators in the referral management and appointment booking processes including timely saving of referrals received via the Service Generic
- To work as part of a team and when necessary, assist colleagues in their workloads and provide cover for them during absence.

Job summary
The Medical Examiner Service are seeking to recruit a motivated and efficient Administration Assistant to our team who has a resilient but compassionate nature.
The post holder will provide administrative support to the Medical Examiner team to assist in ensuring that Medical Certificates of Cause of Death for community deaths are issued in line with, departmental, regional and national policy. Working closely with and supporting both the Medical Examiner Officers and Medical Examiners you will gain an understanding of patient journeys within the health system and help to support those who are bereaved due to the death of a relative in the community.
Main duties of the job
- To support the Medical Examiner Officers and Medical Examiners with the administrative duties that come with reviewing a death.
- By preparing and processing Medical Examiner documentation, scanning medical certificates to the registrar and necessary documents to and from the Coroner's Service.
- To support the bereaved by taking their phone enquiries and informing them of the Medical Examiner service and what support they will be receiving from the team.
- Ensuring all relatives are sent a bereavement pack with supportive literature.
- To liaise with medical staff at all levels to ensure completion of the medical certificate of cause of death and cremation papers in a timely manner.
- To input and maintain comprehensive data on all the deaths reviewed by our service using Microsoft Excel spreadsheets.
- To provide administrative support to the Medical Examiner Officers with the running of the office environment.
- To liaise with external providers (GP practices, community hospitals, Hospice, Coroner Service) to ensure accurate MCCDs and fully completed referrals are sent to the service so that prompt medical examiner review can go ahead and completion of the death certificate. Where there is an issue with the MCCD or ME referral, to raise this with the appropriate stakeholder and chase up any amendments required.

Are you a passionate Speech and Language therapist with a keen interest in Voice disorders? We are seeking an enthusiastic and skilled specialist Speech and Language Therapist to lead and continue to develop the Voice disorders service across North Cumbria. This is an exciting opportunity to develop their clinical and leadership skills within this specialist role and continue to link with ENT colleagues.
The post holder will be responsible for the highly specialist assessment, treatment, planning, delivery and evaluation of evidence based Speech and Language Therapy intervention to adult patients with laryngeal symptoms (voice change; persistent laryngeal symptoms)
This post will primarily be based at Workington Community Hospital but there will be a requirement to travel around North Cumbria.
The North Cumbria Speech and Language Therapy team is a small, friendly, supportive team with good opportunities for ongoing training. We have a well developed supervision and peer support system in place.

Are you an experienced and forward-thinking leader seeking an opportunity to make a significant impact on healthcare services? We are looking for an exceptional Deputy General Manager to join our NHS Trust located in the surroundings of the Lake District. As a member of our leadership team, you will play a vital role in delivering outstanding performance, quality, and safety of services within the portfolio of services you oversee within the Urgent and Emergency Care Collaborative. This role offers an incredible chance to work closely with a Triumvirate leadership team consisting of the General Manager, Lead Nurse and Collaborative Chair to continue to shape healthcare services within the Trust.
In this role, you will hold an operational and strategic leadership position, overseeing development and delivery of service strategy and objectives of a particular group of services within the Collaborative. Working closely with the Collaborative, you will be responsible for delivering operational performance targets, enhancing the quality of services provided and ensuring financial balance.
You will be responsible for the planning and organisation of a range of complex activities and associated services, working with other services and areas to achieve success. This role involves providing effective leadership and management within teams, ensuring that services are supported to maintain the highest standards and performance requirements.
Main duties of the job
The post holder will:
- Have overall responsibility for the effective operational management of Emergency services across several sites within the Collaborative.
- Be responsible for the delivery of targets and performance measures against the national constitutional standards.
- Provide leadership in the development of clinical and support services.
- Be the lead manager for a major area of activity, interpreting corporate and national policies.
- Actively support the General Manager and contribute to the development and delivery of service strategy and objectives.
- Actively support the achievement of objectives within both the corporate and Collaborative agenda.
- Effectively manage numerous budgets and resources whilst maintaining a high level of patient care.
- Participate in any on-call arrangement or shift system which is associated with this role outside of normal working hours, in order to fulfil the requirements of the Trust. These arrangements may vary from time to time depending upon Trust requirements in the best interests in service delivery.
- The post holder will be expected to work on multiple sites.

An exciting opportunity has arisen for a motivated Band 6 Specialist Dietitian who has an interest in working with adults with eating disorders.
The post is part time (18.75 working hours per week). You will work into the Richardson Eating Disorder Intensive Day Service (RED IDS) based at Benfield House in Newcastle upon Tyne.
The Richardson Eating Disorder Intensive Day Service is a specialist service based at Benfield House, Walkergate Park Hospital. The service provides comprehensive assessment and treatment for eating disorders for men and women aged 18 and over living in the North East of England. Patients either attend at Walkergate Park or are seen intensively at home. The service also supports families and carers, and links with other services to help develop safe and effective pathways for patients.
You will join our trust wide Dietetics Service, which has over 30 dietitians, clinical support assistants and a dedicated admin team. We have excellent links to multidisciplinary teams and nutrition and health promotion initiatives across the Trust. We take great pride in how supportive we are as a team, our aim is to create an environment where every member of our team can succeed.
Main duties of the job
You will take role in the specialist dietetic assessment, diagnosis, treatment, monitoring, and management of patients in the clinical setting. You will act as a source of expertise in dietetics and nutrition support to patients with restrictive eating disorders and other health professionals. You will be expected to provide cover for community adults eating disorder service and our inpatient beds for patients with restrictive eating disorders (based at St Nicholas Hospital). You will also be given the opportunity to work on service development and Trust wide projects.
You will receive full support from the Trust wide dietetic professional manager, clinical team leads, advanced dietitians and other specialist dietitians. We hold regular peer supervision, journal sessions and training sessions to ensure our practice is up to date and supported.
You will have the opportunity to work as part of multi-disciplinary teams and develop the skills and knowledge to deliver dietetic input within inpatient, community and specialist services to provide excellent, high-quality treatment and care.
The team have strong links with local universities, and we take dietetic students from Teesside and Newcastle Universities. All staff have a valuable role and are expected to be actively involved in supporting the learning and development of our dietetic students.

To work flexibly within a dental team providing high quality care to a wide spectrum of patients across localities within the integrated Dental Service BDH. This includes attendance at meetings on behalf of the Dental Matron and UOB, Service specific.
Responsible for the ordering of equipment and stock ensuring efficient control of levels within certain specialities, budget control and by vetting all orders and maintaining commitment accounting system. Support the Clinical Leads, Divisional Director of Nursing and Therapies, Heads of Service and Dental Matron in the financial management of the service.
Participate in the management and improvement of an integrated Dental Service to include the development of policies and protocols as appropriate.
Management of staff, including Dental Nurses and support staff at all grades including staff appraisal and development and identification of training needs. Job summary
We are recruiting passionate and proactive Neighbourhood Case Coordinators to join the Hertsmere Neighbourhood Proactive Care Service. An exciting part time opportunity (26.25 hrs per week) to play a key role in a new team delivering community-based, person-centred care for residents. Focusing on holistic assessment, care coordination, addressing health inequalities and delivering the strategic ambition of a 'neighbourhood health service'. This role has both an administrative and patient facing element.Working as part of a multidisciplinary team supporting a cohort of residents with moderate to severe frailty who have had 2 or more unplanned admissions within the last year, linking them to health, social care and voluntary sector services.
This service will see the shift from reactive care to proactive identification using risk stratification searches within Primary Care records. Searches will identify a cohort of patients who would benefit from targeted intervention to improve outcomes, overall experience and reduce the likelihood of further unplanned admissions. Operating alongside a number of acute and community services, with the ability to step patients up into appropriate services - including the highly successful virtual ward models in South and West Hertfordshire.
As this is a community-based role, there is a requirement to hold a full, UK driving licence with access to a car for business purposes (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job
This Neighbourhood Case Coordinator role is a hybrid clinical/administrative role which would suit a person with experience working as a health care or therapy assistant in community settings who also has good administration skills to support with producing clinic letters, MDT meeting minutes, appointment bookings, etc.
Clinically you will be visiting patients at home or seeing them in clinic based settings to carry out assessment or monitoring of their long term conditions, providing advice and support to enable them to live well and avoid unnecessary hospital admissions, and liaising with health and social care professionals to ensure that care is well coordinated.
Within this role, the post holder will be responsible for:
o Working under delegated authority from a registered nurse or allied health professional (Neighbourhood Case Manager) to support patients on the Proactive Anticipatory Care Service, with the aim of reducing unnecessary admission to hospital by the completion of preventative comprehensive care plans and care coordination across a range of services.o This support may take the form of virtual consultation and review, face to face assessment or care delivery, professional liaison, and arranging and attending meetings with colleagues from across health and social care.o Work may be delivered in patient's own homes, in clinic, or other community settings.

We are recruiting passionate and proactive Neighbourhood Case Managers to join the Hertsmere Neighbourhood Proactive Care Service. This is an exciting opportunity to play a key role in a new team delivering community-based, person-centred care for residents. The focus will be on holistic assessments, care coordination, addressing health inequalities and delivering on the strategic ambition of a 'neighbourhood health service'.
You'll work as part of a multidisciplinary team supporting a cohort of residents with moderate to severe frailty who have had 2 or more unplanned admissions within the last year, linking them to health, social care, and voluntary sector services.
This service will see the shift from reactive care to proactive identification by using risk stratification searches within Primary Care records. The searches will identify a cohort of patients who would benefit from targeted intervention to improve outcomes, their overall experience and reduce the likelihood of further unplanned admissions. The service will operate alongside a number of other acute and community services, with the ability to step patients up into appropriate services - including the highly successful virtual ward models in South and West Hertfordshire.

Main duties of the job
The post holder will be responsible for:
Overseeing care provided in the Proactive Anticipatory Care Service. This will include line management of the Neighbourhood case coordinator, ensuring proportionate and holistic patient assessment is completed, developing integrated care plans which anticipate potential future needs, using an effective multidisciplinary platform for discussion of complex cases, and maximising the value added by the role of senior clinicians.
This will include:o Provision of a proactive, personalised and multidisciplinary case management approach to "high risk" patients identified through a risk stratification search in the Neighbourhood. This includes those with end stage disease who are approaching end of life, have moderate to severe frailty or dementia and have had 2 or more unplanned hospital admissions.o Facilitation of timely, effective, and purposeful multidisciplinary discussion and decision making, resulting in coordinated and integrated care plans which can effectively anticipate and respond to changing patient needs.o Undertaking comprehensive assessment of complex care needs using tools and templates available within the electronic clinical record.

Main duties of the job
The main duties of the role will be:

- Receiving client telephone calls and answering queries
- Liaise with clinical colleagues on telephone calls and queries relating to clients
- Monitoring inboxes and actioning queries with guidance from the Admin Team Lead
- Managing incoming and outgoing post in line with service requirements and Trust policies
You should have excellent communication, customer service and organisational skills as you will have to interact with a variety of people. Resilience is also required due to the nature of the service.
The ideal candidate should have excellent communication, customer service and organisational skills, as well as a working knowledge of office procedures including IT skills.

This will be a challenging, but rewarding role, working with people presenting with a range of addiction, mental health issues and risks. The post holder must have the ability to communicate where there are barriers and they must be able to work as part of a team. Be confident in working with people and demonstrate strong recovery values in, inspiring hope and empowerment in others
Main duties of the job
You will be expected to support service users through our respiratory clinic pathway including:
Develop a therapeutic relationship with those identified through screening tools, to ensure there is a clear point of contact, joint appointments with key worker
Initiate contact with service users to ensure they are aware of their planned appointments, arrange any support needs required for attendance
Support on the day of appointment
Support any follow up, including that of ongoing referrals, additional appointments and support to appointments where appropriate
To work as part of the MDT
To support the physical health pathway with consideration for phlebotomy training and supporting of ECG's
To obtain feedback from service users and feed this into the wider treatment system. To lead on Your Voice and form part of the triangle of care
Recording progress and concerns, working in a manner which ensures confidentiality and security
Set up service user forums to promote service improvement and change, led by service users
Set up some health-related groups external to the core service, for example allotment groups, fishing groups, Gyms etc.

Job summary
The newly formed Out of Hours Urgent Community Response (OOH UCR) team combines the patients traditionally seen by both District Nursing and our Intensive Home Support Service. This is an exciting time to join the service as we come together to enhance the level and breadth of care and treatment we are able to deliver to our patients in their own home, preventing unnecessary hospital admissions and meeting the national guidance around Urgent Community Responses.
The post holder will contribute a high level of skilled nursing care to patients within the community setting. They will be responsible for assessment, planning, implementation, and evaluation of care and will work across Pennine Lancashire (East Lancashire & Blackburn with Darwen) overnight to support our 24 hour nursing service. Operating across our two bases at Burnley General Hospital and St Ives House, Blackburn, the post holder must demonstrate professional standards of practice and personal qualities and behaviours as expected by the NMC Code and be passionate about high quality patient care. They will need to be able to problem solve and adapt to change, know when to seek support and guidance, and ensure the delivery of safe, personal and effective care.
Main duties of the job
Assists the Clinical Team Leader/Team Sister in the co-ordination and delivery of care in line with nation and local guidance.
- You will visit several patients daily in their own homes
- You will provide high quality nursing care to a range of patients with varying healthcare needs.
- Provide a high standard of holistic assessment and care planning.
- You will be expected to provide wound management, health promotion, medicines management, end of life care
The post holder must demonstrate professional standards of practice and personal qualities and behaviours as expected by the NMC Code and be passionate about high quality patient care. Main duties of the job
Answer 999 calls that come into the Emergency Operating Centre in a timely manner. Utilise the clinical triage software system you will have been trained in to deliver a category of response to our callers. Please note, there may be a change to this software, in the future and if so, full training will be provided. You are required to be polite, courteous and respectful at all times when talking to patients, callers and your peers, in line with the Trusts values and behaviours. Working for our organisation
