Job summary
We have invested in developing innovative and digital ways of working to support our clinicians and effectively manage patient demand.
Clinicians are welcome to develop their specialist areas of interest and we are always interested in taking on GP's who wish to develop new and more fulfilling ways of working, especially with a digital focus.
We are closely involved with our Primary Care Networks (PCN's) and take an active role in their growth and development. The exciting work we are able to do at a PCN level enables us to work at a community level to improve population health across our PCN's as well as improve the range of services our patients are able to access.
Accordingly the Job Description, Person Specification and the Employment Contract all reflect this new model of working.
Main duties of the job
- To provide personal medical services to the registered practice population and to ensure that targets are met in terms of activity and quality in line with the PMS / GMS / APMS or any other contract that may exist.
- To provide patient centred consultations and ensure that the patients are always provided with excellent health care.
- To contribute to the updating and development of the clinical policies and to ensure compliance with the CQC procedures.
- To contribute to and support the development of digital clinical pathways for the effective support and management of high quality and timely provision of patient care.
- To provide daily clinical supervision to ACPs, Nurses, ANPs and GP trainees at your site, as set out in the current Job Plan. Where you are a designated supervisor for an Advanced Care Practitioner or Trainee, additional protected supervision slots will be allocated and your clinic timetable adjusted accordingly
- To act quickly to protect patients from risk if the post holder has good reason to believe he/ she or a colleague may not be fit to practice by using the First 4 Health Group Whistle blowing policy.

Job summary
To assist in the day-to-day running of the organisation providing administrative, clerical, and reception support to the practice team.
The person appointed will demonstrate the ability to multitask, good timekeeping, organisation, efficiency, professionalism, and the ability to work both independently and as part of a team whilst supporting the delivery of practice policies and procedures.
The post holder will ensure practice targets are met by regularly contacting patients to attend any clinics run by the practice including, Smear tests, Diabetes, Vaccinations, New Patient Registrations, Blood Test Clinics, and by pursuing any other targets required by the practice.
Main duties of the job
Tasks would include:
All general office administrative work, assisting the medical secretary with all clerical duties, and providing front-line service to all patients and trust-related staff at reception
Greet all patients with a smile and remain polite and approachable at all times.
Provide whatever support may be required by the Clinical & Management team to enable them to effectively run and manage the surgeries, clinics & practice
Receive and appropriately direct all patients on arrival to the surgery.
Take telephone calls and messages from patients; deal with their inquiries and/or book appointments with clinicians, as appropriate whilst ensuring that the patients are served and their needs met.
Answer general patients and other inquiries
Make and amend appointments for doctors and other clinicians using EMIS Web or any other computerised system used by the practice.
Explain practice procedures to patients e.g. complaints procedures as and when required.
File /scan medical records/notes quickly, accurately & efficiently.
Maintain a tidy workspace and environment at all times - ensuring that you clear up after yourself before leaving.
Maintain full cooperation and communication between the doctors, nurses, and other practice staff regarding all practice matters.

Job summary
The post holder will be an experienced nurse, who, acting within their professional boundaries will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. CLINICAL ROLE:
The post-holder will:
Make professionally autonomous decisions for which he/she is accountable.
Deliver Immunisation & Cervical Smear programme for the practice
Be competent in dressings, suture removal and treatment of simple wounds and wound care.
Manage Chronic Disease e.g. Diabetes, CHD, within training competencies
Provide safe, evidence-based, cost-effective, individualised patient care.
Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care.
Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.
Refer patients directly to other services/agencies as appropriate.
Work with nursing, medical and health care assistant colleagues to ensure that National Service Frameworks (e.g. Coronary Heart Disease/Older People/ Diabetes /Mental Health) are being delivered.
Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery.
Be aware of and contribute to planning and delivery of practice-based commissioning.
Carry out audits and complete patient summaries.
Assess, plan, provide and evaluate nursing care to meet the care needs of individuals and groups in the practice population.
Assess, diagnose and treat specific diseases in accordance with agreed medical/nursing protocols.
Provide direct access to specialist nursing care for undifferentiated patients within the practice population.
Undertake diagnostic, health screening, health surveillance and therapeutic interventions within a broader health promotion/public health strategy.

Chair Customer Care, Clinical and any other meetings as required by the practice.
Promote a learning environment for patients, nurses and other health professionals.
Contribute to the planning and implementation of the teaching for existing staff, such as medical students, PN students, GP registrars, Health Care Assistants, within the Practice.

Maintain their professional registration.
Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained.

Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards.
Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation.
Work collaboratively with colleagues within and external to the practice.
Pro-actively promote the role of the PN within the Practice, and externally to key stakeholders and agencies.
