Main duties of the job

You will work in a nurturing environment with our specialised support team at Chalgrove care & nursing home. This is a permanent position with a salary of £12.72 per hour. Main duties of the job
As a Pharmacy Technician, your responsibilities will vary. You will
- Provide outstanding quality based patient centred care to our patients seeking out, listening to and acting on their feedback so that care is personalised and informed by what matters to them.
- Focussing on providing a safe and secure medicines management service, ensuring patient medical records are maintained to the agreed standard at all times and ensuring standard operating procedures are followed to a high and orderly standard.
- Deliver non-judgemental care within the scope of professional standards. Involving anything from the timely administration of medicines through to analysing cost effective prescribing as defined through our unique health in justice medicines policy.
- You will make a real difference by working as a key member of a multidisciplinary team of healthcare professionals.

Free parking, low turnover of staff, wellbeing days, strong relationships and partnerships. Working in a busy team of 6, who are a close knit and supportive team, with great team work.Hours
37.5 hours a week working 07:30 - 17:30 three days a week and 07:30 - 17:00 the other day a week. Shifts spread across 7 days. Includes 2 weekends a month and bank holiday working.
Salary
This position is full time, youll receive an annual salary up to £41,658 per annum FTE - depending on experience.

The post is based with the Hambleton and Richmondshire Memory Service a service for the assessment and management of cognitive disorders (working age adults and Older Adults)
You will work within a Community Mental Health Team for older people assessing and treating Functional and Organic Mental Disorders.
Work will involve attendance at Out Patient Clinics and home visits as directed by the supervising consultant.
Main duties of the job
Assess and manage common psychiatric emergencies and conditions - including competent dementia assessment and perform suicide risk assessment.
Be aware of the procedures for assessment and implementation of detaining /admitting patients under the Mental Health Act.
Make appropriate referral to other professionals.
Work with colleagues within the multi-disciplinary setting
Participate in the resident doctor on-call rota

Job summary
Joining our Clinical Pharmacy Team across General Practices and Care Homes in Surrey Heath and playing an important role complementing the clinical pharmacists and other members of the multi-disciplinary team (MDT). Working under supervision of a clinical pharmacist, undertaking resident/patient-facing activities and supporting the team of clinical pharmacists to ensure effective and efficient use of medicines within care homes and general practice, through shared decision-making conversations with residents/patients.
Initially focusing on supporting the Practice(s) and Primary Care Network (PCN) implement the medicines optimisation requirements of the new Network Contract Direct Enhanced Services and other Practice and PCN responsibilities. As our plans for the Pharmacy Technician role mature, the focus of the role will expand accordingly to ensure effective support to the clinical pharmacy team and to meet the needs of the practice(s) and the population served.
A key focus will be supporting the GPs and clinical pharmacists in implementing effective medicine management in care homes and across the practices population, and identifying areas for improvement and initiating and managing change.
Main duties of the job
- Function as part of the general practice and PCN team and working across the practices and care homes in Surrey Heath, developing professional supportive relationships.
- Carry out medicines optimisation tasks, to improve the quality and cost effectiveness of prescribing, and patient adherence and administration.
- Undertake tasks to support structured medication reviews/medication reviews.
- Build and run EMIS reports to identify/stratify patients/residents for review.
- Undertake medicines reconciliation, with support of other clinical staff review medication changes and communicate change appropriately, including for new care home residents / patients and post hospital discharge / out-patients.
- Synchronising medicines for patient transfer between care settings, linking with local community pharmacists, and referring to the clinical pharmacist for structured medication reviews (SMRs).
- Use consultation skills, where required, to work in partnership with patients/residents to ensure they use their medicines effectively (e.g. checking inhaler technique).
- Where competent, provide specialist expertise to address both the public health needs and social care needs of residents/patients, including providing lifestyle advice, service information, and helping tacking health inequalities.

We have an exciting opportunity for a dynamic and motivated Band 6 Senior Radiographer to join The Imaging Department at Harefield Hospital, part of Guy's and St Thomas' Foundation Trust.
The post holder will be required to rotate through General x-ray, Fluoroscopy, Theatre, CT and the Catheter Labatories. Participating in the delivery of a 24/7 service, wherein radiographers are contracted to work out of hours shifts at night, on weekends and bank holidays.

Main duties of the job

About us
Harefield Hospital, part of Guy's & St Thomas' NHS trust is a world-renowned specialist centre for heart and lung care. Famous for pioneering organ transplants and housing one of Europe's largest dedicated heart attack centre. The hospital has 168 beds, more than 1,300 staff, five operating theatres and four catheter laboratories.
The Four catheter laboratories have dedicated angiography equipment, a Toshiba Infinix, Siemens Q.zen, Philips Allura ( about to be replaced with a Philips Azurion) and a Philips Allura FD10/10 Bi-plane. Work in the department mainly relates to cardiothoracic imaging.
Plain Film general work is also undertaken using a Philips DR unit and a Philips fluoroscopy room. Mobile x-ray units for use in the intensive care, recovery units and other wards. There is a mobile image intensifier mainly used for thoracic theatre cases.
There are 2 GE CT scanners, and 2 Siemens MRI scanners, an ultrasound and nuclear medicine service and a Dexa Scanner.
The Imaging Department at Harefield Hospital is comprised of a team of Radiographers, Imaging Support Staff, Admin Staff and Radiologists.
As well as working within the Imaging Department, Radiographers rotate through the Catheter Laboratories working in the larger multidisciplinary workforce. At our dedicated heart attack centre, we deal with heart attack emergencies from outer north-west London and further afield, providing primary angioplasty in our specialist catheter laboratories which operate 24/7.

BERSHAM WARD - ACUTE STROKE UNIT
An opportunity has arisen for Band 2 Health Care Support within the Medical Directorate at Wrexham Maelor Hospital.
The successful candidates will work within acute adult wards within the Medical Directorate caring for patients admitted into hospital.
Applicants must have an approachable manner necessary to deal with patients and their relatives and desirable experience of a hospital environment. Applicants will work as part of a team of nurses on the ward and undertake patient fundamentals of nursing care tasks under the direct or indirect supervision of a Registered General Nurse responsible for the patient's management.

This is a post in primary care within the community mental health team in Conwy
We provide assessments from numerous agencies who refer through our SPOAA daily meetings, and offer brief interventions within groups and occasionally on a one to one basis.
Also a good knowledge of third sector agencies is useful as we work hard to become more familiar and signpost to these very helpful agencies, moving on in improving our contact with them and continually improving our working relationships.
Main duties of the job
An exciting opportunity has arisen in Primary Mental Health Support services for an experienced, motivated and forward thinking practitioner to join a busy supportive team based in the Conwy area.A position have arisen in Conwy Community Mental Health team.
The successful candidate whilst working closely with a multi - disciplinary team, will receive referrals directly from GP's and provide short term interventions for individuals with mild to moderate illness and will work closely with colleagues from secondary care and specialist services as well as third sector organizations and statutory agencies.
The successful candidates will manage a caseload of patients with whom they will work alongside to formulate risk management plans and care planning based on robust assessment of need. We are looking for a good team player who has excellent communication skills and the ability to prioritize and manage their time effectively.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
