Job summary
Oakley Health Group is a large and progressive GP practice supporting over 29,000 patients across four practice sites in Yateley and Blackwater.
We are looking for an enthusiastic, experienced and compassionate Mental Health Practitioner to join our primary care team.
This is an exciting opportunity for a proactive mental health professional who is passionate about delivering high-quality, holistic and patient-centred care within a primary care setting.
The successful candidate will work closely with GPs, practice staff and the wider GPimhs/MHICs team, providing assessment, advice, evidence-based interventions, onward referral and signposting for patients presenting with a range of mental health needs.
We are particularly interested in candidates with experience of working in primary care who are confident managing a varied and complex mental health caseload.
Applicants must hold a degree-level qualification or equivalent in Mental Health Nursing, Occupational Therapy or Social Work, together with current professional registration with the relevant professional body.
Prescribing experience or an existing prescribing qualification is desirable. However, we welcome applicants who demonstrate a willingness to undertake appropriate further training and develop their prescribing skills.
Main duties of the job
The successful candidate will:
Undertake comprehensive and complex mental health assessments, identifying appropriate interventions, support and onward referral.
Provide evidence-based interventions to people experiencing a range of mental health problems and serious mental illness.
Review patients and consider appropriate treatment options, liaising with GPs and the GPimhs/MHICs team.
Act as a professional point of contact for GPs, providing advice, support and signposting.
Manage an autonomous clinical caseload and take responsibility for the quality of care delivered.
Communicate sensitively and empathetically with patients experiencing distress or complex circumstances.
Work collaboratively with GPs, practice staff, mental health services and external agencies.
Liaise with relevant resources including Welfare Rights, Housing Agencies, voluntary organisations and Day Services.
Maintain accurate and timely patient records in line with practice policies, professional standards and NICE guidance.
Undertake relevant caseload administration and participate in multidisciplinary and professional meetings.
Contribute to safeguarding, risk management and serious concern processes as required.
Maintain effective communication with the GPimhs/MHICs team to support safe, coordinated patient care.
Contribute to the ongoing development of mental health provision within primary care and support the continued development of services at Oakley Health Group.

What will you be doing?Day-to-day responsibilities of a Social Worker in Assessment include:
- Acting as the lead professional and ensure all matters in respect of safeguarding are dealt with promptly, effectively and in line with child protection procedures, escalating where necessary.

An exciting opportunity has arisen for suitably qualified Medical Technical Officers to join the All Wales Medical Genomics Service with a flexible approach to work for 37.5 hours per week. These are a permanent and fixed term, full time post based at Canolfan Ieychd Genomig Cymru / Wales Genomic Health Centre in Cardiff. An exciting opportunity has arisen for an experienced, motivated and proactive Paramedic to join our Reactive Emergency Assessment Community Team (REACT).
REACT is an innovative multidisciplinary community service within ESNEFT, bringing together Urgent Community Response (UCR), Hospital at Home, OPAT and Out of Hours pathways to deliver high-quality, patient-centred care closer to home. The service works closely with the East of England Ambulance Service, Hospital at Home, Integrated Neighbourhood Teams and acute services to support admission avoidance and deliver rapid intervention for patients experiencing acute deterioration in the community.
You'll play a key role in delivering the national 2-hour Urgent Community Response standard, supporting patients to remain safely in their own homes wherever appropriate and promoting our Home First philosophy.
This role offers the opportunity to influence the development of innovative community pathways, strengthen system-wide relationships and contribute to the ongoing transformation of urgent community care services across East Suffolk.
The role is available for up to 37.5 hours per week and includes participation in a 7-day service operating between 08:00 and 20:00, including weekends and bank holidays.

Main duties of the job
Provide expert paramedic assessment, clinical decision-making and intervention for patients with acute illness, frailty and complex health needs within their own homes.
Support admission avoidance through rapid assessment and timely intervention, ensuring patients receive the right care, in the right place, at the right time.
Triage and prioritise referrals, including those received through the East of England Ambulance Service, ensuring delivery of the national 2-hour Urgent Community Response standard.
Provide visible clinical and operational leadership during shifts, supporting patient flow, safe caseload management and effective deployment of resources.
Act as a clinical role model and professional leader, supporting supervision, appraisal, education and development of staff across the multidisciplinary team.

Main duties of the job
This role requires the advanced assessment, planning and delivery of rehabilitation programmes for patients within the offender healthcare setting who may have complex and/or chronic presentation. You will be responsible for determining clinical diagnosis and clinical treatment indicated, and to maintain contemporaneous records as an autonomous practitioner.
The successful candidate will have excellent communication and organisational skills and be willing to learn and develop.
You will have Occupational Therapy BSc (Hons) degree level qualified (or equivalent) with current HCPC registration. Additional to this, you will have proven post-registration experience in the assessment and treatment of inpatients with a variety of occupational therapy needs and general rehabilitation needs along with proven continued professional development.

North Cumbria Integrated Care NHS Foundation Trust are looking to recruit a caring and energetic Clinical Lead Dentist to lead our community dental team in Allerdale and Copeland.

The ENERGISE-BD trial (Evaluating Nutritional Ketosis versus NHS EatWell Recommended Guidelines for Bipolar Disorder) is a randomised controlled trial for bipolar depression. It will compare the efficacy of nutritional ketosis approach with the NHS EatWell dietary guidelines.

Contribute as a key member of a team of researchers, alongside a Senior Dietitian (Jude Munn), recruiting patients and guiding these patients on the two dietary interventions being tested in this study and other clinical research (70% of time)
Collecting data on participants in the study, specifically relating to progress with delivering both of the nutritional interventions (10%)
Providing day-to-day supervision and support to other members of the ENERGISE-BD team, including a postdoctoral researcher and a clinical research fellow (5% of time).

Job summary
The successful candidate will work as a Band 6 SLT across the adult acute wards and stroke ward at Newham University Hospital - including the acute admissions unit, intensive care unit, respiratory unit, gastroenterology service and older peoples services.
This post provides an excellent opportunity for a Band 6 SLT to consolidate their knowledge and skills in managing patients with swallowing and communication impairments across the acute and stroke pathways, within a friendly and supportive team. They will provide specialist assessment and treatment to adults with acquired communication and swallowing difficulties in the inpatient setting, in collaboration with the multidisciplinary team (MDT). There will be exposure to, and opportunities to develop competency in, videofluoroscopy and tracheostomy management, as well as quality improvement initiatives.
The post holder should be an established, qualified SLT who is independent in the assessment and management of patients with dysphagia and communication impairment. There will be opportunities to develop and support junior SLT colleagues and/or students. Regular supervision will be provided, including access to joint assessments, formal 1:1 meetings and peer support sessions. There are excellent continuing professional development (CPD) opportunities within the SLT service and as part of the wider MDT.

- To independently manage a caseload of adults with swallowing and communication disorders on inpatient acute wards at Newham University Hospital
- To use specialist knowledge to independently assess, differentially diagnose and treat patients across the pathways, seeking support of more senior colleagues where required.
- To be actively involved in service development and to contribute to the ongoing development of specialist clinical pathways
- To deliver training/teaching to SLT colleagues, the wider multi-disciplinary team and students across Barts Health NHS Trust

Job summary
We are looking for a motivated and enthusiastic pharmacist to join our progressive, pharmacy team to cover an upcoming maternity leave.
To provide advanced clinical pharmacy and medicines related care to service users with mental illness, learning disabilities and substance misuse problems.
Your focus will be to lead on and provide complex medicines optimisation advice and professional leadership to the inpatient services based at one or more LYPFT hospital sites.
You will work as an integral member of the Multidisciplinary Team(s) but also independently to proactively identify, respond to and develop patient centred treatment recommendations for medicines queries, prescribe medicines if appropriately qualified and within scope, speak with service users/carers about medication to improve service users' outcomes and experience of their medication.
You will lead and provide professional guidance to a team of pharmacists and technicians based at the Newsam Centre to deliver an efficient pharmacy service alongside the Lead Technicians. This will include undertaking line management responsibility for Clinical Pharmacists, Band 6 / Specialist Clinical Pharmacists, Band 7 pharmacists and mentor other pharmacists in the Trust (including rotational trainees/undergraduates) as required.
As part of an excellent teaching organisation, you will have a responsibility to educate and develop the pharmacy and wider healthcare workforce.
Main duties of the job
Participate in the on-call and weekend rota working at one of our inpatient sites.

Main duties of the job
We are looking for a motivated individual to support the Occupational Therapy team in delivering rehabilitation focused care that promotes independence and safe discharge home.
Under the supervision of qualified Occupational Therapists, you will manage an allocated caseload and provide interventions to maximise patients' functional abilities following hospital admission. Responsibilities include reviewing mobility and transfers, completing functional assessments such as wash, dress and kitchen tasks, and providing feedback and recommendations to the Occupational Therapy team.
You will assist with therapeutic groups, complete outcome measures including the DERBY Outcome Measure and Elderly Mobility Scale, record basic observations and maintain accurate patient records. The role also involves assessing, providing and reviewing equipment, supporting patients to use equipment safely, and undertaking access and discharge home visits to identify adaptations that promote safety and independence.
You will contribute to discharge planning, complete referrals to community services with support, communicate effectively with patients, families and members of the multidisciplinary team, and participate in team meetings. The successful candidate will also support the development of rehabilitation and healthcare support staff through training, guidance and leadership.

The role of the Community Paediatric Audiology Department is to provide the school entry hearing screen for all eligible children and to provide a comprehensive hearing assessment service to any child from 0-19 years about whom there may be concerns regarding hearing.
The post holder (s) takes responsibility for the testing of hearing capabilities of children in schools, mainly in Carlisle/Penrith/Eden and surrounding areas. The post holder has a designated caseload of schools and is responsible for the hearing screening testing of all children with consent to be hearing screened in those schools. The post holder tests, records results and monitors outcomes and is also responsible for the appropriate onward referral of these children as and when required depending on test outcomes and following local protocols.
The post holder is required to organise the School Age Hearing Clinics and during these clinics is required to perform more complex tests to determine hearing threshold following national protocol. The post holder is required to assist the Community Paediatric Audiologist in testing pre-school age children using a full range of behavioural audiological assessment procedures following national protocols and recommended procedures.
The post holder is the first point of contact for parents / guardians and other professionals regarding aspects of their work.

To communicate daily, and work closely with, the Paediatric Audiologist.
To communicate daily with schools, school nursing staff, health visitors, ENT department staff and specialist teachers within the Education department, communicating complex factual information on test procedures, results and outcomes.
To communicate complex and sensitive information to parents/carers on a daily basis using tact, sensitivity, persuasion and reassurance.
To work closely on an individual basis with school age children of all ages in schools, using tact, sensitivity, persuasion and tact.

Vacuum/sweep, mop floors
Floor buffing
Shampoo carpets with extraction machines
Low/high damp dusting
Clean inside windows, frames and sills
Clean furniture and fittings, mirrors etc, to include moving of furniture and equipment as required to enable efficient cleaning
Assist in induction of new staff
Routine cleaning and basic maintenance of cleaning equipment
Attend all Mandatory training sessions
Order weekly stores and put away on delivery

Job summary
We are developing an Integrated Community Stroke Service (ICSS) within Great Yarmouth & Waveney, bringing together community stroke pathways into a more coordinated, responsive and patient-centred model of care. As part of the wider left shift ambition, this service will support earlier discharge from hospital and improve outcomes through timely, multidisciplinary rehabilitation in the community. Our Year 1 focus is on building a more responsive and integrated local service, increased MDT capacity and improved continuity of care across the stroke pathway.You will be joining a service at a key point of development, with the opportunity to shape how care is delivered and contribute to a more integrated, forward-thinking model of community stroke rehabilitation.
A seven day service will be developed in the future.
Main duties of the job
Working alongside the other MDT members including nurses, physiotherapists, occupational therapists and assistant practitioners your duties will include:
To promote recovery and well-being, supporting families and empowering patients to self-manage and maximise their independence using health coaching techniques and meaningful activity.
To be responsible for delivering Speech and Language Therapy assessments in communication and dysphagia and provide therapy, education and advice to stroke patients, some of which may have complex and/or chronic presentation.
To collaborate with Speech and Language Therapy colleagues and other services to support the stroke pathway.
To assist with teaching and supervision of other team members and students.
To assist evaluation of the Integrated Community Stroke Service and to be actively involved with service development within the speciality and to implement changes in stroke services delivery as required.
To work as part of an integrated Stroke MDT, following evidence based practice. This will include a combination of face-to-face and telerehab sessions, devising therapy programmes carried out by yourself or an assistant practitioner, writing self management plans for patients and signposting to community-based communication and support groups (such as those provided by voluntary organisations).
This role requires you to be competent to a minimum of proficient level on the RCSLT eating, drinking and swallowing competency framework.

Job summary
The CCPNR is a specialist community service for children with non-progressive acquired brain injuries in the East of England. It is a dynamic, innovative service provided by CPFT that has an international reputation for delivery of integrated, multidisciplinary neuro-rehabilitation for children.
CCPNR is also embedded within the Psychological Medicine for Children and Families service at Addenbrookes and is part of the children's directorate within CPFT
The post holder will provide leadership and specialist psychological supervision for community rehabilitation at the Cambridge Centre for Paediatric Neuropsychological Rehabilitation (CCPNR). We would consider a job share.
Main duties of the job
The post-holder will provide highly specialist neuropsychological assessment, formulation and intervention for children and young people, including the selection and interpretation of complex psychometric assessments. They will develop and deliver evidence-based neurorehabilitation plans within a multidisciplinary team, offering expert consultation, advice and training to colleagues, families, schools and wider agencies.
They will take clinical responsibility for risk assessment, treatment planning, care coordination and progress monitoring, communicating complex information sensitively to patients and professionals. The role includes contributing to service development, leading research, audit and evaluation projects, and providing supervision to trainees and psychologists within the team.
The post-holder will maintain high standards of clinical record-keeping, use specialist IT and statistical tools, and support quality, safety and governance processes including safeguarding, incident reporting and adherence to NICE and Trust guidelines. They will also contribute to staff development, recruitment, and ongoing training within the service.

<p>Are you a compassionate and motivated Audiologist with a passion for improving the lives of children and their families? We are looking for an enthusiastic <strong>Band 6 Paediatric Audiologist</strong> to join our friendly and supportive team, providing high-quality, family-centred audiological care.</p><p>The post holder will be responsible for providing an audiological assessment service for children aged 0 to 19 years including children and young people with complex needs. The post holder is required to work autonomously and to make decisions about the management of the children that they see in clinic. They will be required to work in a multidisciplinary team alongside Associate Audiologists, Paediatric Audiologists from the local acute trust, Sensory Support Teachers, and Specialist Paediatricians.
