Job summary
We are looking for an enthusiastic and motivated Associate Practitioner to join our Physical Health and Wellbeing Team, supporting the delivery of physical health and wellbeing clinics and clozapine services across Bradford District Care NHS Foundation Trust's community mental health services.
This is an exciting opportunity to play a key role in improving the physical health outcomes of people living with serious mental illness. You will work alongside nurses, clinicians and the wider multidisciplinary team to provide physical health monitoring, health promotion, wellbeing interventions and lifestyle support for service users across community mental health services.
The successful candidate will be passionate about reducing health inequalities, promoting healthy lifestyles and supporting people to achieve better physical and mental wellbeing. You will have the opportunity to develop specialist skills in physical health assessment, health coaching, venepuncture, ECG recording and long-term condition monitoring within a supportive team environment.

Main duties of the job
o To provide physical health monitoring and wellbeing interventions to people requiring physical health checks or when initiating new medication, within Community Mental Health and Clozapine services.
o To deliver quality care and support which enables clients to be supported throughout their care pathway whilst promoting health and wellbeing.
o To work alongside both registered and unregistered staff running clinics, supporting colleagues and facilitating groups.
o To communicate effectively at all times within the team, with other health care professionals and with the client and their family.

We are looking for motivated and compassionateBand 5 Community Nurses to join our dynamic locality teams. This is an exciting opportunity to deliver high quality person centered nursing care to people in their own homes, supporting them to remain independent, safe and well within their community.
This role is ideal for both qualified and newly qualified nurses, those seeking their first community post, or nurses wanting to broaden their skills and progress with locality teams, within a well structured career pathway.
You will be offered a structured preceptorship and as a community nurse working within the planned care service , you will be providing high quality care for people in their own homes, ranging from long term conditions, to complex and palliative care. You'll be working on a rotational basis as part of a seven-day service, with the support of agile technology to help you deliver timely, high quality care, with support from specialists across the Trust.
Shift patterns are between the hours of 8am and 9pm, 7 days per week and We havevacancies across multiple community locality teams.

Here in Bradford CAMHS we are looking to appoint a Band 6 Mental Health Practitioner. There will be an opportunity to build therapeutic relationships with young people and their support systems, whilst assessing risk and mental health needs. Job summary
We have Trainee EMHP posts available in Bradford and Craven District- a chance to work and study to be an EMHP with a fast growing team across Bradford. The Green paper set out the ambitions to increase the mental health support for young people within school settings. Bradford currently has a developed team and is expanding.
These are 12-month fixed-term trainee posts at Agenda for Change (AfC) Band 4. Successful candidates will undertake the EMHP training programme, delivered at Prestwich, attending university two days per week, while working in service for the remaining three days.
Trainees will be paid at Band 4 during the training period. Upon successful completion of the EMHP Postgraduate Diploma and registration with a recognised professional body, the role will become permanent at AfC Band 5 as a qualified Education Mental Health Practitioner.

Main duties of the job
The post holder will provide leadership, managerial and supervisory responsibility to a group of practitioners working as a multidisciplinary team including nurses, social workers, OT's and Psychological Therapists. providing assessments and interventions within adult community mental health services.
The numbers of staff supervised will determine how the workload is managed between active clinical work and management responsibilities.The post holder will assist the clinical manager and work alongside the other team managers in continually developing the service and ensuring the delivery of an Community Mental Health Team
Partnerships will include GPs, other mental health services, learning disability services, social care, physical health services, consultation to professionals in the primary care services and other adult mental health services including the voluntary sector.
