Job summary
We are looking for a dedicated and compassionate Health Care Assistant to join our friendly and supportive team. Providing an excellent opportunity to showcase your skills and develop your career within the practice.
In this role, you will play an important part in delivering high-quality patient care, supporting our clinical team, and contributing to the smooth running of the practice. You will work closely with GPs, nurses, and other healthcare professionals, providing direct patient care, assisting with clinical procedures, promoting health and wellbeing, and enhancing the overall patient experience. This role offers variety, professional development opportunities, and the chance to make a meaningful difference to the lives of our patients every day.
Main duties of the job
The following are the core responsibilities of the Healthcare Assistant. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
- Undertake new patient and NHS health checks
- Support the practice nurse with health promotion programmes
- Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, recording findings accurately
- Undertake wound care, dressings and other clinical tasks as required
- Support the practice nurse with the management of chronic disease clinics
- Carry out BMI checks as directed
- Act as a chaperone as required
- To undertake phlebotomy
- Administer flu vaccinations
- Carry out ECGs as requested
- Ensure specimens are recorded and ready for onward transportation
- Ensure the clinical area is adequately stocked and prepared for each session
- Ensure clinical waste is removed from the clinical area and sharps bins replaced in accordance with the practice IPC policy
- Deliver opportunistic health promotion where appropriate
- Carry out any other tasks as directed by the lead nurse in support of patient care

We're recruiting for an individual to join our Physiotherapy services within our friendly UCR/ Intermediate Care and Community Therapy Team in Newton Abbot. Whether you're already working at Band 6 level or you're an experienced Band 5 ready for the next step, we'd love to hear from you!
Main duties of the job
This is a fantastic opportunity to provide high-quality, patient-centred physiotherapy across a range of community settings. You'll work autonomously and as part of a dynamic multi-disciplinary team, which includes nurses, paramedics, occupational therapists, social workers, dietitians, and more.
Your day-to-day will include:
- Delivering high-quality, patient-centred physiotherapy to support rehabilitation and independence at home.
- Managing your own caseload across a wide range of conditions and levels of complexity.
- Working closely with GPs, Consultants, Nurses, Paramedics, Occupational Therapists, Social Workers, and other professionals.
- Preventing avoidable hospital admissions and facilitate timely, safe discharges.
- Participating in our supportive weekend rota (no overnight on-call commitment).
- Engaging in internal rotation opportunities, including Urgent Community Response work, to diversify your clinical skills.

The role requires NVQ Level 2 in a relevant healthcare topic, or experience to an equivalent level, along with basic computer and manual handling skills. Good oral and written communication will be supported by numeracy and literacy qualifications and an empathetic approach to patients. Being a car driven is beneficial due to cross site working.

Main duties of the job

We are looking to recruit band 3 Support Workers who are new to care or have some basic experience on Stevenson Ward at The Harbour. We are looking for individuals with excellent communication skills to support you in caring for patients from a diverse background with challenging mental health conditions. The successful candidate must be willing to work in a fast-paced environment, have a positive outlook and ensure that delivering a quality care, which genuinely engages with patients is a key priority.
Due to the nature of the role we can only accept applications from those age 18+
It is essential that you are able to work flexibly to meet the needs of the service. The service provides support over a 24 hour period, 7 days per week, and 52 weeks a year. You will need to be able to work week days, weekends, evenings, nights and bank holidays where needed.
Main duties of the job
Duties will be wide and varied, and include IPC and cleaning duties, supporting with meal service, escorting patients on periods of leave both on the grounds and into the wider community, supporting patients in times of crisis by way of excellent communication skills and when required utilising approved physical interventions such as PAS. You will be part of a large multi-disciplinary team on the ward, and further tasks may be delegated to you in line with patient care and staff safety.

The South Staffordshire Perinatal and Maternal Mental Service is a specialist service delivering person centred care to expectant and new mothers who are experiencing moderate to severe and complex mental health problems within the perinatal period, or families who have mental health needs associated with loss and trauma occurring in the maternity and neonatal context.

The successful candidate will be joining a supportive, engaged team of Perinatal and Maternal MH psychological professionals, MDT and wider service. The post holder will receive regular supervision and CPD as well as informal support. Across MPFT Perinatal Services there is a strong structure in place comprising of qualified clinical psychologists, psychological therapists and assistant psychologists supported by a Lead Psychologist for Perinatal and Maternal MH.
Main duties of the job
The service is part of MPFT Perinatal Services which includes Brockington Parent and Baby Unit and Shropshire Community Perinatal Mental Health Team. Psychological Professionals play an integrative role in the MDT and are valued and well supported by colleagues. While this post will be primarily focused on delivering psychological support to individual families there will be scope for delivering training and consultation to partner agencies and service development.
The team offers a range of individual and group interventions from a range of theoretical models. Clinical supervision structures are in place to support clinicians to access model specific supervision for specific interventions. We have an established CPD programme within the team and you will be supported to develop your psychological skills in this specialty. The psychology team have close connections with other psychologists within the Children and Families care group, the Maternal Mental Health Service within the Shropshire care group, and perinatal services in North Staffordshire, alongside the local DClinPsy courses offering regular trainee placements, research opportunities and perinatal course teaching.
The services covers the geographical region of South Staffordshire. There is an expectation for the successful post holder to be able to travel to meet the service need. We welcome applications for flexible and hybrid working.

You will provide specialist advice and first-line support to clinical and administrative users, analyse complex system and workflow issues, coordinate fault resolution, monitor system performance and help maintain robust audit trails, access controls, data quality processes and asset records.

The role will also include supervising PACS/RIS administrative staff, supporting safe service continuity and deputising for the Radiology Systems Manager when required.

An exciting opportunity has arisen for two Healthcare Assistants to join the Older People and Adult Community (OPAC) Intermediate Care Unit based in Peterborough.
Intermediate Care Unit is currently a 34 bedded inpatient rehabilitation unit.
Our unit is a joint Nurse/Therapy led facility. We have visiting Consultants and daily GP support. We provide short term rehabilitation for individuals from the age of 18 years who have had a change to their physical health. This may be due to an exacerbation of a pre-existing medical condition, an overall general deterioration in their abilities or acute cause.
Main duties of the job
Main duties will include the delivery of direct patient care, following our Trust policies and procedures, being responsive and respectful to patients, their families and colleagues needs.
To observe and report to a registered member of the team, any changes which may indicate an alteration in the general well being and safety of a patients. This includes observations on patient's clinical condition. To be aware of own limitations and when to seek support and guidance.
To provide patients with personal care as required and demonstrate empathy, dignity, respect, and compassion at all times.
To be an active team player and work with patients, families and members of the wider multi-disciplinary team

We're looking for an experienced clinical pharmacist to join our Acute &Emergency Medicine team at the Royal Stoke site. You'll help deliver a high-quality clinical pharmacy service to high-acuity and complex patients across our Acute Medical Unit, AMRA, AEC, SSU and to the Emergency Department.
You'll work closely with pharmacy colleagues and multidisciplinary team members, with opportunities for education, development, research and networking. You will also train and support the development of junior pharmacists within the Acute Medicine rotation.
We're looking for someone who:
Has experience of delivering evidence-based, cost effective care
Is able to support team development
Has a passion for acute and emergency medicine, and thrives in a fast-paced clinical environment
Main duties of the job
Deliver a high quality, patient focused clinical pharmacy service to Acute/Emergency Medicine
Prioritise and review high-risk patients (e.g. those with epilepsy, diabetes, Parkinson's disease), ensuring their medicines are prescribed and administered within a timely manner
Utilise your independent prescribing skills daily, working within your scope of practice
Provide prescribing support for complex medicines e.g. aminophylline, vancomycin
Support patient flow with mechanisms for obtaining discharge medicines both in and out of hours
Review adverse incidents and take appropriate action to reduce future risk

We are excited to announce that we have been awarded funding from Health Education England to offer the opportunity to come and train with us at Sussex Partnership Foundation Trust (SPFT) as a Mental HealthWellbeing Practitioner (MHWP). Sussex Partnership is a mental health trust, offering care across Sussex to adults, older adults and children with mental health problems. The MHWP posts are new national roles based inour community mental health teams and our emotional wellbeing services (EWS). Your will be part of our psychology and psychological therapies team, receiving high quality supervision and continued professional development and support. If you are successful in this post you will receive NHS funded training at Sussex University, and a salary with us at Sussex Partnership for the placement element of your training. Main duties of the job
There has been recent national recognition that MHWP posts can play a key role in the provision of psychological therapies in our community mental health teams. Job summary
The Perinatal Mental Health Service provides Multi-disciplinary specialised care throughout the West Midlands, to treat mothers with mental health problems, both during pregnancy and in the first year post-partum. There is also community outreach team as well as 10 inpatient bedded Unit at the Barberry. The NHS Long Term Plan (LTP) will see perinatal services expanding its remit to seeing approx. 10% of the birth rate, seeing women for up to 2 years postnatally.
Solihull Perinatal Team provide specialist perinatal service for women living in the Solihull Borough or with a Solihull GP, who are experiencing moderate to severe mental illness during the perinatal period.
We are seeking an additional mental health practitioner from a nursing or social work background to provide maternity cover for a period of 12 months to contribute their clinical skills and care coordinate a specialist caseload.
Main duties of the job
Applicants will need to have a genuine interest in working with women and families in the perinatal period, have excellent communication and multi-agency liaison skills and a have good knowledge of safeguarding. Experience of working in the Perinatal Mental Health field is desirable but not essential. The Women's Secure Blended Service (WSBS) at Ardenleigh has an incredible national reputation, having been a successful pilot site for the blended security model and a key influence in the National Women's Secure Pathway Transformation (WSPT) programme. The service is particularly proud of its gender sensitive, trauma informed model of care and commitment to co-production and improvement.
We work alongside a number of professionals, providing safe, containing environments and interventions to help people understand themselves, their mental health and offending behaviour. The team has been at the forefront of service developments, such as establishing the Outreach Service to prevent avoidable admissions, developing partnership working across the pathway to improve quality of care, diversifying psychological therapies to meet the needs of the client group, and leading on the planning and delivery of a national conference in 2023. The service is currently developing a step-down accommodation to support transitions to the community, improving the pathway for women who access secure mental health services.
Main duties of the job
This exciting role offers the suitable candidate opportunities for leadership, service development and complex clinical work, with a rewarding client group. You will be joining a large and diverse psychological therapies team, consisting of clinical and forensic psychologists, arts therapists, specialist psychological practitioners and assistant psychologists.

An exciting opportunity has arisen for a Band 6 Specialist Speech and Language Therapist to work as part of a multidisciplinary team based at one of our hospitals in Birmingham. You will form part of an expanding Speech and Language Therapy service dedicated to supporting the communication needs of clients who present with mental health needs. Our hospital provide assessment, treatment and rehabilitation for service users with mental health needs.
As an integral member of the multidisciplinary team, you will work collaboratively alongside colleagues from Occupational Therapy, Psychology, Nursing etc. A wide range of training opportunities relevant to the role will be available.
You will receive support by senior Speech and Language Therapists ensuring the successful candidate has the relevant skills work to within a mental health setting.
The post is fixed term for 12 months, no weekend working, flexible working requests considered up to 37.5 hours. Secondments requests will be considered.
This post would suit an applicant who has a genuine interest in working with adults who present with complex mental health and communication needs and would enjoy working in a dynamic environment.
Main duties of the job
The role will be varied and include transferable assessment and intervention skills, planning and implementing evidence-based practice, reviewing and evaluating therapy using therapy outcome measures, provide advice to other professionals regarding the management of clients with communication needs, modifying materials, attending and contributing to clinical team meetings, report writing and completing administrative\managerial tasks relevant to the role.

Our hospital locations are Reaside clinic and Hillis Lodge which are secure units for men.

An exciting opportunity has arisen as part of this model for a registered nurse with a keen interest in providing outstanding support within mental health community settings.
This is an exciting Bank role that builds upon the success of other in physical health to create mental health specific roles that work alongside our GPs and other Primary Care colleagues, while receiving clinical support from our multidisciplinary mental health teams in West London Trust.
The role will offer mental health assessments and brief interventions to referred patients and will also work to support the development of mental health expertise within a defined group of GP practices aligned to a specific PCN.
Main duties of the job
The post holders will provide assessments, formulation and delivery of the psychological intervention/s in primary care, as well as supporting access to MINT or other services such as Early Intervention in Psychosis (EIP), Improved Access to Psychological Therapies (IAPT where needs can be better met by these services.
The post holder would be expected to work closely with experts by experience, local communities and will also be involved in raising the awareness of mental health difficulties in the general population.
o Review and support the practice lists of SMI including completion of annual reviews with the GPs.
o Where appropriate; proactively in conjunction with GPs identify and clarify where people would benefit from mental health input (e.g. IAT or other services such as MINT)
o Facilitate people with long term mental health conditions to access mental health services and transfers of care back to their GP from WLT MINT.

Job summary
We are seeking a compassionate, innovative, and enthusiastic Band 4 Assistant Practitioner to join our Southern Rapid Response and Intermediate Care.
This role includes assessing, planning, implementing, monitoring and evaluating exercise and education programmes according to the needs of the referred individuals while working independently and with other members of the multidisciplinary team.
Furthermore, you will have experience of working with patients with complex healthcare or rehabilitation needs and you will able to demonstrate clinical reasoning skills, to enable you to assess and treat patients within service protocols without immediate direct supervision.
You will be supported with training and career development opportunities which includes a Nationally recognised care certificate and local team competency programme. You will have the opportunity to develop your expertise in a community setting with patients with a range of conditions.

Main duties of the job
- Deliver clinical and therapeutic care, within the clinical setting within set protocols and within the scope of the Assistant Practitioner.
- Perform patient assessment, plan and deliver high standards of care. Develop, implement and evaluate individual care/ treatment plans in accordance with proven competency and after appropriate delegation from a registered practitioner.
- Be responsible for carrying a caseload of non-complex patients.
- Co-ordinate and undertake the safe admission, transfer and discharge of patients in accordance with Trust policies. Perform safe, effective and timely discharge planning.
- Assist patients in maintaining their personal hygiene, including oral hygiene, grooming and dressing needs with respects for culture and religious needs, ensuring privacy and dignity at all times. Enable and assist the patient as directed or outlined within the patient's goal planning progress plan appropriate to their ability and condition.
- To be responsible for safe and competent use of all equipment/ mobility aids.
- To promptly report any equipment failures or accidents to the Therapy staff and other appropriate members of staff, following the correct departmental procedures.
