Job summary
The Coco PCN Home Visiting Paramedic is an exciting opportunity for an experienced and motivated Paramedic to make a meaningful difference to patients across the local community.
Working across patients homes and GP practices, you will provide high-quality, compassionate and patient-centred care, using your clinical expertise to assess, diagnose, treat and support patients with a wide range of health needs. You will work closely with GPs and the wider multidisciplinary team to ensure patients receive the right care, in the right place and at the right time.
This role would suit someone who thrives on making a difference, is passionate about improving patient outcomes and enjoys working collaboratively with colleagues across primary, community and wider health and care services. You will have the opportunity to work autonomously while being part of a supportive team, contributing your ideas and clinical expertise to continually improve the service.

Main duties of the job
Key Responsibilities
- Undertake comprehensive clinical assessments, examinations, diagnosis and treatment.
- Provide high-quality home visits to patients unable to attend the practice.
- Develop and review individualised care and treatment plans, including appropriate investigations and follow-up.
- Recognise and appropriately manage acutely unwell or deteriorating patients, escalating care when required.
- Work collaboratively with GPs and the wider multidisciplinary team to provide coordinated care.
- Support patients and families through clear communication, education and advice.
- Make appropriate referrals and onward care arrangements.
- Maintain accurate and timely clinical records.
- Participate in professional development, training and quality improvement.

COCO PCN is a collaborative partnership between three GP practices in West Surrey & Sussex: Chertsey Health Centre, Ottershaw Surgery and Crouch Oak Family Practice. We are a forward-thinking, community-focused PCN committed to delivering joined-up, proactive and personalised care closer to home.
Our multidisciplinary team provides services including mental health support, paramedic home visiting, cancer care support and care-home coordination, working closely with local health and care partners to improve patient outcomes and experience.

We are seeking a skilled, compassionate and motivated Paramedic to provide advanced clinical assessment, treatment and support across COCO PCN.

We are seeking a passionate and motivated Dietitian to join our dynamic team at UHCW. As a Dietitian within the Nutrition Team you will be working within a multidisciplinary team to deliver evidence-based nutritional advice, assessment, and treatment to a variety of patients, including those with intestinal failure who require parenteral nutrition . The role will include a mix of inpatient care on a variety of wards (surgical, gastro, oncology, intensive care) alongside outpatient clinics.

Main duties of the job
This post is based on the Stroke Unit in Lincoln therefore you will be an active member of the Speech and Language Therapy team in a patient facing role. You will be trained to deliver therapy interventions under the guidance of the Speech and Language Therapists.
You will be a vital player in the team ensuring that patients receive intensive speech and language therapy input in line with national guidelines. This will include delivering both communication and swallowing rehabilitation therapy to patients recovering post stroke. You will be encouraged to participate in service development and will receive regular supervision from the team to further your development.
This post is based in Lincoln and includes weekend working as part of a regular rota shared between you and your speech and language therapy assistant colleagues. Bank holiday cover will also be required on a rota basis. Weekend and bank holiday working will be paid at an enhanced rate. There will be occasional requirements to attend meetings/training across the county.

Main duties of the job
As a HCA on the evening team you will be allocated via our electronic system onto a handheld tablet, patients to visit in their own homes, primarily across the South of the county. These would normally be within a geographic area, but some flexibility is expected to cover patients countywide. The role of the HCA is varied, in supporting patients in their own home with personal care, mobility, and simple wound care and catheter care. All appropriate training will be provided as part of the induction process. You will work independently but also as part of a team, with some visits requiring 2 members of staff. Main duties of the job

All post holders will undertake a post-graduate diploma within the first year of employment supported by an internal structured training and mentorship programme.

We are a forward thinking department and support radiographers in their clinical development to advanced practitioner level and we have Advanced Clinical Practitioners and a Consultant Radiographer
Our radiographers are fully involved in the MDT approach to developments within the department. The department has a major commitment to evidence based practice, and participates in multi-centre clinical research trials, led by our Radiotherapy Research Team. On-going professional development is supported and encouraged.

We have an exciting opportunity for a Band 6 Physiotherapist to join the Critical Care, Surgery and Bariatric therapy team at Whiston Hospital. We are seeking an individual who is a keen and confident practitioner with an enthusiasm to develop the Physiotherapy role within the acute surgery wards and ICU settings, with the inclusion of the bariatric surgery service, providing a caring and responsive service.
The successful applicant will be able to demonstrate excellent skills in assessment of respiratory function, formulating a structured rehabilitation plan from critical illness to discharge, including the provision of equipment along with experience of facilitating home and environmental visits. Attending preoperative education sessions to help enhance recovery by giving appropriate information is an essential remit of the team. They will be responsible for overseeing the Bariatric Surgery service, attending Bariatric MDT and pre-operative school.
The successful individual will possess excellent communication skills to enable patient centred care, effective multidisciplinary team working and raising the profile of the profession within the patient setting.

Main duties of the job
The implementation of evidence based practice and a willingness to identify and be involved in areas for future service development is also necessary. This individual will be enthusiastic, hardworking and motivated to develop their skills and the service they are part of. This will include competent organisational and time management skills with the ability to work flexibly due to varying day to day demands of the service.
The Trust is focused on keeping the patient at the centre of all we do. With this in mind, there is a commitment to professional development of employees to ensure high quality care is provided. This includes regular supervision and appraisal. A willingness to develop leadership skills in the supervision of rotational physiotherapists and delivery of clinical education for students from surrounding universities will also be expected.
As the Critical Care, Surgery and Bariatric therapy team involves Physiotherapists and Occupational Therapists working closely, the candidate would have the ability to undertake basic generic therapy assessment and treatment as appropriate. The successful candidate should have the ability to work flexibly and adaptably to a change in working patterns in view of a 7 day service and one rostered Monday evening a month.
A minimum of 3 years post registration experience is desirable.

We are looking recruit an experienced pharmacist with a keen interest in peri-operative management of patients undergoing elective surgery, and the medicines optimisation of patients following falls or trauma to become an integral part of our well established multidisciplinary team.
The successful applicant will take lead responsibility to support and implement quality-driven inpatient and outpatient services within T&O alongside our lead consultants and advanced nurse practitioners. This will ensure treatments prescribed are used in a safe, effective, prudent, patient-centred and efficient manner.
Main duties of the job
The post holder will promote safe and cost-effective prescribing across the Trust in collaboration with the consultants, pre-op clinics and the orthopaedic MDT. You will lead in drug usage analysis, identification of budget pressures arising from new products, co-ordination of projects and audits and the development and maintenance of guidelines, policies and protocols relating to orthopaedic surgery. You will have significant input into the choice of therapy prescribed for people with immobility, pain and preventative bone health based upon their comorbidities and response to treatment, and prescribing of appropriate treatment.
In addition to supporting our orthopaedic services, the post holder will provide a clinical pharmacy service to our acute surgical wards.
The Trust has recently undergone the implementation of EPMA and the post-holder will have a significant input into improved prescribing and therapeutic drug monitoring using electronic systems.

Job summary
An exciting opportunity has arisen for enthusiastic and self-motivated radiographer to join our Ultrasound team.
The post holder will be responsible for the daily supervision of an integrated, responsive and effective imaging service; coordinate resources to ensure continuity of service; be an integral member of the ultrasound team and ensure compliance with national standards. Being primarily responsible for delivering an ultrasound service where required across the trust, in keeping with agenda for change terms and conditions.
This is a sonographer role that includes clinical duties on a daily basis; contributing towards the day-to-day operation of a high quality, flexible and efficient ultrasound department and the clinical caseload.
The post holder will provide support to the other radiology and trust staff on a daily basis, ensuring best clinical practice and full utilisation of the resources available. They will be on hand to discuss scan findings and any relevant adjustments to patient pathways to enhance patient experience and ensure we give the best service possible to all our patients.
They will work closely with the Principal Sonographer to ensure the department objectives are being translated into daily practice. They must ensure the activity of the department reflects the values of the Trust with the aim of improving the quality of the service delivered.
Main duties of the job
The post holder will be responsible for supporting the department in the following ways:
- To ensure images and examination details are stored appropriately.
- To use ultrasound equipment safely and effectively.
- Adhere to ultrasound guidance and protocols.
- Participate in audits and review activities to ensure continued high department and practice standards.

The Radiology department performs over 310,000 examinations per year and provides a 7-day imaging service; therefore the post will include evening, weekend and bank holiday duties on a rota system.
The successful candidate will work as part of a team to ensure the smooth running of this very busy department, by booking a wide range of imaging appointments.
Applicants should be hard working, enthusiastic and well-motivated with excellent organisational skills and both written and verbal communication skills. Previous experience of telephone work will be an advantage.
Successful applicants will require a satisfactory enhanced disclosure from the Criminal Records Bureau.

Job summary
We are looking to recruit 1 sonographer to join our team at Mersey and West Lancashire NHS Trust. Applicants must hold a recognised ultrasound qualification (PgCert, PgDip or DMU), and be eligible to work in the United Kingdom.
A wide range of obstetric and non-obstetric examinations are undertaken across two sites (Ormskirk and Southport Hospitals).
The department also undertakes seven day working and the successful candidate would be expected to undertake evening and weekend duties.
Main duties of the job
- Responsible for autonomously performing Ultrasound examinations and clinically interpreting the images.
- Responsible for providing a high quality Ultrasound scans in accordance with protocols within the Ultrasound department.
- Responsible for supervision and teaching of junior and medical staff.
- Responsible for keeping up to date with current Ultrasound techniques and maintaining a CPD portfolio.
