The GP Assistant will support GPs and the wider multidisciplinary team across Central & West Warrington PCN by carrying out a combination of administrative and basic clinical duties. The role is designed to improve patient access, enhance the efficiency of general practice services, and reduce the administrative burden on clinicians. Job responsibilities
A GPA has completed the City and Guilds Certificate. The role is to support the GPs smooth running of clinics by performing the more routine administration and clinical tasks on behalf of the GP freeing up their time to focus on the patient.
A summary of duties includes:
Phlebotomy
Blood pressure monitoring and support in hypertension review clinics
Offering patients lifestyle advice in relation to weight, lipid management, alcohol and exercise
Information gathering from patients
Understanding of clinical indemnity, vicarious liability in relation to personal and professional practice
Infection control
Maintaining a well-stocked clinical environment
Immunisation within PCN vaccination services
New patient health checks
NHS health checks and enhanced health checks
Completion of referral forms to services such as ADHD, autism services, and routine chronic disease referrals such as diabetes eye screening
Filing of routine results according to level of training and competency
Feno testing
Sorting all clinical post and prioritising for the GP in terms of actions. Signposting some post to others such as clinical pharmacist etc.
Extracting all information from clinical letters that needs coding and adding to notes
Arranging appointments, referrals, tests and follow up appointments of patients
Preparing patients prior to going in to see the GP, taking a brief history and basic readings in readiness for the GP appointment
Dipping urine, taking blood pressure, ECGs & phlebotomy
Completing basic forms and core elements of some forms for the GP to approve and sign such as insurance forms, mortgage, benefits agency forms etc.
Explaining treatment procedures to patients
Helping the GP liaise with outside agencies e.g. getting an on-call doctor on the phone to ask advice or arrange admission while the GP can continue with their consultations
Supporting the GP with immunisations as guided by PSDs
- While seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately

Job summary
Central and West Warrington Primary Care Network (CWW PCN) is seeking to recruit a Lead Practice Pharmacist to develop and support medicines optimisation across our seven GP practices.
This is an exciting opportunity for an experienced and motivated primary care pharmacist to join an established, forward-thinking PCN team. Our pharmacy team includes Clinical Pharmacists, Pharmacy Technicians and a Care Coordinator, supported by a Senior Pharmacist, PCN Clinical Director, and the wider PCN team.
The post will be directly employed by a core practice but will work collaboratively across multiple practices within CWW PCN to support delivery of the Primary Care Network DES and local practice priorities.
We are keen to hear from pharmacists with experience in primary care, who have completed the CPPE Primary Care Pharmacy Education Pathway and Independent prescriber (IP) qualification.
Main duties of the job
The Lead Practice Pharmacist will be committed to improving health outcomes and putting patients first. Key qualities include the ability to build effective relationships with patients, GPs, nurses, community pharmacists, hospital teams, and practice staff, alongside alongside the ability to lead and work effectively within a multidisciplinary team.
The post holder will:
- Work as part of a multidisciplinary team in a patient-facing role, providing leadership to the practice pharmacy function
- Support delivery of Primary Care Network DES specifications
- Conduct and oversee structured medication reviews (SMRs) for patients with complex polypharmacy, including patients in care homes
- Provide expert clinical medicines advice to patients and practice teams
- Lead initiatives to improve prescribing practices across the Practice(s) and PCN
- Drive quality improvement and service development, including audits and implementation of best practice
- Work with community and hospital colleagues to resolve medication-related issues and improve care pathways
- Take overall responsibility for pharmacy governance and patient safety within the Practice(s), including overseeing the timely actioning of safety alerts and ensuring safe drug monitoring processes are in place
- Provide clinical leadership, support, and supervision to junior pharmacy colleagues and trainees

Training Support
Most roles in our PCN require a training element. Staff partaking in training that meets certain requirements will be required to sign a Training Agreement that outlines associated costs to the training. Staff may be liable for reimbursing those costs to the Practice / PCN if they leave their PCN role before, during or after the training. - Working as part of a multidisciplinary team in a patient facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas. The role will contribute to successful clinical management of patients and build capacity in the workforce at the right levels.
- Using clinical skills to provide specialist medication review and assessment of patients with long term conditions having complex medicine regimes and produce the best outcomes for the patient.
- To further proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular Frailty, COPD and Asthma); people with Learning Disabilities or Autism (through STOMP-Stop Over Medication Programme)
- Providing specialist expertise and advice in respect to the management and use of medicines and medicines management to patients, carers, practice staff as well as a wide range of health and social care professionals in helping to tackle inequalities
- Be an Independent Prescriber and safely prescribe within your scope of competence, ensuring evidence-based, patient-centred care.
- Provide leadership on person-centred medicines optimisation (including ensuring prescribers in the Practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement whilst contributing to the quality and outcomes framework and enhanced services
- Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care

Job summary
We are seeking an enthusiastic and experienced Paramedic Practitioner to join our Acute Home Visiting Service, delivering high-quality care to patients in their own homes across the Primary Care Network (PCN).
Working autonomously within a multidisciplinary team, you will assess, diagnose, treat and manage patients with acute illness, frailty and complex health needs who are unable to attend surgery. Using advanced clinical skills, you will develop management plans and facilitate admission, referral or community-based care where appropriate.
This is a part-time role working 10:00am to 6:00pm. Working days will include every Friday, which is fixed due to service demand, together with either Wednesday or Thursday, to be agreed with the successful candidate.
Working closely with GPs, community teams, care homes and secondary care services, you will deliver integrated care, support patients to remain safely at home and help prevent avoidable hospital admissions. You will also contribute to service development, quality improvement and patient education.
The successful candidate will be expected to complete the First Contact Practitioner (FCP) Roadmap to Practice if not already achieved. Full support, supervision and development opportunities will be provided to enable completion of the pathway.
Interviews are expected to be held on the 12th October and 14th October 2026.
Access to a vehicle with business-use insurance is essential for this community-based role.
Main duties of the job
The Paramedic Practitioner will provide autonomous, patient-centred care within the Primary Care Network, delivering acute home visits and supporting patients within both community and practice settings. Working as part of the multidisciplinary team, the postholder will assess, diagnose, treat and manage patients presenting with acute and long-term health needs, developing appropriate management plans and facilitating referral where required.
