Job summary
The post holder will be an experienced healthcare practitioner with experience in acute and urgent care. Ideally they will have completed an MSc in Advanced Practice, and must be independent prescribing. The post holder must be able to evidence a clinical portfolio and relevant competencies in line with the HEE national framework for advanced clinical practice. Advanced clinical practitioners are expected to work across the four pillars of advanced practice; clinical, leadership, research and education.
The post holder will practice as an Advanced Nurse Practitioner across various services, including but not limited to ABC Remote Services (uCATS, Enhanced Access, Additional Primary Care, Smoking Cessation and ADHD Primary Care Connect)
The post holder will autonomously undertake assessment and treatment of both acute and chronic, differentiated and undifferentiated, patient presentations.
The post holder will determine a clinical diagnosis, treatment plan and appropriate discharge, or refer, as indicated whilst maintaining accurate patient records and ensuring patient safety. They will be a source of clinical expertise to less experienced staff, enhancing skills and knowledge, and work in partnership with a multidisciplinary team.
Training will be provided to equip the post holder to manage the prescribing of smoking cessation medication as part of a collaborative service with West Sussex County Council.
We have 12 Saturday and Sunday shifts available.
Main duties of the job
- To independently undertake specialised clinical assessments, diagnose,treatand discharge, or appropriately refer, patients with a wide range of acute and chronic illnesses.
- To ensure patients are fully involved in the design and implementation of their treatment plans.
- To ensure effective coordination and liaison with multidisciplinary teams across primary and secondary care, statutory and voluntary disciplines/groups to provide a seamless service to patients and their carers and prevent hospital admission where urgent care is required.
- To undertake assessments as part of the ADHD Primary Care Connect service, diagnosing and prescribing medication as appropriate and with the support of the Consultant Psychiatrist
- To prescribe smoking cessation medication in line with service protocols
- To refer appropriately to clinical leads/supervising GP/Consultant Psychiatrist where a second opinion, or supervision, is required.
- To adapt styles of communication appropriately to promote empowerment, self-management, and health education for patients.
- To attend clinical case conferences, MDTs, review and clinical meetings as required.
- To be a source of clinical expertise to less experienced staff within the team.
- To delegate duties/tasks to other members of staff where appropriate.
- Provide specialist advice relating to minor illness and/or injury to local healthcare teams, patients, relative, and carers.

Job summary
The post holder will be an experienced healthcare practitioner with experience in acute and urgent care. Main duties of the job

- To ensure effective coordination and liaison with multidisciplinary teams across primary and secondary care, statutory and voluntary disciplines/groups to provide a seamless service to patients and their carers and prevent hospital admission where urgent care is required.

Job summary
Burgess Hill & Villages PCN is looking to employ a Children and Young People's Social Prescriber on either a full time or part time basis. This exciting role is an integral part of the Primary Care team, and as such, the successful post holder will continue to work as part of our current CYPSP service, as well as having the responsibility for collecting user feedback and adapting the role to suit the needs of the users in line with the Thrive model.
The role is primarily to work with children and young people aged up to 19 years (up to 24 years old for users with Learning Difficulties) but may also involve work with their parents/carers. You will work closely with the GP practices, schools and colleges (if required) and a range of established services to provide a range of support for children, young people and their families and deliver the appropriate level of support at the right time.
Main duties of the job
Social prescribing empowers people to take control of their health and wellbeing through referral to a non-medical link worker who gives time, focuses on what matters to me and takes a holistic approach, connecting people to community groups and statutory services for practical and emotional support. Link workers support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners. Social prescribing can help to strengthen personal and community resilience and reduces health inequalities. It addresses the wider determinants of health, such as debt, poor housing, and physical inactivity, by increasing peoples active involvement with their local communities.

Burgess Hill and Villages Primary Care Network is an NHS collaboration between four GP practices - Brow Medical Centre, Meadows Surgery, Mid Sussex Health Care and Silverdale Practice - working together to provide enhanced access services.
Our surgery teams work closely, sharing expertise and resources to develop new services. Our vision is to continue to improve the quality of care that we provide in alignment with the needs of our patient population.
Our PCN builds on the existing primary care services and enables a greater provision of proactive, personalised and more integrated health and social care. Job summary
The post holder will work across the practices with each of the practice teams and will be assigned a lead clinical and operational contact for each contract. This post is part of the Additional Roles Reimbursement Scheme for Primary Care Networks funded by NHS England and includes funded access to the Primary Care Pharmacy Education pathway pilot for pharmacy technicians.

Main duties of the job
The post holder will work as part of a multi-disciplinary team and this role involves provision of technical support for the pharmacy service to ensure professional operational standards and excellence in pharmaceutical care and safety are maintained.
The role will support the practices within Burgess Hill & Villages PCN to ensure the delivery of safe, effective and efficient systems for medicines related clinical correspondence; repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.
The post holder will support training and development of administrators to support safe, effective and efficient processes involving medicines with a particular focus on repeat prescribing processes and will also provide support to general practice staff and patients regarding prescription and medication queries. They will also help to develop and improve repeat prescribing & medicines reconciliation processes and systems for safer prescribing.
The post holder will ensure that the service is integrated with community and hospital pharmacy to improve patient outcomes. The pharmacy technicians will work closely with the CCG Medicines Optimisation teams, community pharmacists and hospital pharmacy teams to support an integrated approach to prescribing and medicines optimisation which works towards shared goals.

Main duties of the job
This role combines social prescribing, health promotion, and smoking cessation support specifically for Gypsy, Roma and Traveller communities across Surrey.
The post holder will:

Job summary
The Senior Clinical Services Manager is a key member of the Clinical Services (CS) team. This role is designed for an experienced leader (at least two years in a clinical service management or similar leadership role) with a strong record in service delivery and transformation. The post holder will play a key role in supporting the wider Clinical Services team to build their knowledge of project planning from the project definition through to the digest stages.
The post holder will be responsible for planning, delivering and evaluating services that are closely aligned with ABCs values and our members priorities, working closely with commissioners and Surrey Primary Care in Surrey to establish new pathways and manage ABCs relationship particularly in the East Surrey patch. Day to day duties will include management of their own portfolio of services such as ADHD Primary Care Connect (Crawley) and Workflow (Sussex) whilst supporting, and deputising for, the Head of Clinical Services as required. Within Sussex the postholder will be work closely with Sussex Partnership Foundation Trust, Primary Care Provider Collaborative, Sussex Integrated Health as well as Sussex Community Foundation Trust and other community providers.
Main duties of the job
This is a high-impact role responsible for driving excellence, innovation, and operational performance across all clinical services. The Senior Clinical Services Manager acts as the principal deputy to the Head of Clinical Services, providing strategic oversight, expert operational leadership, and robust governance to ensure safe, effective, and high-quality care delivery. They will support team development of CS managers and coordinators whilst maintaining strong links with other departments including Finance, Quality, Digital and the PCN team. This is a highly collaborative role requiring strong leadership, proactivity and excellent communication skills.
This role requires exceptional leadership capability, advanced operational judgement, and the ability to influence at executive and system level. The Senior Clinical Services Manager will deputise for the Head of Clinical Services and ensure clinical services are resilient, data-driven, patient-centred, and aligned with organisational priorities.

Job summary
This service is a collaboration between Sussex Community NHS Foundation Trust SCFT and the Sussex Federations: Brighton & Hove Federation, Alliance for Better Care and South Downs Health & Care. The Federations will be delivering the GP-led aspect of the Virtual Ward at a place-level to match SCFTs Urgent Community Response service across Sussex. SCFT provides the service in West Sussex, Brighton and Hove and the High Weald, Lewes and Havens area of East Sussex.
B&HF are the lead contractor for the GP Service delivery and have sub-contracted to ABC for delivery within the federations geographical areas of Mid Sussex, Worthing and Chichester.
Since 2022, SCFT have delivered Virtual Wards Hospital at Home in line with the NHS Englands national definition that a virtual ward is a safe and efficient alternative to NHS bedded care that support patients who would otherwise be in hospital to receive the acute care and treatment they need in their own home.
As of August 2024, SCFT re-modelled the Virtual Ward delivery and mobilised General Virtual Wards within Urgent Community Response UCR, which supports Sussex patients within its catchment area with a higher level of acuity to be safely managed at home/chosen place of residence. This could be patients on either an admission avoidance pathway or early supported discharge pathway, where the alternative would be NHS bedded care.
Main duties of the job
The Virtual Ward activity is now prioritising admission avoidance whilst still delivering early supported discharge for patients from the Acute hospitals. It is delivered 365 days per year, 8am 8pm. UCR teams within the SCFT footprint provide urgent support to help prevent unnecessary hospital admissions and ultimately keeping patients safely at the place they call home. There is a consistent service across Sussex and is made up of multi-disciplinary teams including General Practitioners, Nurses, Pharmacists, Therapists and Carers, ensuring they can meet a range of needs quickly offering the best level of support to patients.
The multi-disciplinary team that oversees and deliver care the General Virtual Ward caseload is led by both medical a General Practitioner and clinical senior decision makers. Virtual and On Call GP cover is available seven days a week, 9am to 6pm 365 days a year, including bank holidays. This cover is linked to the respective teams local UCR teams with a virtual board round each morning
The UCR Hub Based GP will work within their assigned place based SCFT UCR base. We are recruiting for a GP for the Worthing Virtual Ward. The GP will present within the allocated UCR base working face to face with the SCFT MDT delivering GVW including supervision of ACPs and team development. Patient visits may be required. Monday, Wednesday and Friday morning

Job summary
The post holder will work with a number of Primary Care Networks in West Sussex, guiding and supporting each PCN board in strategically developing and expanding their network to better serve their patient populations. You will be integral to developing the capacity of individuals, teams and services within PCNs, whilst holding line management responsibility for Additional Roles Staff, both clinical and non-clinical, deployed across the PCNs.
The post holder will build strong working relationships with the Sussex Integrated Care Board, Community Trust, Local Authorities and various Voluntary, Community, and Social Enterprises (VSCEs). They will represent the PCNs and ABC at multiple external partner meetings, serving as a local advocate for Primary Care.
Main duties of the job
The successful candidate will join a small team, working closely with the Associate Director of Transformation, the Head of PCN and Practice Support and other PCN Development Managers that support Mid Sussex, Horsham and Surrey PCNs. The role involves close collaborative working with other ABC teams including; the People Team, Finance, Learning and Development, Communications, Clinical Services and the Refugee and Asylum Seeker Service.
To operate effectively, the post holder will need to establish excellent working relationships with:

Job summary
The post holder will be an experienced healthcare practitioner with experience in acute and urgent care. Ideally they will have completed an MSc in Advanced Practice, and must be independent prescribing. The post holder must be able to evidence a clinical portfolio and relevant competencies in line with the HEE national framework for advanced clinical practice. Advanced clinical practitioners are expected to work across the four pillars of advanced practice; clinical, leadership, research and education.
The post holder will practice as an Advanced Clinical Practitioner/Paramedic across various services, including but not limited to ABC Remote Services (uCATS, Enhanced Access, Additional Primary Care, Smoking Cessation and ADHD Primary Care Connect)
The post holder will autonomously undertake assessment and treatment of both acute and chronic, differentiated and undifferentiated, patient presentations.
The post holder will determine a clinical diagnosis, treatment plan and appropriate discharge, or refer, as indicated whilst maintaining accurate patient records and ensuring patient safety. They will be a source of clinical expertise to less experienced staff, enhancing skills and knowledge, and work in partnership with a multidisciplinary team.
Training will be provided to equip the post holder to manage the prescribing of smoking cessation medication as part of a collaborative service with West Sussex County Council.
Main duties of the job
- To independently undertake specialised clinical assessments, diagnose, treat and discharge, or appropriately refer, patients with a wide range of acute and chronic illnesses.
- To ensure patients are fully involved in the design and implementation of their treatment plans.
- To ensure effective coordination and liaison with multidisciplinary teams across primary and secondary care, statutory and voluntary disciplines/groups to provide a seamless service to patients and their carers and prevent hospital admission where urgent care is required.
- To undertake assessments as part of the ADHD Primary Care Connect service, diagnosing and prescribing medication as appropriate and with the support of the Consultant Psychiatrist
- To prescribe smoking cessation medication in line with service protocols
- To refer appropriately to clinical leads/supervising GP/Consultant Psychiatrist where a second opinion, or supervision, is required.

Based in East Sussex location, close to Tunbridge Wells with the potential requirement to travel to ABC main offices in Horley/Crawley. Must have a full drivers license with business insurance and access to car.
The role is to provide coordination and administrative support to the Asylum Seeker & Refugee Support Service with further scope for expansion into other areas of operational management within Inclusion and Clinical services Team.
The coordinator will be responsible for liaising with the Service Manager and wider team and supporting the operationalisation of the service. This is a continually evolving service which flexes, and is shaped, by the needs of our patients. The coordinator will need to be comfortable with a process of ongoing change and improvement to enable the service to provide optimal care.
This is a rewarding role within a close and supportive team of both clinical and non-clinical staff. Whilst the role is fast moving and one where activity may have to be stepped up rapidly, the environment is calm and there is a focus on resilience and teamwork.
Main duties of the job
- To support the mobilisation of the response to Asylum Seeker & Refugee service into a site in East Sussex
- To set up for and be on site administrator during clinic days, including registrations and health checks
- Planning appointment lists for health checks, vaccinations and screening clinics
- Preparation for clinics, including test requests, printing, labelling, and restocking equipment
- Management of clinical handover sheets
- Supporting the management of stock of clinical equipment, prescription medication, vaccines, over the counter medication and consumable supplies
- Use of SystmOne and EMIS clinical systems to build rotas, clinics, input patient data and run reports
