Job summary
BHFT is developing a specialist multidisciplinary Assertive Outreach (AO) service, aligned with Community Mental Health Services, to provide safe, compassionate and flexible support for people with severe and enduring mental health needs who may struggle to engage with services.
The service will initially operate from two hubs in Reading and Slough, and we are recruiting two Band 6 nurses to support its rollout.
This role is based within the Reading team and is one of several new specialist posts. The successful candidate will also work across the Trust to help develop a consistent AO approach and promote best practice.
Our approach is centred on:
- Working alongside individuals, families, carers and wider support networks to overcome barriers such as trauma, mistrust, communication needs, co-existing conditions and social challenges.
- Delivering proactive, intensive, holistic and flexible care, meeting people where they are and tailoring support to their needs.
- Promoting fair access to mental health care, particularly for marginalised and underserved communities, while addressing health inequalities.
To achieve this, we work with smaller caseloads than traditional Community Mental Health Services, enabling more intensive support. The post holder will be part of the Reading Assertive Outreach Team and work within the service's operational hours. Care will be delivered primarily in community settings, rather than expecting service users to routinely attend health facilities.
Main duties of the job
- Within BHFT's Assertive Outreach Service, the post holder will provide intensive, flexible and proactive support to people with severe and enduring mental health needs who struggle to engage with traditional services. This Band 6 role offers an opportunity to help shape and develop a new service while delivering high-quality care to service users and those who support them.
- Based in Reading, with occasional cross-cover across Berkshire, the post holder will deliver recovery-focused, evidence-based nursing interventions to adults experiencing significant challenges with engagement.
- Working within a shared caseload, the post holder will build and maintain therapeutic relationships with service users, families and carers, using assertive, flexible and person-centred approaches to improve engagement, reduce relapse and crisis, and promote recovery and independence.
- The role involves completing comprehensive assessments, developing and reviewing personalised care plans, and delivering evidence-based interventions tailored to individual needs.
- As a core member of the Assertive Outreach multidisciplinary team, the post holder will contribute specialist mental health nursing expertise to formulation, care planning and decision-making. They will work collaboratively with colleagues and partner agencies, applying evidence-based practice and reflective working to support stability, recovery and improved quality of life for service users.

We are recruiting relief/bank Approved Mental Health Professionals to support the work of our 24 hour AMHP Service.
This is an opportunity for an experienced and dedicated person to join our countywide Approved Mental Health Professionals (AMHP) service as a Relief AMHP.
The role is only open to those who are already qualified as an AMHP.
As a Relief AMHP joining the 24/7 AMHP Service, you will undertake work on a rota basis, coordinating and completing Mental Health Act assessments as well as responding to urgent social care requests out of hours, in accordance with the relevant legislation and under the guidance of the management team.

The Avenue Surgery is seeking an enthusiastic and forward-thinking clinical pharmacist to join our like minded team. The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The post holder will be supported to develop their role to become a non-medical prescriber.
PRIMARY DUTIES AND AREAS OF RESPONSIBILITY
Patient facing long term condition clinics
See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required.
Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines, making appropriate recommendations to GPs for medicine improvement including IIF/ QOF requirements.
Patient facing clinical medication review
Undertake clinical medication reviews with patients and produce recommendations for nurses and/or GP on prescribing and monitoring including IIF/ QOF requirements.
Patient facing care home medication
Undertake clinical medication reviews with patients and produce recommendations for the GP on prescribing and monitoring including IIF/QOF requirements.
Work with the care home staff to improve safety of medicines ordering and administration.
Patient facing domiciliary clinical medication review
Undertake clinical medication reviews with patients and produce recommendations for the GP on prescribing and monitoring including IIF/QOF requirements.
Attend and refer patients to multidisciplinary meetings.

To reconcile medicines following discharge from hospital, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure the patient receive the medicines they need post discharge.
Set up and manage systems to ensure continuity of medicines supply to high risk groups of patients.
Signposting
Ensure that patients are referred to appropriate healthcare professional for the appropriate level of care within an appropriate period of time.
Repeat prescribing
Produce and implement a practice repeat prescribing policy.
Manage the repeat prescribing reauthorisation process by reviewing patient requests and reviewing medicines reaching review dates and flagging up those needing a review.
Ensure patients have appropriate monitoring tests in place when required.
Risk stratification
Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches.
Service development
Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components.

Job summary
This is an exciting and rewarding opportunity to help make a real difference to Older Adults in acute Older Adult functional and dementia inpatient care in North Wales.
This senior post is to provide psychological input and oversight in acute care Older Adult psychology services in the in BCUHB Central area.
Core aims will be to provide psychological leadership for the continued development and delivery of a biopsychosocial MDT for acute Older Adult functional and dementia care, MDT interventions within older adult dementia care inpatient settings, and to lead the provision of expert advice, consultation, training and supervision in psychological approaches within these settings with the acute care pathway in mind.
The post holder will provide psychological interventions, behavioural and neuropsychological assessment, staff training, support and supervision within the acute dementia care pathway in Conwy and Denbighshire. The post holder will also be required to provide direct expert psychological clinical input to service users and families.

Main duties of the job
To be an active and integrated member of the inpatient multidisciplinary mental health team.
To provide a highly specialised clinical psychology service to clients of the sector, providing specialised psychological assessment and therapy to clients, and consultation to staff.
To work autonomously in accordance with The British Psychological Society Code of Conduct, Ethical Principles and Guidelines 2004, and Health and Care Professions Council.
To plan and provide individual, and group interventions, both directly and via the guidance of MDT members.
To carry a specialist clinical psychology caseload of adult clients who present with complex mental health presentations and in an inpatient setting, including offering specialist neuropsychological assessments, formulation, individual and group interventions.
To provide effective clinical leadership by demonstrating and promoting excellence in the provision of highly specialist assessment, formulation, evidence-based intervention and offer advice and consultation in complex clinical cases to other health professionals.
To act as a highly specialist resource to the wider professional community.

We are looking for an experienced Physiotherapy Assistant to join our specialist 7-bed bariatric rehabilitation service at Ribble Court Care Home.
Working alongside our registered Physiotherapist, you will support the delivery of individual rehabilitation programmes designed to improve residents mobility, strength, function and independence, with the aim of enabling them to return home following their period of rehabilitation.

Job summary
Join our dedicated Breast Imaging team as a Band 6 Mammographer and play a vital role in delivering high-quality, patient-centred breast imaging services across Betsi Cadwaladr University Health Board. This is an exciting opportunity for an experienced, enthusiastic and compassionate mammographer to develop their specialist expertise while contributing to a service that makes a real difference to patients' lives.
As a member of the team, you will undertake mammographic imaging and support a wide range of assessment and interventional procedures within a modern symptomatic breast imaging environment. You will work closely with Consultant Radiologists/Mammographers, Breast Care Nurses and multidisciplinary colleagues helping to provide an efficient, responsive and high-quality service.
The post offers opportunities to expand specialist skills, contribute to service improvement and help shape the future of breast imaging services within the Health Board.
We are looking for a motivated professional who currently holds a PG Certificate in Mammography or equivalent, is passionate about delivering exceptional care and committed to continuous learning and innovation.
Main duties of the job
As a Mammographer you will perform high-quality mammography examinations supporting a range of specialist breast imaging and diagnostic procedures, ensuring excellent patient care, safety and experience at all times. You will work autonomously within your scope of practice while acting as a key source of specialist knowledge and support for colleagues.
Your responsibilities will include assisting with assessment clinics, supporting Practitioners during image-guided procedures and techniques and contributing to multidisciplinary patient pathways. You will provide reassurance and emotional support to patients throughout their breast care journey, often during sensitive and challenging situations.
You will contribute to the day-to-day organisation of the service, support quality assurance and governance activities, participate in audit and service development and play an active role in teaching, mentoring and supervising radiographers, assistant practitioners and students. The postholder will promote best practice, innovation and continuous improvement to ensure the delivery of a safe, effective and compassionate breast imaging service.

We are looking for Healthcare Assistants who enjoy the diversity and variety of caring for patients in crises within their own home, as well as supporting people to have the best possible quality end of life care.
Main duties of the job
This role involves providing personal care, meal preparation and medication support, wound care, prompting prescribed exercise programmes, providing prescribed equipment and completing night-sits. Good communication and collaboration as part of the Urgent Community Response nursing and therapy team is essential. You will have regular supervision and professional development opportunities.
Urgent Community Response is a short term service, we make people safe and refer them on to other appropriate services as needed. Our focus is primarily admission avoidance to secondary care, keeping people safe in their own homes, including people at the end of their life, and also supporting discharges from secondary care or in-patient units.
The role also supports community Nursing teams with Wound care and injections.

Are you due to qualify for your Occupational Therapy Degree? Or are you currently practicing as a Basic Grade but are interested in the experience that an acute hospital rotation will provide? If so, we have the job for you!
We are looking for dynamic and enthusiastic Junior Occupational Therapist to undertake a post within the Therapy Teams at the University Hospital of North Midlands. This rotational post offers a wide range of opportunities to work in an exciting Major Trauma Centre across the medical, surgical and specialised divisions across both sites.
There are a wide range of rotational experiences available; hand therapy, acute rehab trauma unit, emergency portals, cardiology and medicine, dementia and delirium, amputee rehab, critical care, oncology and renal. For more detail of the type of skills and experience you will gain, please see 'Main duties of the job' below.
Main duties of the job
Below outlines details of the clinical opportunities available on this rotation:
Hand therapy-Treat patients in an out-patient setting who have hand injuries including splint design and fabrication.
Acute Rehab Trauma Unit-Early rehab of neuro surgical and poly trauma patients including cognitive and functional assessments.
Emergency Portals-Rapid decision making for patients in A&E to support discharge planning, equipment provision and admission avoidance.
Cardiology and General Medicine-Treat patients with medical conditions such as MND, Cardiology and post Critical Care Step down.
Dementia and Delirium (Frail Elderly)- Develop skills in frailty and falls management and supporting patients with dementia and delirium.
Amputee Rehabilitation (Surgery)-Treat patients following lower limb amputations including wheelchair and environmental assessments.
Oncology and Renal Treat patients with oncology/renal conditions, including fast track discharge planning, manual handling assessments and equipment provision.
Critical care- Develop skills in anxiety management, cognitive assessments and early functional rehabilitation in a high dependency environment.
