Job summary
The Community Inpatient Unit provides short-term, bed-based rehabilitation, re-enablement, and recovery support for individuals following illness, injury, surgery, or significant health events. The service promotes independence, prevents unnecessary hospital admission or long-term care placement, and supports timely hospital discharges. Salary Range 3 £25,792 pro rata per annum, fixed salary point.
We are seeking some highly motivated and passionate Healthcare Assistant to join the MDT team at Community inpatient unit. The service provides short term rehabilitation and reablement for Services Users who may need to regain their confidence and skills on discharge from hospital or to get over a short-term health exacerbation. We encourage and support our Service Users to live as independent lives as possible and we always act in a manner that supports the care, protection, health and well-being of the Service User.
You will work alongside a wider MDT based at CIU including Nurses, Social Workers, Therapists and Assistant Practitioner to support service users to reach their rehab potential and move forward in their health journey.
You will work under direction of the Assistant Practitioner and Nursing team and will carry out assigned tasks involving personal and nutritional care needs to patients.
You will follow protocols, guidance, and procedures under the direction of senior staff/ manager/supervisor.

Job summary
Open to young people aged 16-21, with extended eligibility for care leavers, prison leavers, and those with an Education, Health, and Care (EHC) Plan up to age 24.
The NEL Community Therapy provides Occupational Therapy and Physiotherapy to support people with health and social care needs to stay in their own home and help them remain independent. Community Therapy staff are employed by both Care Plus Group and Northern Lincolnshire and Goole NHS Foundation Trust working in partnership and provide direct clinical interventions through assessment of need and person centered goal planning. The range of interventions includes treatment, rehabilitation, minor and major housing adaptations, loan of equipment and moving and handling solutions for children and adults with a primary physical disability.

Main duties of the job
Apprentice Clerical Assistant
Community Therapy Team
Based: Cromwell Road Primary Care Centre
30 hours per week - Mon - Fri
National Minimum Wage dependent on age.
Apprenticeship course- Level 2 Administration Assistant
This apprenticeship standard is open to:
- Young people aged 16-21.
- Young people aged 22-24 who are care leavers, have an EHCP, or are prison leavers.
Duration - 12 months
Qualifications- Level 2 qualification in English and Maths is required (GCSE grade 4+/C+ or functional skills).
The post holder will work with supervision close by and within established procedures and will:
- Provide administrative support to the manager and clinical staff in providing Occupational and Physiotherapy Therapy assessment and intervention to physically disabled residents living in North East Lincolnshire thereby helping to maintain people's independence and improved health and wellbeing.
- Undertake routine clerical duties, e.g. photocopying, scanning, computer work, message taking, telephone enquiries.

Job summary
The Urgent Care Teams are a 24/7 service providing, first point of contact, comprehensive triage service, telephone and face to face clinical assessment to patients and/or their carers/relatives, enabling a safe and appropriate clinical decision and plan of care.

The post holder will provide a comprehensive, administrative service to the Urgent Care Sector supporting team members, managers, senior managers and executives by performing a variety of administrative tasks. You will undertake an important role in the delivery of performance monitoring within CUCT whilst continually developing and utilising new systems to further streamline, modernise and improve the service. The Urgent Care Team is a 24/7 service; providing a first point of contact and triage service, including telephone and face to face clinical assessment of patients and/or their carers/relatives, resulting in appropriate decision making and plan of care. We are a friendly, supportive team of clinicians providing a Clinical Assessment Service to the residents and visitors to North East Lincolnshire. We play a pivotal role in the support of community healthcare and hospital avoidance. We are a unique service being able to provide a direct access point for both professionals and members of the public with all clinicians having an understanding of the area/services available and social/economic considerations ensuring a holistic approach is maintained. The service is proactive and works closely with other organisations to pilot new, innovative strategies.
By utilising agreed protocols and policies in conjunction with listening, questioning and probing skills you will support in planning individual patient care. You will complete clinical assessment of minor ailments through to life threatening emergencies and arrange appropriate and safe Ambulance and GP attendance, if required.

We are seeking a highly motivated and passionate Band 5 Nurse to join the multidisciplinary team on a temporary basis at the Community Inpatient Unit (CIU), which provides nursing and enablement services to patients within the Rehabilitation and Recovery Inpatient Unit.
CIU provide short term reablement for Services Users who may need to regain their confidence and skills on discharge from hospital or to get over a short-term health exacerbation. This includes assessing complex and specialist needs and developing, implementing and evaluating programmes of care, in order to support individuals to remain in community settings, in their own home, residential home or nursing home.
The role will require you to undertake various clinical duties including assessment, wound management, injections, and continence care as well as dealing with acutely unwell. Ideally we are looking for some with previous community experience however this is not essential as you will received training and support.
In return you will be part of a forward thinking MDT team, receive opportunities for further development, regular supervision and support.

Job summary
The Finance Team provides expert financial, procurement, and business support services across Care Plus Group (CPG). The team is responsible for financial planning, reporting, budget management, procurement support, financial governance, and the operation of effective financial systems and controls. You will:
- Have detailed understanding and knowledge of accounting standards, procedures and processes to enable complex project work and financial queries to be undertaken and resolved without supervision. You will support the annual external audit process and assist with the statutory accounts and regulatory returns.
- Identify, investigate financial variances and trends and ensure compliance with Standing Financial Instructions (SFIs) and financial procedures.
- Prepare monthly management accounts to a high standard and in a timely manner for the CPG and its subsidiary companies. Review, monitor and report on management accounts position, budget reports to management and budget holders.

As Registered Manager you will be responsible for the day-to-day management, leadership and ongoing development of the service, ensuring the delivery of safe, effective, high-quality and person-centred health and social care services. Community Inpatient unit is a bed-based facility supporting people to recover and regain their independence following a period of illness, trauma, surgery, or significant event. We are seeking some highly motivated and passionate Recovery Worker to join the MDT team at Community inpatient unit. The service provides short term rehab and reablement for Services Users who may need to regain their confidence and skills on discharge from hospital or to get over a short-term health exacerbation.
We encourage and support our Service Users to live as independent lives as possible and we always act in a manner that supports the care, protection, health and well-being of the Service User.
You will work alongside a wider MDT based at CIU including Nurses, Social Workers, Therapists and Assistant Practitioner to support service users to reach their rehab potential and move forward in their health journey.
You will work under direction of the HCA/ Assistant Practitioner and Nursing team and will carry out assigned tasks involving personal and nutritional care needs to patients.
