About us
This innovative service provides responsive out-of-hours clinical cover for adult inpatients residing in Worcestershire community hospitals and other local inpatient settings.
Working alongside multidisciplinary teams, you will support timely clinical decision-making, assessment, prescribing, treatment planning, escalation decisions, end-of-life care and safe transfers of care during evenings, weekends and bank holidays. Your contribution will help ensure patients continue to receive safe, effective and person-centred care whenever they need it.
This role offers the opportunity to work with a wide range of patients and clinical presentations across multiple community hospital sites, whilst maintaining a high degree of professional autonomy and clinical responsibility.
Base/Location

Job responsibilities
Working as an autonomous clinician, you will provide expert assessment, clinical decision-making, treatment planning and, where qualified, prescribing support for adult inpatients. You will work closely with nursing teams and wider healthcare professionals to ensure patients receive timely, safe and effective care during evenings, weekends and bank holidays.
The role offers a varied and rewarding caseload, supporting patients across rehabilitation, intermediate care, frailty, sub-acute and palliative care pathways. You will be trusted to make complex clinical decisions, identify deterioration, coordinate escalation where required and contribute to positive patient outcomes across multiple community hospital settings.
Key areas of responsibility include:
1. Clinical Triage, Assessment and Response
Providing a responsive clinical service, including telephone advice, remote clinical review and face-to-face patient assessment in line with agreed service response standards. You will assess patients presenting with acute illness, deterioration or clinical concerns, ensuring timely intervention and appropriate escalation where required.
2. Autonomous Clinical Practice
Undertaking comprehensive clinical assessments, formulating management plans and making evidence-based decisions within your scope of practice. You will work independently whilst recognising when patients require input from senior clinicians, GPs or emergency services.
3. Prescribing, Medicines Optimisation and Treatment Review
Where appropriately qualified, you will provide independent prescribing, medication review and medicines optimisation support. You will help ensure safe prescribing practices, antimicrobial stewardship and effective use of electronic prescribing systems to support high-quality patient care.
4. VTE Assessment and Admission Support
Supporting admission processes through clinical documentation, risk assessment and identification of key patient risks. This includes VTE assessment and ensuring important clinical information is communicated effectively to the wider clinical team.
5. End of Life Care and Escalation Planning
Providing compassionate clinical support for patients approaching the end of life, working alongside nursing teams, specialist palliative care services and families. You will contribute to escalation planning and ensure patients' wishes and treatment preferences are recognised and respected.

Main Duties:
o To provide autonomous diabetes care for patients with Diabetes using advanced clinical skills and knowledge of diabetes in the community and acute setting.
o To lead and autonomously manage a caseload of patients with Diabetes, who have multiple and complex needs including complex Type 1 and 2 Diabetes, ante-natal, renal and CGM/pump/HCL therapy within the inpatient and outpatient Diabetes service.
o To take responsibility for own evidence-based practice and implement and add to the body of knowledge that supports expert practice within Diabetes.
o To lead and innovate diabetes related specialist practice, both within the Trust and through local networks.
o To develop diabetes guidelines, protocols and policies for the Trust

About us
The Integrated Diabetes Service is a nurse-led service supported by a multidisciplinary team, including a diabetes consultant, clinical nurse lead, diabetes specialist nurses, dietitian, psychologist, healthcare assistant, and a health and wellbeing coach.
The service delivers specialised care for adults with Type 1 and Type 2 diabetes, including support for pregnancy, renal, and preconception clinics. Care is provided through a fully integrated model, with close collaboration between community and acute teams.
The service adopts a multidisciplinary team (MDT) approach. Clinical activities include assessment, diagnosis, shared decision-making, and the delivery of personalised treatment and management plans, focusing on achieving the three treatment targets and completing the nine key diabetes care processes.
This includes comprehensive assessment of patient history and pathology, management of diabetes-related complications, and review of dietary and lifestyle interventions. The service also provides structured education programmes for individuals newly diagnosed with diabetes.

Job summary
We are looking for a confident occupational therapist to work as part of the adult mental health Occupational Therapy service within BCUHB, delivering client centred and specialist, occupationally focused assessments and interventions to adults with mental illness.
The adult mental health Occupational Therapy service endeavours to support the patient recovery journey across the service pathway from crisis, through to admission and community services, supporting self-management and functional independence. The successful candidate will become part of a proactive, BCUHB wide, mental health Occupational Therapy team who work together to develop clinical skills and effectiveness across the adult mental health pathway. We seek to expand the mental health Occupational Therapy provision across all three Areas of BCUHB by evaluating service provision, developing business cases and running pilot projects. The Occupational Therapy service is committed to lifelong learning and evidenced based practice. All staff receive support and encouragement to utilise continuing professional development (CPD) to maintain a CPD portfolio.
The current vacancy is within both Inpatient services and Community services .
Main duties of the job
The post holder will have relevant experience and knowledge of mental health Occupational Therapy for adults with mental illness, be able to effectively use tailored 1:1 and group interventions, and establish and harness community networks to facilitate the recovery journey.
The post holder will be a team player and be committed to excellence in seeking the most robust models of intervention to enhance occupational participation. They will be responsible for supervision and development of junior staff and be required to take Occupational Therapy students on Practice Education placements.
The post holder will work as an autonomous practitioner following professional codes of conduct and ethics carrying a varied specialist caseload in a specialist area of practice, which will include patients with complex needs.
The post holder will contribute to and lead some areas of service development duties within their department, which can include attending meetings, and leading in audit, evidence based practice research and service evaluation.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Job summary
Provide a comprehensive Nutrition and Dietetic service to the Integrated Diabetes Specialist Service at the Royal Free London NHS Foundation Trust - North Middlesex.
Explore and support the continuation of patient care within available workload capacity. Contribute to the development and delivery of education programmes for Enfield residents, primary care teams, and Trust staff. Work collaboratively with the clinical lead to develop, implement, and regularly update departmental policies, guidelines, and procedures. Lead and actively participate in projects, research, and audits within the wider diabetes service.

About us
The Integrated Diabetes Service is a nurse-led service supported by a multidisciplinary team, including a diabetes consultant, clinical nurse lead, diabetes specialist nurses, dietitian, psychologist, healthcare assistant, and a health and wellbeing coach.
The service delivers specialised care for adults with Type 1 and Type 2 diabetes, including support for pregnancy, renal, and preconception clinics. Care is provided through a fully integrated model, with close collaboration between community and acute teams.
The service adopts a multidisciplinary team (MDT) approach. Clinical activities include assessment, diagnosis, shared decision-making, and the delivery of personalised treatment and management plans, focusing on achieving the three treatment targets and completing the nine key diabetes care processes.
This includes comprehensive assessment of patient history and pathology, management of diabetes-related complications, and review of dietary and lifestyle interventions. The service also provides structured education programmes for individuals newly diagnosed with diabetes.

Using pharmacy IT systems for dispensing, stock control, and medicines management processes.

Providing support to wards using the Trusts ePrescribing system.

Job summary
This is an exciting opportunity for this post holder to join an integrated diabetes service in developing psychological support for Enfield residents living with diabetes and its complications. The post holder will work closely with the Clinical Lead for Diabetes, Medical Consultants, Diabetes Specialist Nurses and other colleagues to determine to how best to provide effective, evidence based psychological support for people with diabetes attending the service. Such input will be aimed at reducing diabetes related distress and disability and promoting wellbeing, adherence to treatment, and positive treatment outcomes in adults with complex diabetes related problems. Input will also support with signposting / referring people with diabetes to other relevant services for example. mental health teams.
Main duties of the job
To be responsible for the provision of a high quality, specialist clinical/counselling psychology service to the Diabetes Centre. This will include providing highly specialist psychological assessment and therapy, offering advice and consultation on psychological aspects of diabetes to other health professionals and supporting clinical team members who provide psychosocial care. Where appropriate, the post holder with exercise autonomous professional responsibility for clinical decisions, working within professional guidelines and Diabetes Centre policies and procedures.
The post holder will be expected to utilise their knowledge of evidence based psychological interventions and research skills to identify and implement audit, research and service development projects within the Diabetes Centre (if appropriate)

Main duties of the job
This role will involve performing a broad spectrum of MRI scanning procedures, including neuro, renal, hepatobiliary and gynaecological imaging. Integral to this are patient safety screening, administration of iv contrast / iv cannulation and maintaining documentation of procedures.
Currently this is mostly clinical work, although a small amount of research-based work is undertaken in the department. Communication and liaising with other staff groups including Radiologists, Nursing staff, admin and clerical.

Job summary
Camden Language and Communication Service (CLCS) are excited to be able to advertise for a Specialist Speech and Language Therapist to join our unique jointly commissioned team of speech and language therapists and specialist language and communication teachers. This post is a full-time position, but we are interested in applications from people interested in a part-time role. We are particularly interested in hearing from candidates who are interested in working at an additional resource base within a mainstream school.
All maintained Camden Primary schools have access to this team who provide support at universal, targeted and specialist levels. Speech and language therapists are sited with their educational colleagues to enable continuity and coordinated support. CLCS is dedicated to developing a sustainable and effective service model that puts effectiveness and impact as the focus for client contact. We support children with Education and Health Care Plans (EHCPs), Exceptional Needs Grants (ENGs) and also those without additional funding.

Main duties of the job
The post holder will be based within Camden Speech and Language Therapy Service with a focus on supporting mainstream school environments at primary level. The majority of the work is based within Camden Language and Communication Service which is a specialist multi-agency service for pupils with a range of spoken language and communication difficulties attending over 40 mainstream primary schools, and a primary Language Resource Base, in Camden. The post holder will work in a multi-agency team comprising of Speech and Language Therapists and Specialist Language and Communication Advisory Teachers. The service provided consists of assessment, diagnosis and intervention for individual children as well as providing a whole school support service to schools themselves.
Many of the children that attend Camden schools arrive with low levels of language and literacy for a host of reasons including EAL, social, emotional and mental health factors, as well as a significant clinical population of children with developmental language disorders, ADHD, Autistic Spectrum Disorders and learning difficulties. We support children with and without Education and Health Care Plans and with a range of SLCN and additional needs.

Job summary
Building on the NHS contribution to the 100,000 Genomes Project, the introduction of a national genomic medicine service has created 7 Genomic Laboratory Hubs (GLHs). The North West GLH will be hosted by Manchester University NHS Foundation Trust.
An exciting opportunity has arisen for an Assistant Genetic Technologist to join the North West GLH. This is an excellent opportunity to work in a laboratory environment and be part of a team delivering specialist diagnostic genomic services.
The post will operate from the Genomic Diagnostics Laboratory (GDL), Manchester University NHS Foundation Trust. The GDL is one of the largest and most comprehensive clinical genomics labs in the UK, consisting of biochemical, cytogenetic and molecular genetics laboratories which together provide diagnostic services to a population of over 5 million people.
The labs employ over 100 staff including clinical scientists, genetic technologists, bioinformaticians and administrative staff. The Genomic Diagnostics Laboratory is a well-established, UKAS-accredited service.
The Genomic Diagnostics Laboratory aims to continue to deliver a patient-focussed service with an international reputation and be a premier supplier of laboratory testing in the NHS. We are implementing ambitious plans for modernising and improving service delivery.
Main duties of the job
As an Assistant Genetic Technologist, you will work in the laboratory on one of our Technical Teams (Pre-analytical, Cell Culture and Karyotyping, Molecular Cytogenetics, Fragment or Sequencing/NGS) under the supervision of a Senior Genetic Technologist performing a range of protocol driven tasks.
You should ideally possess A levels in relevant subject areas or be able to demonstrate equivalent literacy and numeracy skills. Experience in a Genetics laboratory environment is desirable; alternatively a diagnostic laboratory. Computer literacy is also required. You should have good communication skills, be able to work well in a team, remain calm under pressure, and be able to take the initiative and prioritise workload.
This is a highly skilled job for which specific training will be given. You will be fully trained to carry out your duties and you will be appraised on a yearly basis with a view to developing knowledge and skills where appropriate.
The work requires constant attention to detail and good organisational / multi-tasking skills. You will work under supervision. Job summary
The post holder will work alongside the Deputy Chief Pharmacist for Emergency Planning and Business Development to help provide professional and strategic leadership and advice to the Chief Pharmacist, and the Trust, on contingency planning necessary within Pharmacy to handle a range of emergency situations. Main duties of the job
Develop and implement plans within Pharmacy, aligned with Trust Policy, to handle a range of emergency situations including major incidents, pandemics & extremes of weather.
Collect and analyze numerous data sets to support service delivery development.
Accountability for preparing and conducting exercises in the department to ensure staff can carry on functioning in the event of an emergency and are familiar with Trust & Departmental procedures.
Responding to incidents, helping assess the situation and level of response required liaising directly with the Chief Pharmacist, other Deputy Chief Pharmacists as required and / or Trust Site Team

The post will involve working in an established team of Cardio-Respiratory Assistants providing high quality diagnostic tests across the cardiac and respiratory pathways. These tests include, electrocardiography, spirometry, fitting and removal of ambulatory ECG/respiratory devices, as well as general administrative work.

Job summary
An exciting opportunity has arisen for a skilled and motivated Physiotherapist to join the Integrated Reablement Team, incorporating the Urgent Community Response (UCR) Service, Pathway 1 Discharge to Assess (D2A) and Reablement Services across Nottingham City.
Working within a multidisciplinary team of Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists, Nurses, Pharmacy Technicians, Assistant Practitioners, Rehabilitation Support Workers and Admin Staff, you will support patients to maximise independence through rehabilitation and crisis intervention within their own homes. The service aims to prevent unnecessary hospital admissions and facilitate timely, safe hospital discharges.
As part of the UCR service, you will respond to urgent referrals from emergency and out-of-hours services, providing a rapid two-hour response to crises such as falls, functional decline and acute illness. Comprehensive in-house training will be provided, including falls recovery, safe assistance from the floor and enhanced clinical assessment skills.
The successful candidate will have a strong understanding of health and social care systems, excellent assessment, clinical reasoning and intervention skills, and experience supporting adults with complex physical, cognitive and mental health needs. You will plan, deliver and evaluate person-centred, goal-focused rehabilitation programmes and work closely with partner organisations to achieve the best outcomes for patients.

Job summary
Purpose of the role
To provide high-quality, compassionate clinical support to patients within the practice, working closely with the nursing and GP team to deliver a range of clinical procedures, health checks, and health promotion activities. The Healthcare Assistant plays a key role in helping Penrose Health deliver outstanding, patient-first primary care.
Main duties of the job
- Providing face-to-face clinical support to patients within the practice
- Carrying out core HCA clinical duties including blood pressure checks, ECGs, phlebotomy, wound care, weight management, and NHS health checks
- Administering B12, shingles, pneumococcal and flu injections/vaccinations
- Chaperoning patients in support of the nursing and GP team
- Assisting in the assessment and surveillance of patients' health and wellbeing
- Carrying out new patient health checks and other health screening
- Performing urinalysis and preparing specimens
- Undertaking long-term condition reviews where appropriate
- Gathering lifestyle data from patients
- Monitoring and recording vaccine and cold chain storage
- Maintaining accurate, up-to-date clinical records
- Supporting infection control practices and equipment management
- Promoting health and wellbeing, and supporting patient education
- Managing clinical stock control, including clearing and restocking consultation rooms
