Job summary
Oakham Medical Practice and Market Overton and Somerby Surgeries have the exciting opportunity to recruit a Practice Pharmacist.
Applications are warmly welcome from Pharmacists at any stage in their career. We can provide a stable and supportive background for newly qualified Pharmacists and those in their first 5 years or equally the opportunity to grow and develop if you have more experience. The role will be based across the three Practices, with flexibility to work from home (hybrid).
We are a dynamic, creative and forward thinking Partner led team, training practice for GPST1-GPST3s and have supported numerous ACPs and Pharmacists throughout their training. This new role will be integral to our medicines management team and involve working to provide a safe medication service as well as innovatively managing our patients with long term conditions as part of our multi-disciplinary team.
The Practice uses SystmOne and operates Monday to Friday.

To work as an autonomous practitioner, within their professional boundaries, as part of the Practice multidisciplinary team. They will be supported by the Partners and Lead Pharmacist.
The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

Market Overton is a picturesque village in the North of Rutland with a branch surgery in the village of Somerby just across the border in Leicestershire. The Practices serve 20000 patients as well as providing medical cover for the local public school in Oakham.
We are part of Rutland Health PCN and work collaboratively and effectively with our local partners in general practice, the community and social care.

Undertake clinical medication reviews, including structured medication reviews, with patients.
Answer relevant medication related enquiries from GPs, other practice staff, other healthcare teams, hospitals, District Nurses and community Pharmacies.
Undertake clinical audits of prescribing in areas directed by the GPs and ICB, feedback the results and implement changes in conjunction with the Practice Team.
Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.

Main duties of the job

Main duties of the job
Operate the National Breast Screening System efficiently on a daily basis. Making and changing appointments over the telephone. Accurately entering complex clinical information. Monitor the efficient running of the service on a daily basis by co-ordinating and prioritising clerical duties. Your role will involve...
Completing holistic assessments that include all housing factors impacting on the individual and their household.
Working alongside Health colleagues to assist with processing referrals to achieve safe, early discharge and prevent delays.
Referring to universal services where appropriate and liaising with other colleagues, including Family Operations, to meet the needs of the individual.
Seeking to resolve needs associated with specific impairments by the provision of equipment, support and signposting to alternative services where appropriate.
Making recommendations based on assessed need, for the provision of certain property types, adaptations and equipment, taking account of both Health and Housing policy and legislation.

About You
Qualified Occupational Therapist or equivalent qualification, with current Health and Care Professions Council (HCPC) registration.

We are now looking to appoint an ACC Clinical Coder who will enjoy the challenge of working in a Specialist Cardiothoracic Coding team at Royal Papworth Hospital (RPH) NHS Foundation Trust on the Cambridge Biomedical Campus. You will be providing accurate clinical coding to National Standards and Clinical Coding rules and conventions. The ideal candidate will have good organisational skills, an eye for detail, accuracy & the ability to work as part of an established team. The coding team is based across our Cambridge Biomedical Campus and Huntingdon administrative offices, the post is hybrid, a mixture of office and home based. Initially the successful candidate will be expected to spend two/three days in the office during their onboarding period.

About the Role
As a senior Health Care Assistant, you will:
- Deliver high-quality, compassionate palliative and end of life care in patients' own homes.
- Support patients with personal care, emotional support, and practical needs.
- Work closely with registered nurses to ensure coordinated and holistic care.
- Provide respite and reassurance to families and carers, helping them feel supported and empowered.
- Uphold the values of compassionate, trustworthy, respectful and inspirational in every aspect of our care.

We are looking for individuals who are:
- Passionate about delivering person-centred care at the end of life.
- Excellent communication skills.

During our programme you will work through the 15 Standards of the Care Certificate, learning how to meet the daily living needs of your patients as well as being a part of the NHS healthcare team. This includes supporting Nurses and other healthcare practitioners in providing patient care, from hygiene to personal and physical comfort.
Typical duties will involve but not limited to:
- Carry out clinical procedures/tasks as required.
- Clinical tasks will be determined by the nature of your work and local policy e.g. monitoring the effects of medication.
- Obtain and test samples and other specimens
- Clearing and re-stocking consulting rooms
- Contribute to discharge process
- Care of wounds
- Check pain levels and requesting pain relieving medication on patients behalf
