An exciting opportunity has arisen for a Occupational Therapist to be part of the Home First service in Staffordshire.
This is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs within Midlands Partnership University NHS Foundation Trust (MPFT).
The Home First Team prides themselves on the delivery of quality care to community patients, many with complex needs.
The team also work to facilitate safe hospital discharges for patients leaving acute care and to offer them nursing support in their own home environment.
Thisis a dynamic post which will be evaluated and adjusted to reflect the needs of the service, local population and feedback from users and staff. Your feedback and input will be essential to the development and shaping of the role.
The North Home First covers the whole of North Staffordshire and Stoke-on-Trent providing clinical support for patients in the community to either avoid potential hospital admissions or to support people returning home through the Discharge to Access pathway following a period of acute care.
Main duties of the job
The post holder will be part of the Allied Health Professions (AHP) service providing Occupational Therapy and will work in a variety of settings.
This is a dynamic post that requires the post holder to work closely with uni/multi-disciplinary teams/agencies in addressing a range of care/support issues across the spectrum of conditions, preventing unnecessary hospital admissions and improving the health and wellbeing of the population served. The nature of the service will be determined by local service specifications

Main duties of the job

The Home First Team is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs within Midlands Partnership University NHS Foundation Trust (MPFT).
The service seeks to offer support to people ranging from those that need minimal support for a time limited period to rebuild confidence, learn new skills and relearn skills through to people at the end of their life requiring skilled health & social care interventions. The service operates 7am to 10pm 365 days per year.
The post holder will be responsible for the scheduling and allocating care calls on the Complex Discharge Tracker System. They will be working in partnership with the Home First Locality Teams to ensure that all referrals are allocated care calls within the discharge to assess timescales. They will ensure that the Complex Discharge Tracker system is validated and updated within each locality teams.
The post holder will be expected to commute to the office bases across North Staffordshire, if required.
The shift patterns are on a rotational basis over 7 days, early shift 7am - 3pm and late shift 12pm - 8pm working 2 weekends a month, therefore flexibility is required for this role.

The successful applicant will receive direct line management and will be delegated team work priorities from the Home First Shift Leads. The post holder will be enthusiastic, passionate and a dynamic individual. You will need to be motivated, flexible and an effective team player, in order to prioritise and implement effective negotiation and communication skills.
Review all daily Home First referrals with the Locality Teams and ensure that all patients are allocated the appropriate care calls within the discharge to assess appropriate referral timescale.
Ensure that all Home First referrals from the acute portals are immediately sourced and prioritised throughout the working day.
Attend all CDT training and briefing sessions as the system evolves.
- Ensure that any and all updates recorded in CDT are accurate before entering them. Ensure that any identified mistakes are corrected immediately.

An exciting opportunity has arisen in the Stoke NHS Talking Therapy Service, a Primary Care Psychological Therapy Service in Stoke-on-Trent offering innovative and effective treatments for a broad range of mental health problems in primary care, focusing on anxiety disorders and depression.

Job summary
The Associate Practitioner in Audiology will provide interventions with routine / non-complex hearing pathologies / disabilities. This will involve developing the individual care / management plan with appropriate accountability to the Audiologist. The Associate Audiologist is NOT an independent practitioner, and it is anticipated that some clinical supervision and mentoring will be planned during the normal schedule of the working day. The role may involve some administrative duties subject to the service needs.
Main duties of the job
Provide and assist with basic adult hearing aid rehabilitation and to (under indirect supervision) deliver the patient individual management plan.
Expected to perform routine audiological investigations such as Puretone audiometry for hearing aid re-assessments / reviews, tympanometry, aural impressions and programming and evaluation of hearing aid systems which would include real ear measurements under mentoring programmes.
May be required to participate and assist with paediatric audiological assessment and carry out school screening when required and provide a domiciliary service for house bound patients.
Always recognise own limits of competence and levels of responsibility and autonomy within the approved scope of practice.
Maintain own personal competence and work as a reflective practitioner to deliver high quality client care.
