Job summary
We are seeking an experienced physiotherapist to join our rapidly developing stroke service in Carmarthenshire, supporting acute rehabilitation and early discharge from hospital.
If you have a passion for stroke rehabilitation working across the stroke pathway this could be the opportunity for you. You will be based at Glangwili General Hospital, Carmarthen supporting people who have predominantly experienced a stroke to return home from our acute stroke units.
The role will aim to deliver rehabilitation in hospital and at home in the local community to support transition from hospital and return to independence. To deliver a person-centred rehabilitation and transition pathway the post holder will work alongside the established acute and community stroke teams (in-reaching and out-reaching) to ensure good continuity of care.
We are advertising for 1 post holder - based in Glangwili General Hospital, Carmarthen.

Main duties of the job
Successful applicants will have experience of working with people who have experienced stroke and/or other life changing events and enjoy working as part of a multi professional team within the acute and community setting. You will work alongside acute and community colleagues and services to create a joined up rehabilitation pathway and positive experience for our population.
Whilst the role focusses on stroke, we require therapists who are adaptable to meet needs of patients with multiple and a variety of conditions as well as service priorities. Due to the nature of the role you will need to be able to travel within the County, as well as being flexible to travel across the Health Board on occasions.
The post holder will be required to participate in the provision of on call respiratory service and future developments in 7 day working.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Main duties of the job
The Patient Flow Coordinator plays a vital role in the Operations Delivery Centre, requiring prior experience in emergency services (or equivalent), ideally within a control room (or call centre) environment.
The primary responsibility is to optimise the flow of operational ambulance resources by analysing data from the Hospital Handover and Fleet Barriers Dashboards along with other performance metrics, while maintaining accurate handover data quality.
Key duties include real-time communication with ambulance crews via radio and telephone, interpreting the Resource Availability Dashboard, and working collaboratively to remove barriers to patient care. The role supports the principle that no patient should wait unnecessarily for an ambulance and contributes to efficient hospital handovers and movement of our fleet of ambulances.
Reporting to the Service Delivery Officer, the coordinator works closely with HALOs, Operations Offers, Community Response Coordinators, and EOC staff and many of our partner agencies.

We are seeking an experienced and highly motivated Senior Chief Cardiac Physiologist to provide professional leadership across the cardiac physiology service. We are looking for a dynamic, compassionate and motivated Band 6 Occupational Therapist to join our Integrated Front Door Team, an innovative service at the forefront of improving urgent care pathways, supporting safe patient transitions and reducing system pressures.
This role provides an exciting opportunity to support safe, timely and effective patient care at the front door, with a focus on preventing unnecessary hospital admissions, reducing length of stay and promoting proactive, person-centred care through early therapy assessment and intervention. The post holder will undertake holistic therapy assessments for patients presenting at the Integrated Front Door, identifying functional ability, rehabilitation potential, risks and immediate support needs. They will provide timely, evidence-based therapy intervention to support admission avoidance, early discharge planning and safe transfer onto appropriate community pathways, working collaboratively with the multidisciplinary team to support clinical decision-making, patient flow and safe, person-centred care. The role will include identifying and facilitating referrals to appropriate onward services, including Urgent Community Response, Virtual Ward, reablement, rehabilitation pathways, community therapy services, social care and voluntary sector support where appropriate. Main duties of the job
To produce high quality diagnostic images and provide high quality patient care to the service users adhering to national guidelines and taking into account relevant legislation including, but not limited to, the Ionising Radiation (Medical Exposure) Regulations (IR(ME)R) 2018 and the Ionising Radiation Regulations 2017 (IRR17).
The post holder will participate in all areas of the clinical rota to include dental, theatre, accident and emergency, orthopaedics, endoscopy, mobile procedures, paediatrics, CT and fluoroscopy and work independently when required. There will be a requirement to work outside of normal working hours, where you will be working unsupervised in a multi disciplinary role as part of the decision process in patient management (A&E red dot).

What would a working day look like?
You're working early shifts this week and you've been tasked with moving medical equipment from the third floor of the Abbey Wing to the Day Surgery Unit on the ground floor. The equipment is very valuable and delicate, so you use your skills and expertise to move it.
Then you're off to Fountains Ward to take a patient to the x-ray department. You wait until the x-ray is taken and then bring them back to their ward.
The rest of your shift is spent moving patients around the hospital and delivering stocks of emergency drugs to the right wards.
You are an essential part of the hospital team, working with so many different patients and members of staff.
What skills do you need for this role?
The postholder must be able to move heavy items easily and push them considerable distances. They must have good people skills and be able to work a number of shifts to meet the service's needs.

The REACT team is a multi-disciplinary integrated community service within ESNEFT incorporating OPAT (Outpatient Parenteral Antibiotics Therapy), OOHs (Out of Hours) and Virtual Ward pathways patients.

The role involves travelling throughout East Suffolk delivering acute care in patient's home as a part of REACT nursing team, both during core hours and out of hours. The post holder will be working in an internal rotation. The successful candidate will be expected to work a mixture of shifts that include the following: 11-11 triage shifts, early (0800-1600), late (1500-2300), twilight (1800-2300) and night shifts (2230-0830) including weekends and bank holidays as the service requires. During out of hours between 18:00-8:00 the Nurse will be accompanied with a Health Care Assistant. Pool cars are accessible for staff to use and it is essential that post holder has valid driving licence.
Main duties of the job
In this role, during the core hours, teams' main responsibilities include working in partnership with the OPAT (Outpatient Parental Antibiotic Therapy) to administer intravenous antibiotics via PICC line or cannula for patients receiving antibiotics in community. Core hours will also include assessment and delivery of care to patients in crisis to allow admission prevention. During the out hours main responsibilities include routine and emergency response to wound care, catheter care for all types of catheters, diabetic care, End of Life care, Anticoagulant medication, Pressure Area care, Venepuncture and Sub cut Fluids in order to ensure 24 hours Nursing care cover in community. You would be liaising with District nursing service to ensure continuity of care and another community services where needed.
The role will also include being the shifts team lead, triaging and co-ordinating case load, scheduling visits, carrying out appraisals and clinical supervision for more junior staff. Part of this role is also to support senior management in daily running of the team and promoting change. The successful applicant should be motivated, flexible and committed to working in a changing environment.

We have an exciting opportunity to join our established team of pharmacists in post that offers structured progression from a Band 6 to Band 7 position within an 18-24 month period.
Newly or recently qualified pharmacists will begin at a Band 6 level which will provide you with a solid foundation in all the essential areas of hospital pharmacy. We will support you to gain funding for a clinical diploma and help you to build a portfolio aligned to the RPS Post Registration Core Competency Framework. On completion of well-defined developmental gateways and submission of a portfolio of evidence, you will progress to a Band 7 pharmacist.
During your foundation rotations, you will have the opportunity to work with our external partners including Avon and Wiltshire Mental Health Partnership (AWP) and later, progressing to more specialist rotations such as in critical care, paediatrics, local neonatal unit (LNU) and HIV medicines supply.

We are seeking an experienced Deputy Director - Commissioning PMO to provide senior leadership for the development and delivery of the commissioning cycle across Kent and Medway ICB.

Our Theatre Store Keeper / Senior Theatre Healthcare Support Workers form an essential and well-respected part of our healthcare teams across a number of dynamic settings within our theatres teams.
Previous experience in a care environment is essential and English & Mathematics at GSCE grade A-C (9-4) or level 2 functional skills are the qualifications required. If you do not have a full practice-based level 3 qualification or equivalent experience, you will be required to complete this as part of your role. You will quickly become a valued member of the team through our comprehensive training and induction programme and our commitment to learning and development will satisfy even the most ambitious person.
The successful candidate will be as part of the team within the PEOC theatres with the flexibility to support the other theatre departments if required.
The team
Princess Elizabeth Orthopaedic Centre (PEOC) Theatres is part of the Critical Care Cluster within the Surgical Services Division. We have five operating theatres two of which have laminar flow facilities.
The department caters for the following elective orthopaedic specialities:

Our department is served by the PEOC Recovery which provides patients with immediate post-operative care following general, regional and/or local anaesthetic.

Our Theatre Store Keeper / Senior Theatre Healthcare Support Workers form an essential and well-respected part of our healthcare teams across a number of dynamic settings within our theatres teams.
Previous experience in a care environment is essential and English & Mathematics at GSCE grade A-C (9-4) or level 2 functional skills are the qualifications required. Princess Elizabeth Orthopaedic Centre (PEOC) Theatres is part of the Critical Care Cluster within the Surgical Services Division. We have five operating theatres two of which have laminar flow facilities.
