Are you a motivated and enthusiastic District Nurse with a Specialist Practice Qualification looking to continue developing you career providing NHS care?
We are looking for a highly skilled and enthusiastic Band 6 NMC registered District Nurse to join our South Tandridge team.
You will need to be forward thinking, motivated and able to work collaboratively with patients, their carers and health and social care colleagues.
You need to have a flexible approach to working some weekends and bank holidays as the teams provide a 365 day service.
This role is part-time, 22.5 hours per week (potentially 30 hours) across a 7 day a week service, with an expectation to travel across the geographical patch.
Main duties of the job
Working as part of a team, you will be supported by the Team Lead to be responsible for the continued management of a caseload of patients. You will be working with other health care professionals across the neighbourhood, providing high standards of care to people in their own homes who have a wide range of nursing needs; aiming to avoid hospital admission, promoting self-care and enabling people to be cared for in their preferred place.
As a District Nurse you will have line management responsibilities and will support the Team Lead with the teams ongoing development and well-being.
Due to the nature of the role, it is essential that you can drive, have a car suitable for business purposes, and a valid driving licence.

We are seeking a motivated and enthusiastic Band 6 Registered Nurse or Allied Health Professional to work within Care Connexions. Main duties of the job
This exciting role will support the ongoing development of an integrated community response model, working in partnership with acute trusts, primary care, Northwest Ambulance Service, the voluntary and third sector, Social Care, therapy services and wider community teams.
The role will focus on clinical triage, ensuring referrals are responded to in a timely and effective way so that patients are directed to the most appropriate service. You will work closely with a Multi-disciplinary team consisting of Advanced Clinical Practitioners, Nurses, Therapists and Assistant practitioner to support safe, coordinated care planning.

An exciting opportunity has arisen for a band 5 nurse to join Doncaster intensive Home Treatment Team. We provide short-term intensive support in the community to individuals with acute mental health needs to prevent hospital admission.
We provide a service between 08.00 and 21.30 7 days per week and staff are expected to work shift patterns to cover this. Most team members work 3 long shifts, which helps promote a good work life balance.
The team works in partnership with third sector and peer organisations to deliver person centred crisis care, therefore transparency and openness is vital.

Main duties of the job
To provide a clinical service that is patient focussed, accessible and responsive on a 7-day a week basis.
To triage all mental health referrals into the Team to determine the priority for service response and the appropriate clinical pathway.
To provide and deliver highly skilled nursing care, including full mental health assessment, care planning and care delivery, biopsychosocial interventions, clinical risk assessment and management, and promoting recovery and inclusion.
To recommend and refer, where treatment is indicated, to the most appropriate team/service.
To work collaboratively with the crisis house to lead on care for admissions as a service.
To facilitate early discharge from the inpatient wards where clinically indicated.
To deliver home based treatment interventions for all patients, ensuring best practice is implemented.

What you will bring to the roleYou will have significant experience in the assessment, differential diagnosis and management of communication and dysphagia in the adult inpatient setting and as part of a multidisciplinary team. Established leadership skills are essential for this role, joining our dynamic team of Band 7 leaders across our adult SLT service. You will be expected to operationally and clinically lead the acute inpatient team working cross sites (Leeds General Infirmary and St. James University Hospital). You will work alongside B7 peers and support the clinical development of the team.
There is an established instrumental service within LTHT with weekly clinics for both Videofluoroscopy and FEES (fibre optic evaluation of swallowing) assessments. We have recently purchased new FEES equipment which shows our investment in providing a quality service for our patients.
Governance is a key component of this role with the post holder expected to undertake regular audit, collate clinical outcomes, and participate in quality forums within your assigned clinical support units and Speech & Language Therapy with a view to continually improving quality, safety and productivity. It is desirable to have previous experience in completion of quality improvement projects.
Patient safety is a key focus area within the NHS, LTHT and our department. We are keen to continue to support and work collaboratively with staff in LTHT to minimise patient swallow related safety incidents, and drive improvements in safety and quality.
Teaching will be a significant part of your role, both SLT and MDT teaching. You will have experience in providing teaching and supporting competency development with Speech & Language Therapy colleagues. You will have evidence of experience in mentoring and clinical supervision.

Job summary
The post holder will work as a member of a multidisciplinary CMHT, the post will be for those patients that have already been assessed and given a formal diagnosis of Dementia by the team.
The model for the post diagnostic care is a tiered step up and step-down model of care in partnership with the Alzheimer's society led by Rdash. This post is for the moderate level of patient need that requires clinical advice and interventions with multiple complexity of need to prevent break down in the community.
To be the lead professional for the review and coordination of care needs, planning and implementation of evidence-based care to this defined patient group and their carers as part of a multidisciplinary approach.
To provide professional advice and support to other agencies and members of the multidisciplinary team. To act as a role model and resource for junior/less experienced staff.
Main duties of the job
To be responsible for the further assessment, planning implementation and evaluation of the evidence-based care required, including health promotion for an identified group of patients, who have already been diagnosed as having Dementia.
o Responsible for the appropriate clinical risk assessment of patients contacting the team as appropriate.
o Providing an effective advice, guidance and decision-making process culminating in the coordination of any reviewed plan of care, within MDT support.
o Responsible for the promotion of carer and patient involvement within the service and for its provision.
o Responsible for working alongside a Dementia advisor from the Alzheimer's society in a tiered model of care whilst been part of the larger MDT within the CHMT.

Job summary

Job summary
An opportunity has arisen to recruit a talented, enthusiastic, determined and resourceful Drug and Alcohol Practitioner to work within this challenging service to provide services within Spectrum's prison-based Drug & Alcohol recovery-orientated treatment model as part of a forward looking, flexible, innovative, and creative team. Our 0-19 integrated service works to achieve the best outcomes for our children, young people and families living in the borough through the Healthy Child Programme. As a result we are looking for qualified registered nurses and midwives who are passionate and motivated to train as specialist public health nurses for Health Visiting or School Nursing.
The entry requirements for the 12 month programme are:
- Qualified registered nurse or midwife with an active NMC registration willing to undertake a fully funded one year academic course at the University.
- BSc 2:2 degree (or above) qualification equivalent to 360 credits at level 6 or evidence of previous education and learning to this level (If unsure of the above please contact the university)
- Applicants must be able to travel independently and have access to a vehicle for business purposes throughout the course. Where necessary, adjustments will be made in accordance with the Equality Act
This is a fully funded and salaried course starting in January 2027 (comprising 50% academic and 50% clinical placement in accordance with university and Trust regulations) subject to confirmation by Health Education England.

We are looking for an enthusiastic clinical or counselling psychologist to work within the Brent Integrated Psychology & Psychotherapy Service (BIPPS) and Brent Community Mental Health Hub (BCMHH) .
BIPPS provides psychological therapy to clients experiencing complex mental health problems. The post holder will be expected to provide specialist psychological assessment and evidence-based interventions.

The Psychosis Pathway is a supportive and dedicated psychological therapies -led treatment pathway that is committed to supporting people aged 18+ who are experiencing symptoms of psychosis. Service users supported by this pathway often have been given the diagnosis of Schizophrenia or Bipolar Affective Disorder.
The Post holder will primarily be working with service users who are receiving care from the Depot Clinic, which provides monitoring and medication management. The role will focus on supporting access to psychological interventions to this client group. Training and supervision will be provided to support the post-holder in the role.
The post holder will be commitment to the recovery model, team working, and service user / carer involvement. Experience of working with diversity in a multicultural setting is vital.
Main duties of the job
The post-holder will work with clients presenting with a range of complex emotional and social difficulties. The Mental Health Hub, Early Intervention Service and inpatient wards, provide care for those clients whose needs cannot be adequately met within a primary care setting .
The post-holder will provide specialist psychological assessment and therapy for clients with complex mental health problems. They will also support the provision of the psychosocial programmes offered within the service. The psychologist will be expected to offer advice, training, and consultation to other members of the team and non-professional carers. Training and experience in models such as CBT, CBTp, FI, DBT, EMDR and ACT will be utilised within this role.
The post-holder will attend weekly team meetings and case discussion forums. They will have an active role in managing referrals and planning clinical care. They will contribute to the ongoing development of effective recovery-oriented treatment models. They will also play an active role in initiatives to ensure optimal use of psychological therapy resources, that are widely accessible and offered in a timely manner. The post-holder will support the team in assessing the psychological and social needs of patients referred to the service.

We are looking for highly motivated people to be part of a multi-disciplinary team, providing positive care for our service users across Camden and Islington NHS Foundation Trust's Hospital Division, in the role of Band 3 Activity Worker.
Working on our acute inpatient wards at Highgate Mental Health Centre campus, the post-holder will be supervised by the ward OT and will work with the ward team to co-ordinate a programme of meaningful ward activities based on service user choice and interests.
Training and a robust induction will be provided to support the post holder in this sometimes challenging environment and support role and professional development. Post holders will be expected to participate in their own skill development and engage in supervision and reflective practice as part of the role. All ward based staff are required to undertake training in the Prevention and Management of Violence and Aggression (PMVA).

Active Neuro, part of Active Care Group, is transforming neurorehabilitation in the UK and were now looking for a Specialist Occupational Therapist to join our Birmingham team. Youll support adults with acquired or traumatic brain injuries, neurological conditions, and complex needs, helping them rebuild independence, confidence, and quality of life.
Main duties of the job
We've invested heavily in enhancing the service, including the development of a brand-new, purpose-built neurorehabilitation gym designed to support the delivery of world-class therapy. The new facility will connect seamlessly with the main centre and will include advanced rehabilitation technology, robotics, and specialist treatment spaces, giving therapists access to the tools they need to deliver innovative, goal-focused rehabilitation. Equipment such as Luna Robotic EMG, Exo Motus, and Arm Motus will enable intensive, integrated therapy approaches that are tailored to each persons goals whether that's baking, reconnecting with art, gardening, or everyday living tasks. As part of the therapy team, you'll also have the opportunity to contribute to the development of this exciting new space, helping shape an environment that supports both clinical excellence and exceptional patient experiences.
You'll be part of a supportive network across Active Neuro, sharing expertise, accessing mentorship, and benefiting from personalised CPD tailored to your clinical interests from functional rehabilitation to cognitive and perceptual retraining. As part of our transition to Active Neuro, the therapy service is evolving into a seven-day model of care to better support the rehabilitation needs of our clients. As a result, occasional pre-arranged weekend working will be required. You will also be expected to work Bank Holidays as required by the service, with enhanced pay provided in line with company policy.
