Job summary
A opportunity has arisen to join a well-established team within the District Nurse Triage Team at Oxford Health NHS Foundation Trust.
Using sound clinical knowledge, professional judgement and risk assessment skills, clinicians will assess referrals, clearly identify urgent and non-urgent care needs, and coordinate the most appropriate response.
This may include arranging urgent, same-day or routine visits from District Nurse clinicians, signposting patients to alternative services, or providing clinical advice where appropriate.
The team plays a key role in coordinating timely, responsive and patient-centred care, ensuring patients receive the right support in the right place and at the right time within the community setting.
The successful candidate will:o Clinically triage referrals received by telephone and email.o Use clinical knowledge, professional judgement and risk assessment skills to identify the appropriate level of care and urgency.o Arrange and coordinate urgent, same-day and routine visits for patients in the community.o Provide clinical advice and support where a face-to-face visit is not necessary.o Work closely with District Nurse teams and wider community services to ensure safe and effective care coordination.o Contribute to the ongoing development of the District Nurse Triage Desk and the future delivery of community care.
Main duties of the job
The services currently operates 7 days a week, 365 days a year, with core hours of 08.00-18.30.
Shift patterns are 08.00-16.00, 10.30-18.30 or 08.00-18.30.
What will we give you?
Once you have started with Oxford Health you will be on the road to develop and hone your clinical skills. We offer each person a bespoke induction in your team with a period of being supernumerary , which is usually up to 4 weeks. We will ensure you can attend the District Nursing Induction programme, this is tailored to district nursing staff, encompassing as facets of Oxford Health and the lived experience of what you will do, giving you an insight into the organisation. The preceptorship programme is another opportunity for you and you will be able to undertake a service development project. We are Oxford Health pride ourselves in supporting and developing our staff to achieve their potential and to fly in their career.
This role is a Triage based office job and requires to you to be on site.

We have anBand 7 opportunity in our Children and Young Person's team.
In this position, you will work with a broad range of mental health presentations to develop in-depth expertise in young people's mental health and to hone skills for supporting individuals with neurodiversity. Your role is integral to the CAMHS moderate to severe pathway, known as FIRS & SIRS, which provides expert support to young people experiencing moderate to severe mental health conditions through a single point of access. You will be part of a multidisciplinary team collaborating closely with partner organisations to deliver holistic, high-quality mental health services.
We operate a hybrid working model, combining both face-to-face and remote service delivery, with bases across the county, including Aylesbury and High Wycombe.

Main duties of the job
In this role, you'll have the opportunity to really get involved in the Psychology provision offered within Buckinghamshire. On a day-to-day basis, this will include:
- Offering psychology assessments.
- Offering psychology consultation to both CAMHS clinicians (within and external to our team) and Multiagency services (e.g. social care and schools) providing opportunities to engage with other professionals to share and disseminate psychological knowledge/understanding.
- Delivering psychological interventions using a range of models through 1:1 sessions, joint parent and young person work. There are opportunities for Cognitive Behavioural Therapy, DNA-V (adolescent model for Acceptance and Commitment Therapy), Narrative Exposure Therapy (NET) and Compassion Focused Therapy in line with the existing evidence bases
- Delivering clinical supervision and supporting Psychology Trainees (both CBT trainees and trainee Clinical Psychologists).
- To support the duty provision, by undertaking the role of duty on a rota basis as shared out within the team.
The ability to travel independently between sites within the Trust is essential for this role.

Job summary
We are looking for a full-timeForensic Administrator to join our mental health team at HMP Bullingdon near Bicester. You will work within the prison with senior members of the multidisciplinary team to provide direct administrative support. We are looking for an enthusiastic, flexible, empathetic and self-motivated individual to work within our team.
You'll need to have proven administrative experience with a good eye for detail, excellent communication, administrative and organisational skills. Knowledge of Microsoft Office suite, managing diaries and minute taking is essential. You'll be liaising with employees at all levels so you'll need to be comfortable handling contacts with compassion, patience and professionalism.
This role would suit someone who enjoys being busy with a variety of duties, working as part of a team, and using their own initiative. This is an important and rewarding role which helps us run an efficient service across the prison.

Main duties of the job
- Produce correspondence and reports as required for the MDT.

We are looking for a part-timeForensic Administrator to join our team at HMP Huntercombe near Henley working 22.5 hours per week. You will work with senior members of the multidisciplinary team to provide direct administrative support. You'll need to have proven administrative experience with a good eye for detail, excellent communication, administrative and organisational skills. Knowledge of Microsoft Office suite, managing diaries and minute taking is essential. This role would suit someone who enjoys being busy with a variety of duties, working as part of a team, and using their own initiative. This is an important and rewarding role which helps us run an efficient service across the prison.

- Organising and attending meetings, preparing agendas, ensuring all appropriate arrangements have been made prior to meetings, including collation and distribution of any relevant papers, taking minutes, and ensuring their timely
- Ensure electronic diaries/System1 ledgers are efficiently maintained and coordinated for the multidisciplinary
- Receiving complex or sensitive information, and ensuring that the information is shared with clinicians in a timely manner

We are looking for a Mental Health Nurse, Occupational Therapist or Social Worker to join our Older Adults Community Mental Health Team (CMHT) based in the South of Bucks. We are expanding the permanent posts within our duty team and are recruiting into a full time post into this dedicated team, working alongside our Duty team and Clinical Lead Nurses.
We are a friendly, supportive and proactive team with a culture of high standards of care and any successful applicant will need to be committed to maintaining this. The team works closely with a wide range of internal and external partner's including GPs, district nursing, therapy services, ambulatory care, social care and physical and mental health inpatient providers. We aim to deliver well-coordinated patient care and to share our respective skills and knowledge with one another.
Our team covers a wide geographical area bordering Berkshire, Oxfordshire, Slough, Amersham and Marlow and so is ideally placed for staff commuting from Oxford, Milton Keynes, Reading and Aylesbury.
Our main office is in High Wycombe and our inpatient beds are based in Aylesbury.
Core working hours are Monday-Friday 09:00-17:00. Out-of-hours support services are managed via a team rota. Staff will be expected to work on this rota which includes occasional weekend 09:00-17:00 shifts.
Main duties of the job
Working as part of our MDT you will be responsible for supporting the coordination of our duty team, triaging referrals, planning assessments and liaison with the bed flow team, and local Psych wards. You will be expected to respond to urgent and emergency referrals, providing crisis support for people with dementia, and brief interventions to our older adult patients with mental health needs. You will also work closely with the crisis team who provide support for older adults with functional disorders.
The MDT is made up of Medical, RMN's, OT, SW and Psychology colleagues and we highly regard one another's professional background and contribution to the care of our patients. We view supervision as fundamental to the functioning of the team both clinically and in terms of the well being of our staff.
The responsibilities of the role include, but are not limited to:
- Work as an autonomous practitioner as an active participant of the multi-disciplinary team undertaking specialist assessments in collaboration with the patient and their families/carers and presenting back to the wider MDT.
- Work in partnership with service users and their families/carers to ensure the development of a treatment plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community. This may include working closely with partner agencies.

The post holder will be able to:
- Perform venepuncture under supervision of the senior phlebotomy team and managers as designated; after achieving competence post holder will be expected to work alone when and where required.
- Follow the general policies and protocols of the department and, in association with the clinical scientific and technical staff, assist in maintaining the highest professional standards.
Main duties of the job
Post holder will be required to:
- Collect samples from inpatients, patients attending outpatient clinics.Is required to liaise with clinicians, other healthcare professionals including laboratory staff. Post holder will be required to perform a range of venepuncture techniques in a variety of clinical settings.
- Be aware of the clinical conditions and precautions that are necessary for sample quality to be maintained during the venepuncture procedure, and develops an awareness of requirements for site selection, correct use of tourniquet and selection of blood sample bottles.
- Ensure that blood sample bottles are accurately labelled in accordance with departmental policies and procedures.
- Ensure that clinical samples are correctly packaged for transportation to the laboratory in accordance with the policies and procedures of the Pathology Department.
- Seek guidance from experienced Phlebotomists as necessary.
- Be aware of, and works to relevant departmental policies and Standard Operating Procedures, ensuring that personal performance complies with ISO (International standards organisation).
- You will also be required to perform reception and telephone duties and refer more complex enquiries to a senior member of staff.

Main duties of the job

The Transforming and Connecting Care (TCC) programme is a £1Bn major national transformation programme and a key enabler of the NHS 10 Year Health Plan. Job summary
We are seeking an experienced and dedicated Dermatology Specialist Nurse to join our team on a full-time basis. This is an exciting opportunity for a motivated nurse who is passionate about delivering high-quality, specialist care to patients with a wide range of dermatological conditions.
Essential Requirements:
- Registered Nurse with a minimum of 2 years' experience working as a Dermatology Specialist Nurse
- Proven experience in delivering specialist dermatology care
- Strong clinical assessment and decision-making skills
- Excellent communication and patient-centred care skills
Key Responsibilities:
- Independently manage and deliver specialist nurse led dermatology clinics
- Provide phototherapy treatments and support services
- Conduct patch testing clinics
- Monitor patients receiving systemic dermatology medications
- Respond to patient enquiries through the dermatology helpline
- Maintain accurate clinical records and documentation
- Undertake administrative duties essential to the efficient running of the department
- Work collaboratively with the multidisciplinary team to ensure the highest standards of patient care

Main duties of the job
- Phototherapy Clinic: Administer phototherapy treatments (narrowband UVB, PUVA) to patients with conditions such as psoriasis and eczema, while monitoring their response and ensuring safety protocols are followed.
- Patch Testing Clinic: Perform patch testing application and removal ensuring appropriate follow-up and patient education.
- Systemic Drug Monitoring Clinic: Monitor patients on systemic treatments (e.g. biologics, immunosuppressants), coordinating regular blood tests and tracking medication side effects.
- Dermatology Helpline: Provide support and advice to patients via phone, addressing medication concerns, treatment plans, and follow-up queries.
- Administrative Duties: Schedule patient appointments, maintain accurate records, and assist with departmental audits and reporting as needed.

Are you an IHRIM Accredited Clinical Coder or experienced Clinical Coder working within an acute NHS hospital setting?
The Clinical Coding team at our hospital site Hinchingbrooke are looking to recruit to our substantive team of multi skilled specialist Clinical Coders. The ideal candidate will be ACC qualified with experience in coding a broad range of clinical specialties.
Applicants must demonstrate excellent communication skills with the ability to engage with clinicians and all relevant support staff to ensure our coded data reflects accurately the care of our patients. Excellent analytical and judgement skills are essential in this role as is good working knowledge of medical terminology and human anatomy across a range of clinical specialties. You will be responsible for managing your own workload each day within the usual deadlines for submission of data.
We would welcome applications from experienced clinical coders (Band 4) without the IHRIM qualification with the option to study towards the National Clinical Coding Qualification.
Main duties of the job
Excellent analytical and judgement skills are essential as is a good working knowledge of national clinical coding standards, medical terminology and human anatomy.
Translation of clinical information into diagnostic and procedure codes from patient health records scanned on to an electronic document management system and variety of other electronic sources.
Applicants must demonstrate excellent communication skills with the ability to engage with clinicians and all relevant support staff to ensure our coded data reflects accurately the care of our patients.
Conduct validation exercises as required with the responsible Consultant to ensure correct assignment of diagnostic and procedure codes and to identify gaps in clinical documentation to support clinical coding.
