Main duties of the job

- To be allocated and manage own case load effectively under the supervision of qualified staff.
- To complete therapeutic assessments of Children and Young People using structured observation and assessment tools under the supervision and guidance of the registered professional.
- To assist qualified members of the team in the development of interventions which are based on assessment outcome and the principles of the relevant professional groups.
- Provide both direct and indirect intensive intervention, advice and support. To monitor and review interventions providing timely and concise feedback to qualified members of the team.
- Work alone with clients/Parents/Carers/Families/other agencies as required following appropriate preparation and liaison with qualified members of the relevant team.

The Trust comprises an inner city, culturally and ethnically diverse population with the Learning Disabilities Intensive Support Team covering the Black Country area. The service is provided in a variety of settings, including day services, colleges, respite care settings, clients homes, the assessment and treatment and step down units. Job summary
The Care Navigator Role is a fast paced , demanding role within Primary Care that is constantly developing. You will be responsible for reading patient triages and working closely with the clinical team to signpost and book patients into the correct clinics.
You will work alongside your care navigation team to oversee the smooth running of reception by assisting with all reception duties as well as overseeing absences, leave management, staff development, sorting IT issues as well as driving on Qof and Enhanced services and working closely with the Practice Management to assist in any area that is needed.
Main duties of the job
a. Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition
b. Answer incoming phone calls, transferring calls or dealing with the callers request appropriately
c. Process personal, telephone and e-requests for appointments
a. Action incoming emails and correspondence as necessary
b. Accurately detail information on patients records within the clinical system
- Assist with total triage administration
d. Maintain and monitor the practice appointment system
e. Initiate contact with and respond to requests from patients, team members and external agencies
g. Data entry of new and temporary registrations and relevant patient information as required
h. Direct requests for information, i.e., DSAR, insurance/solicitors letters and DVLA forms to the administrative team
i. Manage all queries as necessary
j. Conduct system searches as requested

To support the practice clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the practices multidisciplinary team.
To act as the central point of contact for patients, be responsible for the distribution of information, messages and enquiries for the clinical team, liaise with multi-disciplinary team members and external agencies such as secondary care and community service providers.

Job summary
Dr Sharma & Rai Practice Group is looking for an experienced Healthcare Assistant (HCA) / Phlebotomist to join our team across GP practice sites in Barking and Dagenham.
This is a full-time, permanent role for 37.5 hours per week. The successful candidate will work across different practice sites according to the rota, but will normally remain at one site for the full working day.

Job summary
We are looking for an enthusiastic nurse to work within the Forensic Directorate. The post holder will be joining for our new physical health team as a Physical health Nurse Band 7. This is an exciting opportunity to work closely with staff and service users to improve the physical health outcomes for our service users within the forensic service. We are looking for someone who is keen to improve physical health in mental health nursing skills, who will be working alongside a group of physical health professionals including dietitian, sports therapist, physical health nurses, physical health engagement practitioner and inpatient multidisciplinary teams. They will be responsible for embedding the physical health care systems inpatient wards, facilitating physical health clinics, working close with primary care services and supporting education and training needs for inpatient staff. The post holder will need to be able to work closely with the inpatient services and manage a caseload of service users & support primary nurses with physical health care planning.
Main duties of the job
The post holder will join a multidisciplinary NHS mental health service dedicated to providing secure services.
There are opportunities to participate in physical health audits and the educational program within the directorate. You will also be required to attend regular clinical supervision with a senior clinician.
The Physical Health nurse will carry out regular reviews on the in patients, supervised by a consultant psychiatrist, and liaise with the multidisciplinary team in the forensic services to deliver effective healthcare for patients. To also attend and contribute to weekly ward rounds, team formulation meetings and physical leads meetings.
The post holder will undertake a key role in the organisation and delivery of physical health care in forensic services an area which is increasingly recognised as essential in delivery of safe and effective mental health services. The post will involve liaising with the GP, Opticians, Dentists and all other local clinics for the service and delivery of effective healthcare for the management of acute and chronic physical conditions.
The Post holder will compliment roles already developed within the service and strengthen the multi professional team. The post holder will also work with occupational therapists, Tissue viability nurses, dietetics and nursing colleagues and liaise as needed with local Acute Hospitals.

An exciting opportunity has arisen for a skilled and enthusiastic Physiotherapist to join the Kidderminster MSK Physiotherapy Outpatient Team at Worcestershire Acute Hospitals NHS Trust.
This is a part time, permanent role (26.5 hours per week).
We are seeking a Physiotherapist with NHS experience in MSK outpatients, who possesses advanced skills in assessing and treating MSK conditions.
The successful candidate will need to be proactive, highly motivated in their own professional development, able to demonstrate strong clinical reasoning, and committed to delivering evidence-based practice.
In this role, you will manage a complex and varied caseload in a busy, friendly department. There will also be opportunities to run group sessions and assist in fracture clinics, providing initial assessments and advice to patients following injury or surgery.
Experience working with trauma or post-operative patients is therefore preferred. The department also enjoys close collaboration with our Extended Scope Practitioner service, which specialises in spinal, knee, shoulder, and pain clinics.
You will be supported in your continuing professional development by experienced and supportive team leaders, and you will be actively encouraged to contribute to the development of both colleagues and the service.

Main duties of the job
Working as an autonomous practitioner, to provide high quality patient focussed care through assessment, diagnosis, development and delivery of individual treatment programmes.

The post will be primarily working on the 9 bedded in-patient unit however, there may be at times a requirement to work within our district nursing team.

Main duties of the job
Assist the staff nurse to deliver a high standard of care to all our patients either on the in-patient unit or in the community.

We are looking for clinically orientated, enthusiastic, highly motivated and adaptable Pharmacists to join our team. We have the opportunity for a full time and part time permanent band 6 rotational pharmacist position available at our Royal Lancaster Infirmary site, located within minutes from the city centre.
Applications are welcomed from trainee pharmacists due to register shortly and pharmacists with experience in other disciplines within pharmacy, with training to support moving sector provided.
The successful candidates will join our ward-based clinical team and have opportunities to develop clinical skills through carefully managed rotations in different areas of inpatient specialities including cardiology, respiratory, orthopaedics, paediatrics, general surgery and acute medicine, as well as department-based specialist roles including aseptic, oncology, homecare and clinical trials.
We encourage all our junior Pharmacists to undertake a clinical diploma and will support them with this.
Main duties of the job
Using our trust-wide Electronic Prescribing and Medicines Administration tool, you will work alongside our technical team to review medicines reconciliations, optimise patient's medications and verify discharge prescriptions.
As part of the team of Pharmacists you will on be the responsible pharmacist in our dispensary, and will progress to taking part in the weekend rota. At this time, band 6 Pharmacists are not required to take part in the on-call rota.

We have an exciting opportunity for a highly motivated Physiotherapist to join our Integrated Community Stroke Therapy Team, based at Furness General Hospital, providing advanced clinical leadership across both inpatient and community settings.

The role sits within the wider Clinical Health Psychology department, which provides services across all Imperial College Healthcare hospital sites -- offering you support from peers and a wide range of CPD opportunities offered by CNWL Foundation Trust.
You will receive supervision from a Consultant Clinical Psychologist who also supervises sex therapists in training. This provides a rich environment for ongoing development, reflective practice, and specialist skill-building.

Pharmacy Technician Job Description
Theale Medical Centre is looking for an enthusiastic, organised and motivated Pharmacy Technician to join our multidisciplinary practice team.
The Pharmacy Technician will play an important role in supporting the safe and effective use of medicines across our patient population. Working alongside our Clinical Pharmacists, GPs, nurses and wider multidisciplinary team, the role will support medicines optimisation, medication monitoring, clinical administration and quality improvement.
This is a varied role with opportunities to develop your skills and take responsibility for specific areas of medicines management within the practice.
Key ResponsibilitiesMedicines Optimisation
Support the Clinical Pharmacists and wider clinical team with medicines optimisation.
Undertake appropriate medication-related reviews and administrative medicines management tasks within scope of practice.
Identify patients requiring medication reviews, monitoring or follow-up.
Support safe, effective and cost-effective prescribing.
Identify prescribing issues, medicines safety concerns and opportunities for improvement.
Support patients with medication-related queries and signpost or escalate to the appropriate clinician when required.
DOCMAN and Clinical Correspondence
Review and process relevant DOCMAN correspondence within agreed timescales.
Identify medication changes, monitoring requirements and actions arising from hospital correspondence.
Ensure appropriate actions are completed or allocated to the appropriate clinician.
Update patient records accurately following review of correspondence.
Escalate any clinical concerns or issues outside the scope of the role.
Medication Monitoring
Support the practice with medication monitoring and recall processes.
Identify patients requiring blood tests and other monitoring before medication can safely be prescribed.
Support searches and recalls for medicines requiring regular monitoring.
Help ensure monitoring requirements are followed up and appropriately documented.
Work with the clinical team to reduce medication-related risks.
Blood Pressure and Long-Term Conditions
Support clinical teams with patient searches and recalls relating to hypertension and other long-term conditions.
Identify patients who may require medication reviews or monitoring.
Support the practice with blood pressure-related prescribing and monitoring work within the scope of the role.
Assist with cardiovascular disease prevention and medicines optimisation initiatives.
