Job summary
An exciting opportunity has arisen for an enthusiastic and experienced Therapy Support Worker to join the North Cornwall Community Rehabilitation Therapy Team based at Carew House in Bodmin.This post will be a generic post working across Physiotherapy and Occupational Therapy so we are looking for a candidate who has experience at a Band 4 level in both of these areas, however this a supportive learning environment and we will help you to develop in both areas with competencies, our monthly in service training, and working jointly with our experienced therapists and support workers. The Community Rehabilitation team work in the North Cornwall locality, covering a large rural area, to provide rehabilitation to people in their own homes which also includes care homes.We are looking for a colleague who is reliable, flexible, and wishes to develop their assessment, care planning and rehabilitation skills whilst being part of a dynamic, progressive multi-disciplinary team.
Main duties of the job
Under the guidance of the Occupational Therapists/Physiotherapists you will be responsible for the assessment, treatment and discharge planning of patients within your care. You will need to assist in the interpretation of information relevant to the patient's condition/progress, identify problems and patient needs, setting realistic goals.You will need to have excellent organisational skills combined with experience of working in older person's services. You will need a valid UK driving licence and the ability to travel to different locations to facilitate hospital discharges.We are a supportive team, including OTs / PTs and support workers with excellent MDT working with other disciplines. We offer a friendly , supportive working environment where you can develop your clinical and interpersonal skills. CPD is actively encouraged and supported through regular supervision/appraisal and training.

Cornwall Partnership NHS Foundation Trust is seeking an enthusiastic and motivated Consultant Psychiatrist to join our established inpatient team on Carbis Ward, based at Longreach House in Redruth. As part of a dedicated inpatient medical team, you will provide consultant leadership for adults experiencing acute mental illness, working alongside highly skilled multidisciplinary colleagues, including other approved clinicians, within a Royal College of Psychiatrists AIMS-accredited service. Main duties of the job
You will play a key role in the ongoing development of inpatient services, working collaboratively alongside other approved clinicians, community teams and system partners to ensure safe, effective and person-centred care.
Key responsibilities include:

Providing consultant psychiatric leadership and clinical expertise to the multidisciplinary team

Undertaking comprehensive psychiatric assessments, diagnosis, formulation and evidence-based treatment planning for patients with complex acute mental health needs.

Acting as Responsible Clinician and Approved Clinician and providing expert advice on complex legal and ethical matters.

Providing senior medical input into risk assessment, crisis management and complex decision-making for patients presenting with severe mental illness and associated risks.

Contributing to service transformation, quality improvement and the ongoing development of integrated adult mental health services across Cornwall.

Fostering a culture of learning, compassion and clinical excellence.

Participating in undergraduate and postgraduate medical education, including teaching and supervision of medical students and resident doctors.

Through a comprehensive induction programme and ongoing clinical supervision, you will develop the skills and confidence to deliver high-quality care within patients' homes and community settings.
You will assess, plan, implement and evaluate care for patients, carers and families, promoting independence, wellbeing and quality of life. Responsibilities include undertaking holistic assessments, developing personalised care plans, providing health education and support, and delivering evidence-based nursing care. You will work autonomously within your scope of practice while collaborating closely with GPs, therapists, social care teams and other partners to provide seamless care.

Community Nursing operates seven days a week between 08:00 and 22:00, with a range of shift patterns including 08:00-16:00, 08:00-18:00, 12:00-22:00 and 14:00-22:00. Through a comprehensive induction programme and ongoing clinical supervision, you will develop the skills and confidence to deliver high-quality care within patients' homes and community settings.
You will assess, plan, implement and evaluate care for patients, carers and families, promoting independence, wellbeing and quality of life. Responsibilities include undertaking holistic assessments, developing personalised care plans, providing health education and support, and delivering evidence-based nursing care. Job summary
Liskeard Minor Injuries Unit has a rare opportunity covering maternity leave for a fixed term contract of twelve months for an MIU Practitioner, to work in a practitioner-led Minor Injuries Unit (MIU). The MIU currently operates 8am-8pm.
This is a Band 6 post and we are looking for a Practitioner who is able to work in an advanced practice role which includes minor illness and paediatrics at band 6 level .
You need to have a passion for minor injury care, good communication skills, and a commitment to providing excellent care as part of the MIU team. You must be responsive to change and flexible in your approach as we are in an exciting period of development for the MIU service. A sound knowledge of professional issues will be required.
It is an expectation of the role that the post is offered on the basis of regular rotation shifts to cover evenings and weekends. We are a small team and flexibility is vital to ensure service provision.

Job summary
The opportunity has arisen to lead the community based Acute care at home team for the North and East locality of Cornwall. This role offers an exciting opportunity to lead a team of senior nurses providing support and treatment to people in their own homes. The Acute care at home team deliver treatments which include the administration of IV medications and the monitoring of people during an acute episode of illness to enable patients to recover at home safely. The team will support early discharge from the acute settings, whilst also providing care to prevent a hospital admission where possible. The team work closely with the Virtual wards and Hospital at Home teams to identify people requiring this care. The team leader will be responsible for the professional and operational leadership, for quality and governance within the team ensuring the service is cost effective and efficient whilst also seeking opportunities for team and service development.
Main duties of the job
Keyelementsoftheroleare:daytodaycoordinationand supervisionoftheteam, providing complex casemanagementandsupport for the team. To be an autonomous practitioner responsible for the assessment and provision of treatments for patients on the caseload. To manage the performance, and clinical effectiveness of the team ensuring the delivery of evidence based care. To be able to provide assurance that performance standards are being met whilst meeting service targets. To identify new opportunities for service development.
