There is no one-size-fits-all when it comes to how we cope and the help we need, but with our support, no one has to face dying or grief alone. We are there when it matters. About us -At the Sue Ryder Duchess of Kent Hospice and community nursing services across Berkshire West, we look after people living with life-limiting conditions and offer support for families too. Sue Ryder offers a range of hospice and community nursing services in Reading, Newbury and Wokingham. About the Role In palliative care no day is the same. Working on our inpatient unit provides a unique opportunity because we care for patients with such a variety of conditions and symptoms. If you would like to develop your knowledge of many different conditions and the impact this has on patients and their families a role in palliative care with Sue Ryder is for you. Our nurses play an indispensable role in providing compassionate, holistic care for patients with life-limiting illnesses. Their work extends far beyond medical tasks, encompassing emotional, psychological, and spiritual support for both patients and their families.
Main duties of the job
The successful applicants will play an active role within our multidisciplinary team. You will work alongside Specialty Doctors, therapy staff, volunteers and other vital members of the MDT taking a holistic approach in providing the best possible outcomes for each patient.Duties include but are not limited to: Providing support, leadership, recognition and supervision to junior members of staff Assess, plan, implement and evaluate individualised clinical care for service users Using evidence-based practice to develop and maintain high-quality care
About you - Sue Ryder prides itself on giving our patients the best possible quality of life by providing a holistic and individualised service. Our ideal candidate will have experience of working in Palliative and End of Life Care. This experience may come from a hospice or alternatively, for example, from a high dependency unit or oncology service. To become a registered Nurse at Sue Ryder you will need: NMC registration and post-registration experience ideally in acute medical, hospice, or community settings. Passion for palliative and end-of-life care (training provided for nurses from other specialisms). Excellent communication, teamwork, and initiative. Evidence of continued professional development and willingness to learn. There is plenty of scope to further develop your own skills in specialist palliative care, and planned career progression through our education programme and development framework is encouraged.

At Care in Mind our homes are always a hub of activity and energy where the young people can feel safe, and our staff enjoy coming to work. Our young people can still be quite active at night so you will have lots of interaction and will be required to support them their wellbeing, education, household tasks and general life skills. There will also be more challenging aspects to the role such as incident management and de-escalation of challenging behaviours. You will be required to update online care records, complete medication administration and provide effective handovers to the day staff. In addition, you will be responsible for the general upkeep and tidiness of the home ensuring the young people live in homely environment. Even though you are working nights, you will have the support of another night worker or a sleep in and have access to an on-call manager for additional support.
At our home in Stockport, you will be working with young people who have a variety of mental health diagnosis including personality disorder, those who have difficult relationships with food and those who use self-harm as a strategy to cope with their emotions. As such, you will be required to be patient and resilient as the support you will be providing to our young people will include supporting them to improve their relationship with food and managing incidents of self-harm. A number of our young people are preparing for moving on positively from the home into the community, therefore you will be supporting this transition through things such as: supporting them to self-administer their own medication, getting back into education, accessing public transport and appointments independently. Job summary
We know that service users recover quickly at home, and you will be an integral component for ensuring that this is done in a safe and specific way as an alternative to hospital admission, working alongside the wider MDT.
As a Band 6 Nurse in the CRHT, you will be responsible for providing urgent and emergency triage, advice and assessment for people in a mental health crisis. You will also be responsible for undertaking home treatment visits as indicated by the MDT. This will include the provision and delivery of highly skilled nursing care, including care planning and care delivery, biopsychosocial interventions, clinical risk assessment and management, and promoting recovery and inclusion.
You will be required to work autonomously, understand capacity and flow of the service and the organisation, attend MDT meetings, undertake gatekeeping assessments for inpatient admissions and crisis house admissions, work collaborative with the crisis house to lead on care for admissions as a service and facilitate early discharge from the inpatient wards where clinically indicated. As a Band 6 Nurse in the CRHT, you will also provide clinical leadership and supervision for junior staff.
Main duties of the job
To provide a clinical service that is service user focussed, accessible and responsive on 24-hour, 7-day a week basis.
To triage all mental health referrals into the Team to determine the priority for service response and the appropriate clinical pathway.
To provide and deliver highly skilled nursing care, including full mental health assessment, care planning and care delivery, biopsychosocial interventions, clinical risk assessment and management, and promoting recovery and inclusion.
To recommend and refer, where treatment is indicated, to the most appropriate team/service.
To undertake gatekeeping assessments for inpatient admissions and crisis house admissions.
To work collaboratively with the crisis house to lead on care for admissions as a service.
To facilitate early discharge from the inpatient wards where clinically indicated.
To deliver home based treatment interventions for all clients, ensuring best practice is implemented.
Where appropriate to participate in Mental Health Act assessments so that alternatives to formal hospital admission are considered.

The Service Manager will provide operational, performance and business management support to Respiratory Medicine at Queen Elizabeth Hospital, working closely with the Operations Manager and General Manager to deliver service, quality, workforce and financial objectives.

Main duties of the job
The successful candidate would provide highly specialist input and leadership to the Medicine, HCOP and Respiratory teams, alongside a Highly specialist SLT within the same team.
You would work closely with your peer Highly Specialist SLT, Specialist Speech and Language Therapists and NQP Speech and Language Therapists and assistants to provide an excellent level of care to our patient group.
The successful candidate will have a clear passion in assessing and managing dysphagia and communication difficulties relating to the acute medicine and HCOP caseload in a fast-paced environment.

Main duties of the job
We are currently developing a Universal, Targeted Specialist model in mainstream schools, delivering EHCP intervention in mainstream schools, as well as expanding our buy-in service in mainstream schools, special schools and provisions.
Day-to-day, you will provide assessment and therapy services to a mixed paediatric caseload with speech, language and communication needs (SLCN), as well as report writing and opportunities for training parents/carers and those working with children with SLCN. - Carrying out detailed and timely appraisals in keeping with KPI's
- Co-lead on the coordination of all discharges from the Home Pathway Team.

We are offering an exciting opportunity for an enthusiastic dietitian to join our growing Hepato-Pancreato-Biliary (HPB) Surgery Service on a 12-month fixed-term basis. This post is based at Queen Elizabeth Hospital Birmingham.
You will be working alongside our specialist Band 7 HPB Dietitian, playing a pivotal role in delivering and developing a new dietetic service for patients undergoing complex cancer surgery and treatment. You will be working in both the inpatient and outpatient setting, supporting patients to optimise their nutritional status before surgery, enhance their post-operative recovery and manage the nutritional consequences of cancer and treatment throughout their treatment journey.
This is an excellent opportunity for a motivated and proactive dietitian to gain specialist experience in HPB surgery, cancer care and service development. You will be joining the service at an exciting stage of growth and will be required to make a real contribution to its development and sustainability, improving outcomes for patients undergoing complex cancer treatment and demonstrating this though service evaluation and audit.

Main duties of the job
As a key member of the HPB multidisciplinary team you will:
-Manage a specialist caseload of HPB surgical and oncology patients and deliver regular outpatient clinics and inpatient assessments.
