Job summary
The Experienced General Practice Nurse plays a pivotal role in delivering safe, high-quality, and person-centred care. This role is integral to achieving the objectives of the 2026/27 Network Contract DES, with a focus on continuity of care, health inequalities reduction, digital transformation, and long-term condition (LTC) optimisation.
Full-time (37.5 hours per week), or part-time (minimum of 20 hours per week) considered in line with service requirements.
This role is included within the extended hours provision and will involve working an evening on a rota basis.

Main duties of the job
The postholder will work as part of the existing practice nurse team on long-term condition management, champion public health and prevention, and embed population health principles within general practice.
They will support the development and supervision of new-to-practice nurses and other multidisciplinary staff, contributing to workforce capability across the Primary Care Network. The role also involves fostering integration with community, secondary, and social care systems to ensure proactive and joined-up care delivery.
The post holder will be working at registered nurse level practice as detailed in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.

Primary Care Networks (PCNs) were established in July 2019 as part of the NHS Long Term Plan to improve the quality and accessibility of care by encouraging collaboration among various healthcare services and community organisations.
One of 13 PCNs in Somerset, we are a collaboration of five GP practices in West Mendip: Glastonbury Health Centre, Glastonbury Surgery, Wells City Practice, Wells Health Centre and Vine Surgery Partnership (Street).
At Wells City Practice, you'll be joining a friendly, supportive team committed to providing high-quality patient care. Serving around 8,500 patients, the practice has a 'Good' CQC rating, high levels of patient satisfaction and strong QOF achievement.
Our multidisciplinary team includes six partners, two salaried GPs, a paramedic practitioner, nurses, HCAs, FCP, Clinical Pharmacist and an experienced administrative and management team. We are actively involved in training, research and collaborative working through our local Primary Care Network. We are seeking a professional, motivated, and compassionate Medical Receptionist to join our busy and friendly GP Surgery on a permanent basis working 24 hours a week.
The successful candidate will be the first point of contact for patients and visitors, providing a high-quality reception and administrative service to support the delivery of patient care.

Main duties of the job
The post-holder will provide clinical expertise as an integral member of the Wells Health Centre multidisciplinary team, to assess, diagnose, treat, refer or signpost patients/service users, who attend the surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury.
A paramedic is authorised to undertake treatments and procedures as directed by the GPs for which they are trained in, and which are within the Royal College of Paramedics guidelines and in line with the HCPC standards framework. The post holder will use advanced clinical skills to provide education to patients and service users, promoting selfcare and empowering them to make informed choices about their treatment.
The post-holder will also perform any other clinical and administrative duties, where trained and determined safe to do so by the partners. This may include assisting with medical reports, safeguarding, and clinical support.

Main duties of the job
You'll work directly with individuals and groups, offering time, space and practical support to help people identify what matters to them and access services, activities and resources that can make a positive difference. We are a six partner team and have a vacancy for a 4 - 6 session salaried GP. Working days will include a Thursday & Friday (4 sessions).
It's a great place to perform the demanding and yet highly satisfying role of a GP, with supportive colleagues and a wonderful practice team which altogether means we are well placed to navigate the ever changing world of primary care with autonomy and a sense of well being.

Main duties of the job
The post-holder will provide high quality general medical services to a personalised list of patients and work as part of the practice team to manage a range of health needs in a purpose built and modern primary care setting, ensuring the continuous delivery of the highest standards of care.
Your working day will follow the BMA guidance on safe daily contacts, with an established blend of pre-booked and same day face to face and telephone appointments.
The experienced multi-disciplinary team of clinicians collectively manage same day patient demand, removing the requirement for limitless duty doctor sessions.

We are a stable, supportive and very friendly training practice that has retained its family and patient-first ethos. We work from purpose-built premises that has been recently extended, in a colourful market town offering semi-rural primary care.
The practice is very successful and has a good reputation amongst the GP community both externally and internally as evidenced by strong staff retention.
We care for a registered population size of 14,000 served by 11 GPs, GP Registrars, Nurse Practitioners, and a team of Practice Nurses and HCAs, physiotherapists, administrative, secretarial and reception.
