Job summary
The Community Equipment Store and Wheelchair Service is a frontline team that provides both a clinical service for wheelchairs and supplies equipment to patients in the community who need special aids to assist and meet daily living needs. The role involves picking and selecting items from warehouse and driving to deliver then fit in location in the community, in individuals homes.
Main duties of the job
To work well both independently and as a team member. To provide timely installation and collection of community equipment and wheelchairs, providing a high-quality rapid service for patients. To be flexible and adaptable, carrying out tasks that change dependant on service needs, keeping the client at the centre of this episode of care. Further tasks will include liaising with clinicians, therapists, carers and other services.

Take responsibility on a daily basis on arrival, to be fully informed of the requirements of the ambulance crew regarding the transport and transfers of patients that may include: Home to Hospice, Hospital to Hospice, Hospice to Home as well as appointments and transfers that are requested from St Johns Hospice through the Hospice Inpatient Unit Be available throughout the day to respond promptly to both planned and unplanned patient transport and transfer requests in liaison with the Hospice Day Care Unit Manager, Hospice Day Care Unit Deputy and Specialist Nurse for Referrals and Discharge Take responsibility where identified for contacting patients or carers, as appropriate, to inform them of pick up times and any potential delays in picking up a patient prior to starting out and while en route, as required. This may include Covid screening questions by phone prior to collection Ensure there is clear, proper, prompt and safe communication with the Hospice Day Care Unit Team at all times in transit or on-site within the hospice environment e.g., by mobile phone Liaise closely with the hospice clinical team in the event of any significant changes in the patients condition or changes to plans for the transfer of a patient. Hywel Dda Health Board present an exciting opportunity, for a motivated, passionate and experienced person. They will join our multi-disciplinary front of house team, TOCALS (Transfer of Care Advice and Liaison Service), as a Clinical Lead Physiotherapist. TOCALS support a large variety of patients, presentations and pathologies, including those experiencing falls and frailty, neurological conditions, cardio respiratory conditions and trauma and orthopaedic problems (including major trauma patients).
TOCALS support prevention of unnecessary admission into an acute hospital to avoid the risks associated with this. The aim is to achieve high quality person centred care which considers the patient holistically, promoting their independence and wellbeing. TOCALS play a pivotal role in promoting excellent links between acute, intermediate, community and third sector services; this is vital to ensure continuity of care, and promote the right care, in the right place, at the first time approach.
Main duties of the job
The successful candidate will require extensive post graduate experience across a broad array of clinical areas, which they can apply to the diverse populations who present to front of house services. They will need to be able to provide highly specialist assessments, with a focus upon discharge planning, from which they can develop and implement co-produced, personalised high quality plans for patients. The post holder will play a key role in following the patients' journeys through hospital, and out into the community.
The post holder will be able to evidence a passion for high quality care and experience of service improvement, particularly where this links with national drivers. Strong leadership, organisational and team management skill are essential, with the need to lead a team in pressurised and fast pace environments. Continuous personal and professional development is essential to ensure TOCALS is responsive to current evidence based practice and population needs. The Clinical Lead Physiotherapist will support leading an in-service training programme for the team, promoting the team to be reflective life-long learners. They will also support staff supervision and annual reviews. Supporting students is key to supporting development of the future Physiotherapy workforce, and the successful applicant will undertake Clinical Supervisor duties.

An exciting opportunity has arisen within the Therapy Team at Hillingdon Hospital for a part time band 6.
We are seeking a band 6 Occupational Therapist to join our friendly, integrated inpatient therapy department. The therapy department is the third largest staff group within the Trust and is widely respected. As a Occupational Therapist, you will play an essential part in the assessment, treatment, and discharge of patients. Main duties of the job

To complete the OT process on the Acute Medical Unit, with a complex and varied caseload. You maybe asked to work else where within the Trust and be part of the 7 day service that is in development.

This is an exciting opportunity for a newly qualified Occupational Therapist or an established 5 wanting to gain physical health, inpatient experience, to join our friendly and supportive team at Hillingdon Hospital NHS Trust. The post holder will complete a wide variety of acute and rehab physical rotation alongside preceptorship.
To facilitate your successful transition from student to qualified OT band 5s are supported by senior therapists, and peer support, alongside band 5 meetings and in service training and team meetings. This is a great place to start your career, many of our 6s and 7s have successfully progressed from band 5s within our Trust.
Rotations cover the following specialities:
- Frailty

The department has moved to 7 day service, as a result several of the rotations will involve weekend shifts. This is on a rota basis.
Main duties of the job
As part of the multidisciplinary team (MDT), assess the suitability of patients for provision of OT interventions following a referral.

Job summary
We are seeking an enthusiastic and motivated Highly Specialist Speech and Language Therapist to join our large, friendly and innovative paediatric Speech and Language Therapy team. We can offer the opportunity to work alongside a diverse range of skilled clinicians and support staff across a well-established service. In this role, you will oversee our Clinic and Designated Provision (DP) services, managing day-to-day queries and supporting Team Leads with service targets, planning and ongoing service development. You will provide highly specialist assessment and intervention for children with a range of speech, language and communication needs and provide clinical support to your colleagues. You will work alongside other Highly Specialist SLTs and benefit from regular support, supervision, training and flexible working.
Main duties of the job
The postholder will manage day-to-day clinical and operational queries, offer specialist advice and support to SLTs and SLTAs, and promote consistent, high-quality practice. Working closely with Team Leads, they will support caseload and pathway planning, monitor performance against service targets, and contribute to clinical governance and quality improvement. The role will involve leading service development across both areas of the service. The postholder will also provide highly specialist assessment and evidence-based intervention for children accessing the Clinic and Designated Provision services. The role requires excellent clinical reasoning, communication, organisation and leadership skills, alongside the ability to make independent decisions within the scope of the role and recognise when issues should be escalated to Team Leads.

Job summary
We are seeking a motivated, hard-working, professional, and passionate Physiologist to join our Cardio-Respiratory Department.
This is an exciting opportunity to become part of our friendly, supportive team, working across both Warrington and Halton Hospital sites. Our department undertakes a wide range of respiratory investigations, and we are looking for an individual who can work independently and contribute confidently to our service.
The successful candidate will be competent in all aspects of respiratory diagnostics, including:
- Full lung function testing
- Overnight polygraphy
- CPAP initiation and follow-up
We are looking for someone with strong clinical skills, excellent communication abilities, and a genuine enthusiasm for delivering high-quality patient care.
Main duties of the job
- To be competent in all routine investigations in the department including Spirometry +/- reversibility, respiratory muscle strength, full lung function test using nitrogen washout method and/or body box.
- Reviewing and signing off spirometry performed in community clinics and ensuring reports are sent out within deadline targets.
- To undertake fitting and analysis of limited channel sleep studies and overnight oximetry.
- Analyse and produce complex reports on limited channel and respiratory sleep studies to ARTP standards.
- To support the services as they evolve and change for the benefit of our patients.
- Provide CPAP treatment and change prescriptions as required.
- Provide administrative support as required.

Job summary
Day to day management of the cashiering function.
To ensure that all cash and bank transactions have been accounted for appropriately in line with accounting principles.
To ensure that the monthly timetable is adhered to.

Main duties of the job
- To support the Head of Treasury with the efficient running of the Treasury function of both cash offices in Wexham Park and Frimley Park Hospitals.
- To ensure that the bank account reconciliations are performed to 100% accuracy to train and support staff in this process.
- Ensuring that there is sufficient cross cover within the team to enable key priorities and deadlines to be met.
- To assist the Head of Treasury as and when required with cash flow reporting.
- To ensure that all information and accounting records are available for review by both Internal and External Audit.
