Job summary
The Community GP is the senior clinical decision-maker within the TeamUp integrated neighbourhood team. This is a frontline clinical role. The post holder is expected to be visible, accessible and present across the service's clinical settings on a daily basis, providing the medical leadership and decision-making capacity that enables the wider team to function safely.
Main duties of the job
Attend the daily TeamUp situational awareness meeting: caseload review, new referrals, urgent work and capacity
Workload allocation: home visits, identification of patients requiring senior input, triage of acute referrals
Community visits, ward rounds, home visit triage and direct integrated neighbourhood team clinical support
Board round for patients under the service
Follow-up visits, clinical advice, medication review, governance tasks, supervision and training
Responsive availability for urgent clinical advice and team queries until end of operational day
About us
Erewash Health Partnership (EHP) was established on 1 April 2018 and is a single GP Partnership comprising of six constituent practices (see below). EHP employs approximately 250 people who work within the constituent practices and central team.
As well as providing high quality core primary care services, EHP is the prime provider for delivering services to Erewash Primary Care Network (EPCN), as well as operating its own services for EHP Partners.
Erewash PCN consists of 11 practices and covers a population of approximately 102 ,000 patients.

Care Homes-
Holds clinical responsibility for patients on the TeamUp care home caseload

Job summary
We're looking for a Health Care Assistant to join our friendly, innovative, and busy team at Erewash Health Partnership, based in Long Eaton.
You'll become part of an established practice team, supporting our patients with a wide range of clinical services while helping us keep pace with the changing demands of primary care. Previous experience in a GP practice setting would be an advantage, but we welcome applications from candidates who bring the right skills, attitude, and willingness to learn.
You'll need to be flexible with your working hours, as the role includes covering early mornings, evenings, and occasional Saturdays in line with NHS England requirements.
Main duties of the job
Working under the supervision of our Lead Practice Nurse, you'll play a key role in delivering care and support across the practice, including:
- NHS Health Checks, phlebotomy, and urinalysis
- ECG recording and weight/height monitoring
- Wound and leg ulcer dressings
- Administering flu, pneumococcal, B12, and shingles vaccinations
- Chaperoning duties and supporting minor surgery/LARC procedures
- Diabetic foot checks and other QOF-related duties
- Smoking cessation advice and general health promotion
- Stock ordering, clinical room restocking, and infection control support
- Accurately recording patient consultations on the System1 clinical system

About us

Job summary

The role is pivotal in ensuring people are safely and appropriately transferred between care settings. Working within a multi-agency team, the postholder ensures timely and appropriate onward referrals to community health, social care, Primary care or voluntary sector services. This role involves multidisciplinary collaboration, and effective communication to avoid delays
Main duties of the job
Provide Coordination and navigation for the Local Navigation hub caseload across health and care services, alongside working closely with other Primary Care services.
Coordinate and manage the administrative functions of the Multi-disciplinary team meeting.
Take minutes of the MDT meetings and disseminate, chase progress against actions identified.
This is a pivitol role to the Local Navigation Hub and will be the interface between Primary Care, secondary Care, Community Care, Social Care, Mental Health, Out of Hours and Voluntary sector organisations.
