Job responsibilities
- To be professionally and legally accountable for all aspects of own work and maintain high standards of clinical care.
- To independently manage a caseload of driver, passenger and outdoor mobility assessments, often working autonomously.
- To undertake comprehensive assessments of clients with physical, cognitive, neurological and learning impairments, including those with complex or multiple conditions.
- To undertake assessments informed by evidence-based practice, specialist training and professional development.
- To identify physical, cognitive and psychosocial factors that may impact assessment outcomes and recommendations.
- To build effective working relationships with external agencies including NHS services, Adult Social Care, DVLA, Motability and equipment providers.

We have an exciting opportunity for a Pharmacy Technician to join our Pharmacy team at HMP Moorland, a men's category C Prison and young offender institution located in South Yorkshire.
Free parking, low turnover of staff, wellbeing days, strong relationships and partnerships. Working in a busy team of 6, who are a close knit and supportive team, with great team work.Hours
37.5 hours a week working 07:30 - 17:30 three days a week and 07:30 - 17:00 the other day a week. Shifts spread across 7 days. Includes 2 weekends a month and bank holiday working.
Salary
This position is full time, youll receive an annual salary up to £41,658 per annum FTE - depending on experience.

Job summary
We are seeking to appoint to a Specialty Doctor post within Adult Mental Health Services, based in Scunthorpe and covering the area of North Lincolnshire. The Trust provides a comprehensive adult, child, older adult, learning disability, and community forensic mental health services to Rotherham, Doncaster and North Lincolnshire. It also provides Community Physical Health Services and Substance Misuse Services in Doncaster.
The team in North Lincolnshire is keen to welcome applications from dynamic psychiatrists with a progressive approach to MDT working, to provide medical input to patient care for adults of working age in our Acute Care Pathway.
Applicants must have at least 4 years experience in psychiatry post-graduation. They should have evidence of good communication skills and a commitment to multi-disciplinary and multi-agency working.
This post will work in a team which aims to support people as close to home as possible during mental health crisis, avoid admission where possible, and work to support people being early-discharged from inpatient wards to home to continue treatment. The doctor will also work to support the intended development of our Open Access service which will start to provide a meaningful alternative to people being assessed in A&E from September 2026 and will be done in collaboration with local mental health third sector organisations and peer support workers.
Main duties of the job
Conduct initial psychiatric assessments, mental state examinations, and risk assessments for new patients.
Formulate treatment plans in collaboration with consultants and the wider multidisciplinary team.
Participate in multidisciplinary meetings and case discussions.
Prescribe and monitor psychotropic medications, ensuring adherence to NICE guidelines.Manage medical emergencies and liaise with general medical teams as required.
Work closely and communicate effectively with others involved in patient care.
Participate in discharge planning and liaise with community mental health teams to ensure continuity of care.

Previous experience in a secretarial role supporting a senior manager is essential. Candidates will need to be flexible, be able to work independently and be proactive. The post holder will be proactive in organising the full range of administrative/secretarial support to the Care Group Senior Management Team.

Pro-active, efficient and effective diary management for appointments and meetings (face to face or on via Microsoft Teams, allowing sufficient 'free' time for Director[s] to fulfil their responsibilities. Where appropriate to delegate non-essential meetings to relevant members of the management team/direct reports.
Review, sort, prioritise and respond to email correspondence, providing draft responses (where possible) and in collaboration/agreement with allocated Directors, managing their email inbox to work more effectively and efficiently with the Directors working pattern.
Undertake a proactive approach to planning/rescheduling meetings, and responsibility for organising multidisciplinary meetings, conferences and events as required.

We are seeking an enthusiastic and experienced Mental Health Practitioner to join our Hospital Liaison Team.
Your role will be to provide Urgent and Emergency response to Mental Health Referrals from The Rotherham Foundation Trust (TRFT) with whom we work in close partnership. You will undertake full bio-psychosocial assessments and contribute to providing safe and supported discharge plans. You will work closely with TRFT staff to develop their knowledge of Mental Health- including involvement in delivering training and supporting TRFT staff to develop positive working practices which put the patient at the centre of their own care and promote parity of esteem.
Main duties of the job
You will be a knowledgeable and a competent practitioner with excellent assessment and risk assessment skills, the ability to work well under pressure and demonstrate the confidence and ability to work autonomously and as a member of the wider MDT. You will have experience of working with adults with complex mental health needs and have a willingness to continue your professional development.
The team works over a 24/7 period, and this involves a flexible duty rota covering this period. Full-time applications will only be considered.

You will be responsible for collecting stock and equipment for the clinic, setting up examination rooms for clinicians, and acting as a chaperone during patient consultations and procedures. Additionally, you will take on administrative duties, ensuring all reports are completed with outcomes and providing support related to administrative tasks. Your support will ensure the smooth functioning of the clinic and contribute to providing a compassionate and efficient healthcare experience for our patients.
Please note our service operates seven days a week and you will be required to work 5 out of 7 days. Shift patterns are generally between 07:00/07:30 - 17:30/18:00.
Our Clinic locations are nationwide and include Manchester, Birmingham, Hereford, Warwickshire, Shropshire, Telford, Worcestershire, Stoke, Staffordshire, Doncaster, Nottinghamshire, Wrexham, Gloucestershire, Reading, Frimley, Slough and Bassetlaw. Your day in the life of a Healthcare Assistant
As a Healthcare Assistant at HealthHarmonie, you will be supporting our 2 Week Wait, Dermatology contract across the Midlands, primarily around Birmingham. You will be responsible for collecting stock and equipment for the clinic, setting up examination rooms for clinicians, and acting as a chaperone during patient consultations and procedures. Additionally, you will take on administrative duties, ensuring all reports are completed with outcomes and providing support related to administrative tasks. Your support will ensure the smooth functioning of the clinic and contribute to providing a compassionate and efficient healthcare experience for our patients.
Please note our service operates seven days a week and you will be required to work 5 out of 7 days. Shift patterns are generally between 07:00/07:30 - 17:30/18:00.
Our Clinic locations are nationwide and include Manchester, Birmingham, Hereford, Warwickshire, Shropshire, Telford, Worcestershire, Stoke, Staffordshire, Doncaster, Nottinghamshire, Wrexham, Gloucestershire, Reading, Frimley, Slough and Bassetlaw. This is a mobile role and you will be expected to travel to any of these site locations.

Job summary
We are looking for a dietitian to work in primary care in Doncaster South. This is maternity cover to start in January 2027. This is an in-person role.
We support and help dietitians become first contact dietitians (FCDs).
First Contact Dietitians are qualified autonomous clinical practitioners working in GP practices who can assess, diagnose, treat, and manage dietetic problems and undifferentiated conditions, and where appropriate, to discharge a person without a medical referral.
FCDs are a key multidisciplinary team member in primary care and support reducing GP workload. FCDs working in this role can be accessed directly by self-referral or staff in GP practices can direct patients to them to establish a rapid and accurate management plan to streamline pathways of care.
Main duties of the job
The dietitian will be a highly skilled, senior practitioner able to operate at an advanced level of clinical practice, with expert knowledge in the management of patients dietary issues and conditions. They will work as an integral part of the multi-disciplinary team (MDT) within the PCN.
The dietitian will provide person centred interventions for the patient from initial clinical assessment, diagnosis, treatment and evaluation of their care within set patient groups. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the general practice MDT to meet the needs of patients, support the delivery of policy and procedures, and provide dietetic leadership as required.
The role requires the dietitian to work in-practice, and work closely with a forward thinking and passionate multi-disciplinary team.
The PCN is looking for a dietitian who can support their patients with:

Job summary
The Occupational Therapist will provide the occupational therapy service to Danescourt, a six-bed community forensic step-down unit supporting individuals transitioning from secure forensic settings towards greater community independence.
The post holder will function as the sole Occupational Therapist within the unit, holding responsibility for the delivery, development and evaluation of the occupational therapy pathway for service users with complex mental health needs, offending histories, forensic risks and significant occupational functioning difficulties.

Main duties of the job
Assess, diagnose and treat a wide range of foot pathologies for patients seen within a wide range of settings, some of which may be in isolation, e.g. health centre community clinics, GP practice clinics, domiciliary visits, physical/mental health/learning difficulties and social services establishments. You will be given the opportunity to develop your skills further in the specialist areas:
o MSK conditions, fitting and modifying off the shelf orthotics
o Nail surgery and other minor soft tissue procedures
o Wound care including diabetes foot ulcer management
o Education of staff and patients
o Audit and research projects
o Multidisciplinary working with other specialist services such as TVAL, diabetologists, vascular, podiatry surgery.

Main duties of the job
We are looking for passionate, reliable and progressive staff who work within a strengths and recovery based approach.
Liaison with families and supporting their understanding of crisis management is an important part of the role.
The post will require individuals to work alongside partner organisations, third sector and peer support services, transparency and openness is vital.
This is a great time to begin working in the Crisis Service as we are starting to embark in change around the community working age model and you will have the opportunity to contribute and lead in some of these changes.

We have an exciting opportunity for a General Practitioner to join our Primary Careteam at HMP Doncaster, a category Bresettlement prison located inMarshgate, Yorkshire. The prisonhouses a male population of up to 1145 individuals, approximately 200 of whom are young offenders aged 18-20 years.
Within this role you will be working in receptions supporting the intake of new patients into the prison. You will work closely with the multi-disciplinary team including the substance misuse nurses, and mental health team to deliver high quality healthcare services.
Hours
Mon - Fri: 18:00-22:00
Sat: 13:00-17:00
This is a part-time role working 16 hours per week. While we offer individual 4-hr or a combination of sessions, candidates must be flexible to cover different sessions across the week.
Salary
The role is paying a salary of up to £125,693 pro rata per annum depending on experience.
