Join an exciting new service launching on 1 September 2026 and help shape the future of care closer to home.
We are looking for enthusiastic Occupational Therapists and Physiotherapists to join our new countywide Home First service. This is an exciting opportunity to work within an innovative multidisciplinary team supporting people to recover, regain independence and remain safely in their own homes following hospital discharge.
Working across acute, community and home settings, you will provide timely assessment, rehabilitation and intervention for adults with a range of physical, functional and rehabilitation needs. You will play a key role in supporting safe discharge planning, admission avoidance and preventing unnecessary hospital readmissions, whilst working closely with health, social care and voluntary sector partners.
This role offers the opportunity to work in a dynamic and supportive environment where you can develop your clinical skills, influence patient outcomes and contribute to the successful delivery of a new integrated service designed to improve patient flow and independence across Gloucestershire.
This role will be part of a rota working weekends and also in-reaching to our acute hospitals to improve patient transitions from hospital to home.
Main duties of the job
As a member of the Home First team, you will assess, plan, deliver and evaluate rehabilitation and therapy interventions for adults within acute, community and home settings, supporting individuals to maximise independence and achieve their goals.
You will manage a varied clinical caseload, undertaking holistic assessments, risk assessments and treatment planning for people with a range of physical, neurological, orthopaedic, medical and rehabilitation needs. Working autonomously and alongside the multidisciplinary team, you will support safe and timely discharge planning, admission avoidance and the prevention of unnecessary hospital readmissions.
The role involves providing rehabilitation, equipment assessment, advice and education to patients, carers and partner organisations, whilst ensuring accurate clinical records and outcome measures are maintained.
You will contribute to clinical governance, quality improvement, audit and service development activities, whilst maintaining high professional standards and evidence-based practice. The post holder will also support and supervise therapy assistants, students and other learners as appropriate, contributing to workforce development and the delivery of high-quality patient-centred care.
The successful candidate will be flexible, proactive and committed to helping people remain independent within their own homes while working within a fast-paced, developing service operating across a 7-day model.

Job OverviewThis is an exciting opportunity to join the new Countywide Home First Front Door Service, launching on 1 September, and play a key role in shaping and supporting this innovative service from the outset. - Act as the first point of contact for Home First referrals and enquiries from patients, carers, healthcare professionals and partner organisations.

- Support the safe and effective management of urgent referrals in collaboration with clinical leads.

The Home First service is transforming the way support is delivered to people following discharge from hospital, helping them regain independence and remain safely in their own homes. The Care Plus Group Patient and Events Medical Services has two discrete elements.
The safe transportation of medical well patients for the sole purpose of transporting individuals for discharge from hospital, or from community facilities for example, care home or home to hospice or attendance for Out-Patients Appointment.Provides a community response provision to falls that are criteria levels 1 and 2.
The second element is to provide event management services providing both First Aid support or Medical services for a range of events, including festivals, concerts sporting event and other community events.

An opportunity has arisen to join CPG in the role of Ambulance Technician for our Event and Patient Transport service.
You will:
- Be responsible for the assessment, treatment, diagnosis, and administering of medicines, managing, discharging and referring patients in a range of urgent, emergency and critical out of hospital settings, which may include but not limited to ambulances, single response vehicles (SRV.
- Provide ad hoc training and supervision for junior staff ie first responders when on shift.
- Be responsible for ensuring all Health and Safety regulations and Road Traffic legislations are adhered to in relation to transportation and vehicle safety including using advanced driving skills under blue light for emergency response.

Main duties of the job

ABOUT THE ROLEAs a Night Senior Carer at a Barchester care home, you'll look after the different needs of our residents to enable us to deliver quality, person-centred care and support. Your role will involve supervising and inspiring a team of dedicated Care Assistants alongside the Deputy General Manager. You can also expect to support the clinical team in a specific area, such as medication, assessments, care plans or care reviews. Like everyone here, as a Senior Care Assistant, you'll always respect the dignity and preferences of our residents as well playing your own part in creating an environment that's warm, welcoming and vibrant.
ABOUT YOU You should have an NVQ Level 3 or Advanced Diploma in Health & Social Care to join us as a Senior Care Assistant. We'll also need to see that you have specific knowledge of clinical care for the elderly. We're looking for people who are warm-hearted, compassionate and committed to giving each resident individualised care and support. You'll have the opportunity to develop your career through ongoing training opportunities, so there's no limit to how far you can go with us. Many of our Senior Carer Assistants progress into Care Practitioner, Regional Carer or nursing roles.
