Job summary
This post offers an exciting opportunity to join a highly motivated team of health professionals providing high quality sexual and reproductive healthcare across Worcestershire. The post holder will be re based at the Aconbury North, Worcester site, but required to also do some clinics at Kidderminster Sexual Health clinic as part of their weekly role. They may be also required to cover the Arrowside sexual Health clinic at Redditch should the need arise due to absences or other circumstances. to cover other sites if required.
Seven sessions/week - minimum 6 sessions per week substantive/permanent post.
The post holder will provide medical input to the Worcestershire Integrated Sexual Health service. The team is a countywide (Worcestershire) service that aims to provide a level 3 sexual health service
The post holder will be expected to have access to a car and hold a driving licence. Post holder will be expected to maintain good communication with wider healthcare professionals in the community e.g GPs, in the spirit of person centred and joined up care, within the bounds of confidentiality within Sexual health.
The post holder will be required to maintain a good standard of practice, including the assessments of patients, liaison with other professionals, record keeping, and other associated duties as directed.
Main duties of the job
To provide clinical services in sexual health, contraception and reproductive healthcare for patients with reference to national guidelines and within limits defined in locally agreed operating procedures
To be prepared to provide emergency contraception and counselling and referral of clients requesting abortion.
To provide non-judgemental counselling on all sexual health issues and to refer appropriately any service user. To provide counselling on all the available options to women presenting with an unintended/unwanted pregnancy. To work within the legal processes for abortion , in conjunction with the nurse-led early medical abortion service
To participate in service quality governance, audit and medical meetings across the county wide Sexual Health Service, as well as undertake departmental audit, as appropriate.
To be approachable, non-judgemental and responsive to client needs, irrespective of their age, gender, race, religion or sexuality. Ensure patient confidentiality is maintained at all times
To have a keen interest and to be responsible for keeping up to date
To provide in-clinic teaching, training and supervision of junior doctors, medical students, physician associate students, foundation year pharmacists and nursing students who come to observe in the department, once a sufficient level of experience is reached.

Job summary
As part of the reconfiguration of the Learning Disability (LD) CAMHS service we are recruiting a part-time (27 hours) Consultant Clinical/Counselling Psychologist to play a key role in shaping its future development.
The small LD CAMHS team prides itself on its flexibility and creativity in meeting individuals' needs and on its close collaboration with other agencies.
The team now sits under the umbrella of Learning Disability Services, which means that the postholder will have the direct support of the Trust Lead Psychologist in this area and sit in a structure alongside the two other Learning Disability Consultant Psychologists. This will ensure that their professional development will remain protected during this period of change.

Main duties of the job
The postholder will provide clinical leadership and oversight of the governance of the LD CAMHS service. They will work closely with the Lead Psychologist to shape service development in line with national drivers and good practice.
They will also provide clinical supervision and line management to a small number of team members and empower them to share the leadership responsibilities.
They will help to develop creative ways of working within the current national financial constraints to best meet the psychological needs of children and young people with learning disabilities and the systems around them. Working alongside key stakeholders they will help to bring psychological thinking to the wider network.
The postholder will also hold a suitable caseload of people with learning disabilities and a range of complex psychological needs.

Job summary
We would like to appoint a Clinical/Counselling Psychologist to work in the Adult Mental Health (AMH) Neighbourhood Mental Health Services (NMHS) in Worcestershire to particularly work with people who present with Psychosis and Bipolar. The NMHS teams have integrated psychological pathways for individuals who present with moderate/severe, severe/enduring, psychosis/bipolar and emotional regulation difficulties. The post holder will have the opportunity to input into teaching, training, research and supervision. We have excellent links with the local doctorate training courses and offer training placements.

Job summary
Mortimer Ward is a 20 bedded acute in-patient ward for adults with serious mental health problems, requiring inpatient care and treatment. Patients requiring admission may also have complex physical co morbidities and applicants should have an interest in both mental and physical health care delivery. Patients admitted can either be in formal or detained under the Mental Health Act and require a holistic care approach to work towards their Recovery.
The ward has a multi-disciplinary team approach to deliver holistic care, including nurses, physiotherapists, occupational therapist, activity support workers, Psychiatric Consultants and junior doctors. You will also work alongside colleagues in the Crisis Team and Adult Social Care. Mortimer Ward also welcomes and supports student nurses as part of our team.
Mortimer Ward operates a rotational shift pattern system over a 24 hours / seven day period. . The day shift starts at 07.30hrs till 20.00hrs and the night shift at 19.30hrs till 08.00 hrs. The post is full time but part time hours would be considered.

Main duties of the job
Delivering a high quality clinical service to patients and their families.
Acting as named nurse to a group of patients under the direction / supervision of the person identified with continuing responsibility.
Participating in therapeutic group work programmes.
Taking charge of the ward in the absence of the ward manager and deputy ward manager.
You will be responsible for the assessment of care needs and contributing to; the formulation of the person's needs and the development of a relevant plan based upon the person's negotiated goals and relevant evidence based interventions.
You will, in conjunction with the multi-disciplinary team, provide nursing interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical journey.

We are looking for a Clinical Specialist Discharge Facilitator to join our Inpatient Pathway Team. The team works across both Herefordshire and Worcestershire, supporting timely and effective discharge planning for patients within our inpatient services.
We welcome applications from candidates with experience of working in older adult mental health services and/or those with experience in discharge planning, patient flow, or related services.
This is a part-time role working 18.75 hours per week, based at New Haven Hospital, Bromsgrove. Due to the part-time nature of the post, the working pattern is negotiable. You will be expected to travel to other sites as required; however, travel expenses will be reimbursed in line with the Trust's expenses policy.

Main duties of the job
The Clinical Specialist Discharge Facilitator is responsible for leading and coordinating complex discharge planning processes for patients within Specialist Mental Health and Learning Disability inpatient services.
Working autonomously and collaboratively with multidisciplinary teams, community services, local authorities, commissioners, patients, families and carers, the post holder will facilitate safe, timely and least restrictive discharges that promote recovery and independence.
The role provides specialist expertise in patient flow, discharge planning, Section 117 aftercare and complex care pathways, identifying and addressing barriers to discharge and escalating risks where appropriate.
The post holder will act as an advocate for patients, support service development and quality improvement initiatives, provide expert advice and education to colleagues, and contribute to effective resource management across inpatient services to improve patient outcomes and reduce avoidable delays in transfer of care.

Main duties of the job
Lead and coordinate the delivery of the IPS service across Herefordshire and Worcestershire.

Job summary
Mental Health Support Teams in Schools (known locally as Wellbeing & Emotional Support Teams - WEST) are expanding across Herefordshire and Worcestershire. WEST works with schools to deliver early interventions for children and young people aged 5-18 experiencing mild to moderate mental health difficulties, using evidence-based, low-intensity CBT approaches alongside whole-school support.

We are expanding our schools in Hereford, and we are now recruiting two Trainee Education Mental Health Practitioners. Posts are at Band 4, with EMHP training commencing mid-September. On qualification and registration, posts become at Band 5.

Main duties of the job
As our ACP, you will provide senior clinical expertise and leadership within the Evening and Night Service, supporting nurses and wider teams to safely manage increasingly complex patients in their own homes.
You will:
Undertake advanced clinical assessments, physical examinations and diagnostic reasoning for complex and urgent presentations.
Prescribe within your scope and use appropriate PGDs, request and interpret investigations, and develop treatment plans to support safe care at home.
Assess and manage patients at risk of hospital admission, supporting admission avoidance and safe escalation or transfer of care.
Provide expert clinical advice to community nursing colleagues and work collaboratively with GPs, urgent community response, ambulance, acute and wider health and social care services.
Initiate and review ReSPECT documentation where appropriate.
Provide clinical leadership, education, supervision and professional support to colleagues.
Contribute to service improvement, innovation, audit and research, ensuring opportunities support the four pillars of advanced practice alongside clinical responsibilities.

You will provide a comprehensive administrative service to support the Neighbourhood Team. Main duties of the job
You will care co-ordinate a number of young people on your caseload, taking responsibility for assessments and therapeutic interventions.
You will carry out risk assessments appropriate for children and young people with mental health needs and learning disabilities and monitor and evaluate interventions using outcome measures.

We are looking for compassionate and experienced Band 6 Registered Nurses to join our new Worcestershire 24/7 Palliative and End of Life Care Advice Line service. This is an exciting opportunity to be part of a countywide service providing timely advice, telephone triage, symptom management guidance, reassurance and care coordination for patients deemed to be in their last year of life, their families, carers, care homes and professionals.
Working as part of an integrated model with our Single Point of Access (SPA), Evening and Night Nursing, Neighbourhood Team and Specialist Palliative Care services with strong collaborative working with Primary Care and other system partners, you will help people remain in their preferred place of care and reduce avoidable crisis escalation and avoidable Emergency Department attendances or hospital admissions.
The role is clinically focused and will be co-located with other clinicians with the ability to escalate to more senior clinicians where appropriate. There will be only one clinician covering this phoneline so the ability to deliver the service confidently is essential.

Due to significant service investment we are looking for enthusiastic Occupational Therapists to join our friendly Children's Therapy team in Worcestershire. Job summary

Main duties of the job
Holding a varied caseload completing OT assessments and interventions based on a sound knowledge of evidence-based practice and a range of treatment options. Including assessment for specialist equipment. Being able to identify children and young peoples OT needs and develop care plans using a goal setting approach.
Supervising and supporting other members of the therapy service including technicians and students.
Application of policies and legislation related to children, young people and families including safeguarding children.

We are looking for an organised, professional and approachable Administrator to provide support to our Evening and Night Nursing Team service in the delivery of a new Worcestershire 24/7 Palliative and End of Life Care Advice Line. Working arrangements:- working across multiple sites based from Newtown Hospital, Worcester workshop and travelling from home to site where job allocations required on a daily basis.

Main duties of the job
Reactive Works - include reactive and planned general building repair work jobs received as low to high priority tickets from CAFM mobile solution including such works as roof leak repairs, slabbing for fire exit safe egress, tarmac pot hole repairs (use of whacker plate machine), plastering, tiling, grouting, bricklaying repairs to damaged walls in car parks or building disability ramps; fence repairs, fence post concrete breaking, hole digging, cement mixing, installing new guttering, new fencing, temporary works security fencing - collection, delivery, putting up and down post incident; some carpentry - skirting, worktops, shelves.
Planned Maintenance Works - regular gutter cleaning, road & path safety inspections, draining works, fire escape clearance checks; 12 x monthly ladders & steps estate wide log update check (Jan); zip-up PASMA scaffold erection (training supplied).

The Assertive Outreach Team have an exciting opportunity for a 12 month secondment for a Band 6 registered mental health nurse or registered allied health professional with a passion and commitment to providing excellent, compassionate, trauma informed care as part of a supportive multi-disciplinary team?
The Assertive Outreach Team is a community based mental health team comprised of psychiatrists, psychologists, qualified nurses, OTs, nursing associates and support workers working from a staff base in the centre of Hereford.
We provide assessment and treatment for patients who experience severe and enduring mental health conditions who have historically struggled to engage in mental health treatment and who may have dual diagnosis with co-morbid substance misuse. We are recovery focused, trauma informed and work flexibly with our patients to meet their complex and changing needs.
You will be required to work 30 hours per week over a 9-5 Monday to Friday service. Flexible working is available. You will work with a supportive and valued team of professionals, receive regular supervision and have CPD and career progression opportunities.

We are seeking an enthusiastic and highly skilled Specialist Podiatrist to join our Multidisciplinary Diabetic Foot Care Team. Working closely with Consultant Diabetologists, Vascular, Orthopaedic and Diabetes specialists, you will provide expert assessment and treatment for patients with active diabetic foot disease.
You will play a key role in delivering high-quality, evidence-based care aimed at reducing hospital admissions, preventing complications and minimising the risk of lower limb amputation.
The role involves managing urgent referrals, ensuring patients with active foot conditions are assessed within 24 hours, undertaking advanced wound management, and providing specialist advice and education to patients and healthcare professionals.
This is an exciting opportunity for an experienced HCPC-registered Podiatrist to work within a dynamic specialist service dedicated to improving outcomes for people with diabetes.
Main duties of the job
As a Specialist Podiatrist within the Multidisciplinary Diabetic Foot Care Team (MDFT), you will provide expert assessment, diagnosis and treatment for patients with active diabetic foot disease, helping to reduce hospital admissions, prevent complications and minimise the risk of lower limb amputation.
Working closely with Consultant Diabetologists, Vascular and Orthopaedic Consultants, Diabetes Specialist Nurses and other healthcare professionals, you will manage complex and high-risk foot conditions across community and acute settings. You will be responsible for triaging and responding to urgent referrals, ensuring patients with active diabetic foot problems are assessed within 24 hours and receive timely intervention.
Your day-to-day duties will include advanced wound management, ulcer care, vascular and neurological assessments, offloading and orthotic provision, nail surgery under local anaesthetic, patient education and the development of individualised treatment plans.
You will act as an expert resource for colleagues and partner organisations, contribute to service development, support the supervision of junior staff and students. The role would include covering a one 3-hour session once every 18 months on a chosen Bank holiday (excludes Xmas day).

Job responsibilities
Provide specialist podiatry assessment, diagnosis and treatment for patients with active diabetic foot disease, utilising advanced clinical reasoning and evidence-based practice.
Manage complex caseloads independently and as part of a multidisciplinary team, ensuring safe, effective and patient-centred care.
Directly refer patients for appropriate diagnostic investigations and liaise with acute and community services to coordinate timely interventions.
Undertake advanced wound management, ulcer debridement, vascular and neurological assessments.
Assess the need for orthotic and offloading devices, providing prescriptions and liaising with Orthotics services on complex cases.
