We currently have an exciting opportunity for an IP Clinical Pharmacist, to work full time in our partner practice in Hexham. This opportunity is for a highly motivated and focused Clinical Pharmacist, who will be responsible for the smooth running of day-to-day activities within general practice, while playing a vital role in providing pharmaceutical care and advice to patients focusing on best practice and adhering to safety guidelines.
Main duties of the job
As an onsite Clinical Pharmacist, you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians.
You will help resolve medication-related queries, work as part of a multidisciplinary team, improve patient outcomes and support the improvement of access to healthcare.

Job summary
We are looking for an enthusiastic and motivated registered nurse to join our Anticoagulation Service as an Anticoagulation Specialist Nurse. This is an exciting opportunity to deliver high-quality, nurse-led anticoagulation care across hospital and community settings, working autonomously to manage a diverse patient caseload. The successful candidate will play a key role in improving patient access to specialist anticoagulation services while supporting the safe and effective use of anticoagulant medicines.
Main duties of the job
You will lead and deliver anticoagulation clinics, undertaking comprehensive patient assessments, interpreting clinical information and blood test results, and developing evidence-based management plans. The role involves providing expert advice and education to patients, monitoring anticoagulation therapy, identifying and escalating clinical concerns, and working closely with the wider multidisciplinary team to coordinate care. You will also contribute to service development, governance, audit activities and quality improvement initiatives within the service.

The Anticoagulation Specialist Nurse is responsible for delivering safe, effective and patient-centred anticoagulation care through independent clinical practice. Key responsibilities include:

Are you are looking for an autonomous role, where you can continue to help shape the future of our community same day services?
Are you passionate about helping avoid unnecessary hospital admissions and caring for and treating our patients in their place of residence?
This key role within our urgent community response service supports provision of high quality care for people experiencing immediate health and care crisis and needing intervention at home to prevent hospital admission (where safe to do so).
The role provides the successful clinician with an opportunity to work within our community same day care services and help test, inform and finesse their continued development within the community.
Our same day care services currently operate on a shift basis working over seven (7) days a week 8am to 8pm, so flexible working into the evenings, over some weekends and bank holidays will be required. We are open to flexible and part time hours and the post is available part-time (minimum 30 hours per week) or full-time (37.5 hours per week).
(This role is subject to job evaluation and therefore some duties may change).

Main duties of the job
Working with a high degree of autonomy to:
Provide a rapid respond to people in health and social care crisis situations with the aim of enabling individuals to maintain or regain independence and prevent unnecessary hospital admission.
Help play a key role in continuing to help people manage their long-term conditions, making sure they receive the right kind of support to help them remain independent and live longer and linking in with our healthcare and charity based colleagues in the wider community.
Work alongside our Community Nursing and Therapy Teams, South Central Ambulance Service, General Practice and Oxford University Hospital colleagues with the opportunity to work across many facets of Community services.

HMP Eastwood Park is female prison housing up to 440 women who are both sentenced and remanded. The healthcare service is formed through a consortium of agencies, AWP works alongside the main contractor Practice Plus Group.

Looking for a new challenge in mental health?
We are seeking to recruit a full time Activity Coordinator to join one of our Mental Health Wards based at the Warneford Hospital.
Allen Ward is a 18 bedded female acute working age mental health ward, supporting patients who are in an acute crisis of their mental health diagnosis.
The activity co-ordinator post forms part of the multi-disciplinary team, including Nurses, Doctors, Occupational Therapists, Psychologists, Health Care Assistants, Physiotherapist, Physio Technician, and Dietician.
Each of the wards at the Warneford Hospital have an activity co-ordinator post, providing lots of opportunities to link up with others and receive regular peer supervision.
Ward staff are required undertake PEACE training. This 5-day training course covers the curriculum relating to the prevention and application of Restrictive Practice and Interventions as detailed in the Restraint Reduction Network Training Standards 2019. Therefore, applicants need to have a good range of mobility and fitness to learn the physical restraint techniques taught on the course.

An exciting opportunity has arisen for a highly motivated and experienced administrator to join the Single Point of Access service in Oxfordshire.
The Single Point of Access is a telephone triage service which provides a quick and easy way for members of the public, GPs, and other healthcare professionals to make referrals to community health services such as community nursing, community therapy and other community services for people over the ages of 18 and living in Oxfordshire.

SPA is spread over two bases, Banbury, and Abingdon, operating 8am-8pm, 365 day a year on a shift pattern basis. Shift patterns are 08:00-16:00, 10:30-18:30 and 12:00-20:00.

You will work as part of a close-knit team to ensure the patients receive the best possible service. The workload can be busy and changeable, so a can-do attitude and an adaptable approach is required.

The Head of Production will:

An exciting opportunity has arisen within SLAM for a painter/handyman to join the Estates team. The Trust has four main sites and community properties, therefore applicants should be able to arrange their work, parts and materials and carry out tasks in a safe and tidy manner.
To maintain continuity of service there will be times when extra hours, altered work patterns/duties is required. The post-holder must have a good general knowledge of domestic and commercial buildings.
Main duties of the job
- To undertake planned preventative maintenance, reactive maintenance and emergency works as demanded by a Hospital of this type. Use all skills, experience and knowledge ensuring safe working methods to carry out maintenance tasks in the most cost effective way for the Trust
- To understand the workings of buildings, engineering services and systems in NHS/Trust premises. Ensuring that they are maintained to a high standard and that all plant and equipment are utilised in a safe and efficient manner
- To carry out work in a timely manner as requested using the current works requisition system and ensuring that any associated information is recorded.
- To work as part of a departmental team and where necessary support colleagues of all levels and disciplines as required.
- To provide a maintenance service to all Trust buildings, off site properties and be able to work unsupervised.
- To maintain continuity of service there will be times when extra hours, altered work patterns, duties or location are required.
