Job summary
This is a new and exciting opportunity for an unregistered member of staff to join the Ophthalmology Nursing team at CTM.
The post holder will be based across Pan CTM which includes Royal Glamorgan Hospital, Ysbyty Cwm Rhondda Hospital and Princess of Wales Hospital and will be pivotal in supporting the efficient and effective running of Ophthalmology Clinics.
The post holder will principally provide clinical and technical support to the Ophthalmology service carrying out such tasks as vision testing and pharmacological dilation of pupils, RAPD checks, Optical Coherence Tomography, Ophthalmic Biometry, Pentacam and provide general support for the Pre-Assessment Nurse Practitioners.
The post holder will be trained to perform Ophthalmic imaging such as Optical Coherence Tomography, Pentacam and Biometry on completion of work-based competency assessments.
They will provide any associated administrative support required to run the service as requested.
This post is fixed term/secondment for 2 years to cover a secondment. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.
Main duties of the job
The successful candidate will principally provide clinical and technical support to the Ophthalmology service carrying out such tasks as vision testing and pharmacological dilation of pupils.
The post holder will also be trained to perform other methods of Ophthalmic imaging such as Optical Coherence Tomography (OCT), Pentacam Imaging and biometry on completion of work-based competency assessments.
They will provide any associated administrative support required to run the service as requested.
The successful candidate will also be expected to complete documentation to an acceptable standard and assist in keeping the clinical area clean and clutter free.
The applicant will be expected to demonstrate excellent interpersonal and organisational skills along with the ability to use their own initiative and work flexibly to meet the service needs.

The Home Enteral Nutrition (HEN) team are looking for an administrator to join our team. This position will suit you if you if you have clerical or administrative experience working in a busy office environment, are a good communicator and enjoy working as part of a team.
The team is based at the Waldron Health Centre in New Cross; working pattern is Mon-Fri 09:00-17:00, not including bank holidays.

You will need to be reliable, conscientious, have good organisational and problem-solving skills. You will need to understand ICT systems and the ability to use other systems such as electronic patient record systems, Microsoft Word and Excel. You also need to feel comfortable working independently and knowing when to seek support.
Main duties of the job
You will work as part of the administration team responsible for the smooth running of this specialist community service; where your professionalism, initiative and office skills will be welcomed. You will have plenty of room to be creative and the team is always open to new ideas.
The post holder will be expected to provide a full range of administration duties including:
- Answering telephone calls from patients, carers and health professionals daily
- Receiving and processing patient referrals from hospital
- Dealing efficiently with enquiries and providing basic information to patients, carers and health professionals
- Registering patients on the patient records system and with feed companies and organising feed delivery across South East London
- Creating referrals and uploading documents and text to the patient electronic record system
- Developing and maintaining effective office systems
The HEN Team is a specialist service providing expert support for people of all ages to safely manage their eating and drinking and tube feeding at home in Lambeth, Southwark and Lewisham. The Golden Valley District Nursing Team are a small but very supportive and dedicated team, we are an incredibly engaging, positive and hard-working team, providing care and support to patients in their own homes. Due to the small size of the team with a very large geographical area to cover, community experience/knowledge is beneficial and advised for this post.
We work closely with a wide range of multi-disciplinary teams, including GP's, Palliative Care Nurses, Therapy Teams, and Advanced Clinical Practitioners.

Main duties of the job
Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions.
Due to the nature of this role, the ability to meet the travel requirements of the post are essential.
The Golden Valley cover one of the largest areas within the service, this post will include a high amount of travel.

We are looking for a Retinal Screener / Photographer for our Diabetic Eye Screening Programme in North East London area (City and Hackney, Newham, Barking and Dagenham, Redbridge, Tower Hamlets and Waltham Forest.
The post-holder will deliver a consistently high quality care for the service for the patient with diabetes. The role requires you to work unsupervised within a community based environment; you will be working in a team with team leaders support.
Diabetes is the most common chronic illness affecting people in East London. The detection and early treatment of diabetes-associated problems can prevent disability and premature death. The Diabetes Eye Screening Service aims to detect and offer early treatment to peoplewith diabetes who are developing sight threatening eye disease.

Main duties of the job
The post holder will already have an easy rapport with people and have good organisational skills. You will be someone who enjoys patient contact, and who can make a clinic visit a positive experience for anyone with diabetes and will work on a one to one basis with patients for much of the day and will be responsible for both examining and gathering information from patients to support the clinical assessment of whether or not diabetes eye disease is present.
You will need no prior skill in diabetes eye screening and will be trained by us. This training will include the use of eye screening software initially and later on using the retinal camera. In time the post holder is expected to enrol in, and then to undertake the Health Screening Diploma, supported by local training and mentoring. When fully trained you will be expected to rotate between screener, photographer and receptionist roles, and at times undertaking database entry.
Screener Photographers will have a main base where they are predominantly located, but must be flexible, and able to work across any other screening location as required by the service.

We are looking for a motivated and compassionateCommunity Support Worker to join our falls response service, within the Urgent Community Response Team. The successful candidate will respond to adults who have experienced a fall within the community, providing timely assessment, support, and intervention to help avoid unnecessary hospital admissions and promote recovery at home.When you're not out supporting our patients, you will be in the Single Point of Access HUB as part of the call handling team, you will be answering the phones, taking referrals from member of the public and healthcare professionals and supporting the wider teams.
The Community Referral Hub will soon be operating 24/7, 7 days a week, 365 days a year. Due to the role requirements, you ideally need to be able to work 12 hour shifts and be able to work 1 weekend a month. This role will involve working rotational day and night shifts.
Flexibility is a vital part of this role to support service needs such as annual leave, sickness or service pressure. Flexibility goes both way, we offer a range of flexibility to support you. This can be discussed at interview.
Main duties of the job
Once a call has been made to our Community referral hub (CRH) you will be required to attend the patients home within a 45 minute target time.
- Assess the patients home to ensure that they are not injured and it is safe for you to assist
- Support the patient in regaining an upright position, with the support of a multitude of equipment
- Support the patient with any basic care requirements
- Complete manual handling as appropriate for the scenario and in line with manual handling guidelines
- Complete the IStumble evaluation form using our EMIS system for the patient
- Complete the patient referral in its entirety
- Complete any onwards referrals for the patient
- Add any follow up calls & complete when necessary
- Be part of the whole CRH Team
- Take phone calls from community services, patients and other HCP's and ASC professionals and complete the referrals on the EMIS system
- Being able to multi-task and using an array of different systems
- Using a computer will be a big part of your role
- Being able to either touch type or type confidently at a steady speed
