Job summary
The Adult Community Rehabilitation Team provides rehabilitation for adults aged 18+ across City and Hackney. We deliver high-quality, person-centred care that supports people to maximise independence, achieve meaningful goals and improve their quality of life within their local communities.
We are seeking an experienced and motivated Band 7 Physiotherapist to join our Physical Conditions pathway. The role offers the opportunity to provide specialist assessment, treatment and rehabilitation for adults with complex physical health conditions.
Working closely with Band 7 colleagues and the wider multidisciplinary team, you will provide clinical leadership, contribute to service improvement and quality improvement initiatives, and support the development of innovative rehabilitation pathways. You will also supervise junior staff and students and work with service leads.
Applicants should have substantial community rehabilitation experience, excellent clinical and communication skills, experience of multidisciplinary working, strong leadership qualities and a passion for delivering high-quality patient care.

Main duties of the job
o Senior member of the specialist interdisciplinary Adult Community Rehabilitation Team which includes Occupational Therapy, Physiotherapy, Psychology, Rehabilitation Assistants and administrators support the team. The team works with clients aged 16 onwards with complex physical disabilities (including palliative, HIV and clients with mental health issues).
o Lead, manage and co-ordinate the provision of the Physiotherapy service within the Adult Community Rehabilitation Team.
o Undertake highly specialist physiotherapy assessment and intervention as an autonomous practitioner.
o The post holder will be working in a challenging socioeconomic environment involving clients who have highly complex social problems and those from culturally diverse backgrounds.

This post is part of our service to Special Schools, Complex Needs and Autistic children.
The current allocation for this specialist speech and language therapist post is:
- Ickburgh Special School
- Complex Needs Specialist Support (communication, AAC and dysphagia)
As a specialist speech and language therapist, in the future you may work across sessions in:
- Specialist Settings
- As well as some mainstream schools (most often with autistic and complex children).
It is an excellent chance to be part of an exciting and growing SaLT network within this clinical area raising awareness of SLCN and providing holistic, relevant and functional support to children, and the professionals around them. Preferences will be discussed at interview, and the exact combination of the post will be dependent on interest, experience and service need.
Main duties of the job
The sessions will as part of the Complex Needs, Autism and Special Schools service. In special schools this will be with primary and / or secondary/post-16 age students. Main duties across special schools and Complex specialist support include:
- Provide specialist assessment, diagnosis and intervention for children and young people with dysphagia, speech, language and communication needs across community, education and specialist settings.
- Work collaboratively with families, schools, nurseries and a multidisciplinary team to deliver integrated care and empower others to support children's communication and feeding needs.Lead and contribute to multidisciplinary planning and management of complex cases, ensuring high-quality, evidence-based practice.
- Support the development of the wider workforce through supervision, joint working, training and clinical education of therapists, assistants, volunteers and students.

This is a permanent, full-time post comprising 10 Programmed Activities (PAs) per week.
Main duties of the job
This post offers an exciting opportunity to join a progressive, collaborative and forward-thinking General Surgery department. The post has been developed to support service expansion and to ensure continuity in the delivery of high-quality elective care. We are seeking to appoint a consultant with prior Upper GI/Hepatobiliary training and anticipate that the successful appointees will be able to undertake a broad range of elective and emergency procedures from an early stage in their appointment.
The appointee will also be expected to contribute to the Emergency General Surgery (EGS) service as required to support the effective running of the department. They will undertake routine cholecystectomy and, where clinically appropriate, contribute to the delivery of 'hot cholecystectomy' as part of the department's acute biliary service.
In addition, the postholder will be expected to undertake a broad range of general surgical procedures in line with their experience and the needs of the service, including hernia surgery and the management of benign lumps and bumps.

Job summary
The post holder will join the RTT (Referral to Treatment) Validation Team within the Patient Administration Contact Centre, supporting a range of administrative and patient pathway activities.
We are looking for motivated, detail-oriented individuals with experience in RTT validation, patient pathway tracking, waiting list management and elective pathway standards.
The successful candidate will have a strong understanding of NHS RTT rules, consultant-led pathways, clock starts and stops, and validation processes. You will help ensure the accuracy of RTT data, identify and resolve pathway discrepancies, validate patient journeys and support the Trust's elective access standards.
Experience of PTLs, PAS systems, data quality processes and managing complex patient pathways is essential, alongside strong analytical, organisational and communication skills and the ability to work independently within a fast-paced environment.

We are seeking Radiographers to join our diverse and enthusiastic Radiology Team due to the continuing expansion of our Imaging Service. We provide Radiographers and Image Intensifiers to up to three theatre lists daily, including Trauma & Orthopaedics, Urology and Endoscopy. Radiographer rotation includes supporting Outpatient Clinics at a Healthcare Centre in the Olympic Park, Stratford. Main duties of the job

After gaining experience and competence in Plain Imaging, Fluoroscopy and DEXA you will undertake full CT training in order to progress into a Band 6 role.

The Transfer of Care Hub (ToCH) is based at Homerton Hospital, based within the Integrated Independence Team( IIT) and facilitates discharges across City and Hackney as well as surrounding areas. The purpose of the job role is to support the Transfer of Care Hub with Administration duties, being an integral part of the Hub.

Main duties of the job
Together with the team of Clinical Co-ordinators, manage patient information referred into the ToCH.
To provide first point of contact assistance/advice for referrers, health professionals and service users
Liaising with primary, secondary, health and social care providers, to facilitate service users into appropriate discharge pathway.
Assess client's referral discharge information, ensuring any inappropriate referral is rerouted to the correct pathway or missing information is gathered
To update all electronic systems in conjunction with ToCH Clinical Co-ordinators including referral email account, spreadsheets and Rio/EPR systems.
To assist with the production and analysis of data to produce reports supporting relevant KPIs and targets
To work with other teams, departments or local authority to facilitate timely, appropriate and seamless discharge from hospital into the chosen discharge setting.

Newborn Hearing Screening Programme is seeking highly enthusiastic, organised and methodical individuals to join our very busy and thriving screening team.
- This post is based at Homerton Healthcare and at outpatient clinics within Hackney and the City.
- You will work primarily with mothers and their newborn babies (0-7 days old) including premature babies and those that need Special and Intensive Care. Therefore, experience of handling babies is essential and working with children and families would be desirable.

Main duties of the job
- The post holder will act under the leadership and management of the NHSP Local Manager and be responsible for carrying out hearing screening on babies in both inpatient and outpatient settings as required.
- The post holder will also have shared responsibility in ensuring the recording, collection and data quality of all patient data for the service.
- The post holder will carry out the necessary administrative tasks associated with the patient journey.
- The post holder will be expected to work as part of a team, altering working patterns to accommodate the changing needs of the service, across 7 days a week, 365 days a year with regular working during weekends (Saturdays and Sundays), bank and public holidays including on Christmas day.

The RTT and Validation Performance Manager is responsible for leading RTT performance and operational delivery across key patient administration functions, including Outpatient (OPD) bookings, Surgical Bookings, and RTT Validation teams.
The post holder ensures that patient pathways are efficiently managed from referral through to treatment, with a strong focus on data accuracy, validation, and delivery of national RTT standards.

Job summary
The CAMHS Single Point of Access team are recruiting an Assistant Psychologist post at Band 4 within Community CAMHS.
This post holder will support the CAMHS Single Point of Access by screening the incoming referrals, tasking them to the correct clinician for thorough triage, triaging some referrals, and completing any follow up actions for families and young people.
The post holder will also support with quality improvement projects on SPA including developing our signposting guide, obtaining feedback about the SPA service from young people and families, and developing and maintaining relationships with our community partners, including all the local CAMHS Teams (First Steps, Specialist CAMHS, CAMHS Disability), our 16-25 year old services (Off Centre and Growing Minds), our other NHS Children's Services (Occupational Therapy, Physiotherapy, Speech and Language Therapy), and our voluntary sector organisations in Hackney.
We will provide opportunities for shadowing and joint working with our CAMHS teams depending on the learning and career goals of the post holder.
Main duties of the job
The post holder will:
- Conduct and evaluate outcomes for duty by being directly involved at every stage of the process including screening.
- Duty tasks involving liaison between all clinicians and staff to ensure referral process are scheduled to fit with service targets
- Attend and contribute to team meetings, involving recording relevant information for the purpose of tracking referrals, maintaining robust data collection and overseeing assessment/triage clinic operations.
- Be responsible for data entry and RiO progress notes
- Support the Community CAMHS Service in developing clinical resources for referrals, triage, duty, initial assessments and risk assessments.
- Work in partnership with clients and colleagues, maintaining respect for differing opinions and discussing all appropriate options for ways of working together effectively.
- Work flexibly (independently, with MDT members, wider systems/agencies) to keep duty and the referrals process running smoothly.
- Assist with research projects, audit activities and teaching.
- Undertake research and personal development activities appropriate to the post, under the guidance of the line manager.
- Assist qualified staff in setting up and co-facilitating group interventions as appropriate.

Community CAMHS is part of Community Services at Homerton University Hospital. Community CAMHS is situated within Hackney Ark. The Hackney Ark is a specialist multi-disciplinary service for children and young people with developmental delay and disabilities in Hackney and the City of London.
Community CAMHS Consists of three teams: First Steps (an early intervention psychology team), CAMHS Disability (a psychology team for young people with moderate learning disabilities and autism), and CAMHS Single Point of Access Team.
The CAMHS Single Point of Access Team triages all of the referrals to CAMHS for Children and Young People across the borough of Hackney. The Single Point of Access Team works closely with our Hackney Specialist CAMHS Team (East London Foundation Trust), and other voluntary sector organisations in Hackney.

This is a 9-month fixed-term contract (FTC) working 28.5 hours per week. There is potential for the post to increase to up to 36 hours per week, depending on service needs.
An exciting opportunity has arisen for an experienced Speech & Language Therapist, Dietitian, Occupational Therapist or Physiotherapist with excellent leadership skills, sound multi-professional management experience and extensive community rehabilitation experience to undertake the role of Deputy Head of Service in the Adult Community Rehabilitation Team (ACRT). This role works in close partnership with the existing Deputies to manage both the physical and neuro clinical pathways in the team. Physiotherapy and Occupational Therapy leadership is currently well represented so we would particularly welcome applications from Speech & Language Therapists and Dietitians.
ACRT provides therapy and rehabilitation for clients with neurological and physical disabilities and the team consists of AHPs, Clinical Neuropsychologists and is supported by a Consultant Neurologist, AHP Consultant in Neuro Rehab and Neuronavigators.
The post holder will work closely with the management team to deliver quality community rehabilitation services aligned with Homerton's strategic objectives, including Neighbourhoods, productivity and digital optimisation.
Professional development is supported and there is a strong commitment to QI, research and audit.
Main duties of the job
- Senior member of the specialist interdisciplinary Adult Community Rehabilitation Team which includes Occupational Therapy, Physiotherapy, Speech & language Therapy, Dietitians, Neuro-Psychology, Clinical Psychology, Consultant Allied Health Professional, Consultant Neurologist, Support workers and Administrators. The team works with clients aged 16 onwards with complex physical and neurological disability.-
- Assist the Head of Service in the development of the service in line with local and national directives.
- Act as an expert therapy practitioner in the field of community rehabilitation for the Trust and City & Hackney partners
- Provide expert training, advice and education to Allied Health Professionals, GPs, Consultants and members of the voluntary sector and the Local Authority.

Homerton Hospital provides general hospital and community services to Hackney and the City of London, and specialist care in obstetrics, neonatology, fetal medicine, fertility, bariatric surgery, and neuro rehabilitation across east London and beyond. The comprehensive range of geriatric services currently provided by HUH is as recommended by the National Service Framework for Older People and the British Geriatrics Society. Multidisciplinary team working is strong, with senior physiotherapists, occupational therapists, and social workers dedicated to elderly services. There is close liaison and collaboration with all departments, and formal links with orthopaedics, old age psychiatry, neurology, and community teams.

Main duties of the job
o To work collaboratively and pro-actively with all appropriate clinical colleagues within the Trust in developing and implementing an acceptable service model reflective of best practice

Homerton Healthcare NHS Foundation Trust (HH) is an acute hospital located in the east London Borough of Hackney. The current Chairman is Sir John Gieve (Mary Elford will take over from 31st March 2025), and the Chief Executive is Ms Basirat Sadiq.

Job summary
This is an exciting opportunity for you to join Homerton Healthcare as a Band 3 Phlebotomist.
We are looking for a highly motivated Phlebotomist's, with experience of evacuated blood-letting systems. We have the Phlebotomy Service running in the Main Outpatients Department, Antenatal, wards, community, children and other clinical areas. Collecting blood samples in accordance of electronic EPR/tQuest system and request forms procedures.
This person must be punctual, reliable, a team player, have willingness to learn and good communication skills.
This person must be flexible and able to work at weekends (this will be on a rota basis). We are hiring for 1 full-time (37.5 hours) Phlebotomist

Main duties of the job
To obtain blood specimens using evacuated blood technique from wards, community and outpatients in an extremely busy clinical area, and other areas.
This job involves dealing with members of the public and liaising with other health care professionals. Time management is essential due to the volume of patients that will attend the phlebotomy service. Computer skills are desirable but not essential.
You will need to be compliant of all trust values and be able to provide examples on Infection Control policies and procedures.
o To take blood samples from patients using both needle and syringe and evacuated blood systems.o To ensure that work areas are clean and tidy at all times.o To take samples to the pathology department as necessary.o To be familiar with all equipment used.o To be familiar with different tests performed and ensuring that correct containers are used.o To ensure that all patient information is correct and specimens are labelled correctly and appropriately.o To inform the appropriate person if the blood sample has not been collected.o To work as part of the team, providing support and assistance to colleagues were necessary.o To ensure that all patients and colleagues are treated fairly, courteously and with respect at all times.o Any other reasonable duties as specified by the Supervisor or Service Manager.o To liaise with all other health care professional as necessary.
