Working with the Procurement and Contracts Team, you'll provide vital support that helps contracts, supplier relationships and procurement processes run smoothly and effectively. This role plays an important part in ensuring services have the agreements, records and information they need to operate efficiently. You'll work closely with colleagues, suppliers and stakeholders, helping to maintain accurate records, coordinate meetings and support key administrative processes. This is a great opportunity for someone who enjoys organising information, working with a range of people and developing their administration skills in a supportive team. Please note that this advertised vacancy does not meet the UKVI eligibility requirements for a Skilled Worker Visa and therefore City Health Care Partnership would not be able to issue a Certificate of Sponsorship for this role

Main duties of the job

What you'll do:

Support the administration of contracts and procurement processes
Arrange meetings, take notes and produce accurate minutes and action logs
Maintain contract records, databases and electronic filing systems
Act as a first point of contact for enquiries from colleagues and external providers
Help keep supplier and contract information accurate and up to date

Job summary

An exciting opportunity has arisen for a highly motivated and enthusiastic individual to join the Hull and East Riding Intermediate Care Team as a Bank Generic Therapy Associate Practitioner. An exciting opportunity has arisen to join the North Community nursing team as a clinical practice Teacher. Job summary

We are looking for an enthusiastic and dynamic, and experienced Band 3 Generic Support Worker to join the well-established Hull East riding Falls service. The post holder will also be expected to work as part of the wider multidisciplinary falls team to provide Therapy and Nursing support to patients/clients within standard operation procedures and contribute to the care planning process, this will include travelling across both hull and East Riding to visit patients in their own homes. Do you have a genuine passion for supporting people living with obesity? An exciting opportunity has arisen, for an enthusiastic and motivated band 4 Physical Activity Associate Practitioner, to join our dynamic team within Tier 3 Specialist Weight Management.

The role will involve working with the Tier 3 Specialist Weight Management Multi-Disciplinary Team (MDT) to support patients to lose weight and improve their health and wellbeing. The role will involve assessing and providing tailored physical activity advice and support to patients who access the service. The Tier 3 Specialist Weight Management Physiotherapist will provide clinical support and supervision.

This is a rewarding and challenging post and the postholder should ideally have experience of supporting patients living with obesity and complex health conditions.

Main duties of the job

The role will include identifying, demonstrating, and observing specific, tailored exercises to meet the needs of the patient. Excellent time management skills are an essential part of the role as is the requirement for accurate record keeping within electronic patient records.

The ability to use a range of communication methods and behaviour change approaches are also key attributes required for the role. The postholder should also have a good understanding of the complexity of obesity and the challenges associated with obesity and physical activity

The post will be based at Marfleet Health Centre in Hull, however the successful applicant will also be expected to work from various other bases across the city.

12 month post in Community Rehab:

Hull and East Riding Community Rehab is a multi disciplinary team consisting of Physios, OTs and Assistant Practitioners. Physios treat some patients in a clinic setting, whilst most people are seen in their own homes, therefore travelling across Hull and the East Riding is essential.

The role of the Physio in the team is to assess, develop and implement a rehab programme tailored to an individual's goals. This programme of intervention could be carried out by the Physiotherapist or delegated to an Associate Practitioner.

The successful candidate will be part of the CHCP rotation, which offers CPD sessions on a number of topics, and access to a Physiotherapy Professional Development Group also.

Job summary

Working with a supportive care team, you'll help create a clean, safe and welcoming environment so that people receiving care can feel comfortable and supported. Main duties of the job

What you'll do:

Maintain high standards of cleanliness and hygiene throughout patient and communal areas

Support meal preparation and assist with basic feeding tasks when required Monitor the environment, report concerns and contribute to quality improvement activities

Use cleaning equipment safely and help manage supplies and stock levels

Work with colleagues to provide a responsive, safe and welcoming service for patients
