We are looking for an enthusiastic, experienced nurse or paramedic to join us as a Visiting Clinical Care Practitioner /Community Matron. If you love building relationships, enjoy working autonomously, and want to make a real difference to frail or housebound patients, we would love to hear from you.
Why this role is special
We have already seen fantastic results from our current Community Matron, who works primarily in local care homes. Because of this success, we are expanding the service to include our housebound patients, giving them better continuity, more personalised support, and high-quality care in the comfort of their own homes.

Main duties of the job
The Post-holder will be an experienced nurse/paramedic, who acting withing their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe, clinical decision making and expert care for patient withing the general practice.
The post-holder will provide this service to our care home residents, patients in their own homes and at the surgery.
The post-holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
The post-holder will build effective working relationships with other health care professionals.
The post-holder will work withing the team of GP's, nurses and health care assistants to deliver excellent primary health care services within the Practice.

Duties and responsibilities:
Management of patient Health and Illness
To deliver a high standard of patient care as an Advanced Care Practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.
Provide a first point of contact within the Practice for patients presenting withundifferentiated, undiagnosed problems, making use of skills in history taking, physicalexamination, problem-solving and clinical decision-making, to establish a diagnosis andmanagement plan.
Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
Where the post-holder is an independent prescriber: to prescribe safe, effective andappropriate medication as defined by current legislative framework.
To prioritise health problems and intervene appropriately, including initiation of effective emergency care.
Promote health and well-being through the use of health promotion, health education,screening and therapeutic communication skills.
To clearly communicate the health status of the patient using appropriate terminology,technology and format.
Provide safe, evidence-based, cost effective, individualised patient care.
To work within the Practice guidelines, policies and protocols.
To maintain accurate and contemporaneous records, utilising computer systems whereappropriate and consider the Caldicott Principles and GDPR Regulations in relation to all data handling.

TEAM ROLE
As a clinician to be available to support other healthcare team members and to accept referrals from other team members.
To liaise with all members of the Primary Health Care Team and other agencies localauthority, social services, secondary care, voluntary sector, and Primary Care workers in order to assure appropriate care is provided for the practice population.
To participate as a key member of the multi-professional team through the development of collaborative and innovative practice.
To value all team members.
To delegate appropriate tasks to the other team members whilst still maintainingresponsibility for this area.
To support the delivery of health and safety guidance as agreed within the Practice.
To support the writing of maintenance and delivery of CQC guidance within the Practice.
ORGANISATIONAL
To complete records, audits, reports and respond to appropriate questions and requests.
To attend meetings as requested.
Support effective communication channels between the whole team.
To keep the Lead ACP informed about pressures and difficulties or problem areas when aware.
To contribute to Practice strategy and developments when requested.
To work according to Clinical Governance and support the Clinical Governance Agenda.
Understand that common patient complaints may also be catalyst for change within Practice procedures.

Are you an experienced Occupational Therapist with an interest in Cardio Respiratory and Surgery? We are looking for a dynamic Highly Specialist Occupational Therapist to work as part of the Critical Care and Surgery Therapy Team across these specialties, including amputee rehab, long term respiratory and cardiac conditions, hence requiring a broad set of transferrable advanced skills as well as a desire to take the MDT on an exciting journey of discovery in the role of Occupational Therapy within this field.
The successful candidate would work in conjunction with the Band 7 physiotherapists within the team supporting staff and service development, utilising local and national drivers and reporting to the lead Therapist.
Healthcare is a 7 day service and therefore the expectation will be for you to support weekend working.
Interviews for this role will be conducted in person to assess relevant skills effectively.
Main duties of the job
The role involves;
- Networking with Clinical specialists within other fields and throughout the NWA Trust to streamline and progress services.
- To work closely with the wider MDT on Critical Care to develop OT service provision in Critical Care, including early rehab, orientation and functional rehab.
- Assisting the MDT, including therapists with the rehabilitation, discharge planning or onward referrals of complex patients on the wards.
- Taking an active role in and support the skill development of staff working on our wards, including junior OT staff and Degree Apprentices

Main duties of the job
As a Ward Manager you will be highly visible and accessible to children, families and staff and will focus on improving the fundamental and essential aspects of family care. The role will be overseeing the children's ward and children's elective care. Training and development opportunities will be offered and bespoke to individual needs.
Reporting to the Matron and/or Associate Director of Nursing for Children's and Neonatal Services, the post holder is responsible for and will have a specific focus on visible, accessible and strong clinical leadership, patient experience and the delivery of safe, timely, high quality care.
The post holder will support service and quality improvement programmes alongside supporting the operational management of the Division. This includes ensuring efficient and effective use of resources and the development and maintenance of the highest standards of patient care and service delivery.
The post holder should have extensive experience of children's nursing and development, able to work clinically with expertise and credibility. Candidates need robust experience post-qualifying, sound knowledge of the Children's Ward environment, be approachable and knowledgeable of managing emergency situations, experienced in training and mentoring staff as well as having proven leadership skills experience of operational management would be desirable.

At WUTH we understand the importance of clinical coding to support patient care and help improve health outcomes. We are looking for an enthusiastic and motivated individual with excellent communication skills to join our friendly Clinical Coding team as a Clinical Coding Team Manager.
Reporting to the Head of Clinical Coding, the post includes responsibility for the daily supervision of a team of coding staff. The role involves working alongside existing Team Manager and a Clinical Coding Trainer and Auditors, to help develop the Clinical Coding service further.We are committed to training and developing our staff so applicants for this position must hold the National Clinical Coding Qualification (UK), have post-qualification experience and be able to demonstrate attendance on Refresher and Specialty Workshops.
The role is flexible in combining on-site and home-based working. Applicants may be required to work on other sites and in other areas within the Trust. All applicants must be willing to maintain their skills by attendance on workshops and refresher training courses.
This role includes an RRP of 7%.

To manage a team of Accredited Clinical Coders to provide accurate, timely and complete clinical data for episodes of care that meets local requirements, are compliant with National standards and meet the requirements for Payment by Results and other processes which rely on high quality coded clinical information.

Main duties of the job
The AMH Clinical Psychology and Psychological Services are seeking to appoint a suitably qualified and experienced 8a (Band 7 development post possible) Clinical Psychologist, to be based and work full time in Wrexham CMHT.
This is a permanent post, and the post holder will work as part of integrated secondary mental health services for adults who have complex and severe mental health needs.
The post holder will be expected to integrate well and co-work with their CMHT colleagues, providing specialist psychological knowledge and expertise to the team. As well as providing direct clinical input to clients, this will also involve working through others by training, supervision, and formulation work. It will also involve attendance at clinical MDT meetings, referral meetings (SPOA), and CMHT business meetings, and strategic and clinical leadership.

We have part time (30 hours 0.8 WTE) Band 7 post available in the Huntingdon and Doddington locality.

Job summary
We are seeking to recruit someone to join our trauma and orthopaedic team in Glangwilli General Hospital, Carmarthen. You will have knowledge in occupational therapy and work as part of a designated team to deliver an occupational therapy service.
Your role will be to support the occupational therapist to deliver what matters to the patient, to enable them to be rehabilitated and discharged home from hospital. You may also at times be required to outreach to peoples homes, when appropriate to support discharge.
You will be part of a supportive team who will provide learning opportunities, formal and informal supervision.

Provide a service to a designated client group, based on instruction & guidance given by the occupational therapist. This will include undertaking specific assessments and interventions independently as part an overall plan of care delegated by the occupational therapist.
Work collaboratively with colleagues from Health, Local Authorities and Third Sector Organisations to achieve best outcomes.
Support team operations, dealing appropriately with enquiries and undertake administrative tasks relevant to the role.
The post holder will be expected to travel within Carmarthenshire.

This has been driven from the Green Paper (2017) to transform Young Peoples Mental Health by focussing on early intervention. Candidates will be offered a place to train at the University of Sheffield, in delivering Low Intensity Cognitive Behaviour Therapy (LI- CBT) evidence-based early interventions, and Whole Setting Approach initiatives (WSA) within education settings.

ABOUT THE TEAM
The ACP team within the Acute Oncology Service (MAOS) review patients presenting to UHS for unscheduled and emergency cancer care 24hrs a day, 7 days a week. The team are also the primary providers of the Cancer Care Emergency Helpline which is fundamental to the delivery of the service. The team work hand-in-hand with the ACPs across the care group to provide a dynamic, ACP-led approach to care delivery.
The Advanced Clinical Practitioner training scheme in cancer care commenced in 2014 and has been recognised for its innovative approach to care, winning a Health Education England Wessex Shine Award in 2015. The Acute Oncology Service also won the Macmillan Service Improvement Excellence Award in 2016 in recognition of its positive impact on the quality of patient care.
WHAT YOU'LL DO
On a day-to-day basis, you'll work within the unit to assess, diagnose, and treat patients who attend following triage by the Cancer Care Emergency Helpline service or by referral from pre-hospital healthcare professionals, primary care or the Emergency Department.
These patients will have a cancer diagnosis and may be experiencing complications of cancer or its treatment. You will handle calls to the Cancer Care Emergency Helpline and will be the principle person responsible for the delivery of this service, ensuring that quality standards are met and that patient safety is optimised.
Additionally, you will provide leadership for the trainee ACPs, guiding and supporting them through their academic pathway and ensuring that evolving service needs are met. As part of the provision of a 24hr service, you will be required to work a mixture of day, weekend (including bank holidays), and night duties.
As a band 8a ACP working in AOS , you will be supported to learn the Senior Clinical Decision Making role where you will develop the skills required to hold the referral bleep for Acute Oncology and provide gatekeeping for patients refereed into the service . This will be alongside the lead ACPs for the service and a team of senior and junior doctors.
If recruited as a band 7 trainee ACP working in AOS, you will be expected to undertake any outstanding modules in the MSc Advanced Clinical Practice and will be supported with clinical and academic development. This will be a developmental role, with progression to band 8a following successful completion of MSc and Advanced Practice panel review.

Main duties of the job
- To practise paediatric occupational therapy at a very competent level across a broad range of duties ensuring that competency levels are maintained and improved by continuous professional development.
- As part of a team to provide a Paediatric Occupational Therapy intervention to children and young people from birth to eighteen years, referred to and accepted by the service in line with service delivery
- To undertake all aspects of clinical duties as an autonomous practitioner.
- To work within a variety of community settings e.g. schools and children's centres to provide OT intervention.
- To take the lead on specific aspects of service development
- To lead a team of junior staff in delivering specific aspects of the OT service.
- To provide training to colleagues, parents/carers and the wider workforce on all aspects of OT delivery and intervention.

Main duties of the job
We are looking for registered or soon-to-be registered pharmacy technicians to join our enthusiastic and motivated team based at Southampton General Hospital (SGH). These are progressive band 4 to 5 posts; however, applications from current band 5s are welcome.
This exciting, fully supported rotational post would be ideal for pharmacy technicians soon to qualify and register with the GPhC. You must be a team player, have excellent communication and interpersonal skills, and be keen to develop yourself.
The role will take place in various areas of our versatile pharmacy department of over 300 staff and will include cross-site working with our partner sites, the Royal South Hants Hospital (RSH) and South Central Ambulance Service (SCAS).
Within the pharmacy department, we pride ourselves on the development of our staff. You will be supported to complete the band 4/5 progressive framework by collecting relevant evidence towards a set of core competencies. Once complete and signed off, your post will be upgraded to band 5.

From 6 April 2017, skilled worker applicants, applying for entry clearance into the UK, have had to present a criminal record certificate from each country they have resided continuously or cumulatively for 12 months or more in the past 10 years.
