The successful candidate will work as an autonomous practitioner within the multidisciplinary GP practice team, providing high-quality nursing care to our patient population. The role will involve delivering a broad range of general practice nursing services, health promotion, preventative care and chronic disease management.
We are looking for someone who is committed to providing excellent patient-centred care, works well as part of a team and is confident working independently within their professional scope of practice.
Main duties of the job
As a Practice Nurse, you will provide a wide range of nursing services to patients of all ages, helping to promote health, prevent illness, and support the management of long-term conditions.
- Deliver high-quality nursing care within a busy GP practice.
- Carry out health assessments, routine examinations, and chronic disease reviews.
- Administer immunisations, vaccinations, injections, and medications in line with current guidelines.
- Provide cervical screening, wound care, dressings, and other treatment room services.
- Support health promotion by educating patients on healthy lifestyles, medication, and self-management of long-term conditions.
- Maintain accurate and confidential patient records using the practice's clinical system.
- Work collaboratively with GPs and the wider multidisciplinary team to provide coordinated patient care.

We are seeking two Assistant Operations Managers to join our Integrated Urgent Care (IUC) team. We have one vacancy based in Farnborough and one vacancy based in Hook. SDC provides services through the Integrated Urgent Care (IUC) contract which covers the in Hours Clinical Assessment Service (CAS) and Out of Hours (OOH) provision for North Hampshire, North East Hampshire and Farnham & Surrey Heath places which are part of the Hampshire and the Isle of Wight ICB and Surrey & Sussex ICB respectively. We also provide an Urgent Treatment Centre at the front door of Frimley Park Hospital whilst also holding some smaller contacts supporting Community Hospitals OOH and Flu response. Finally, SDC has a virtual care arm supporting patients at home.
TalkPlus provide NHS Talking Therapies, supporting people with common mental health problems in Northeast Hampshire and Farnham place. TalkPlus offers a range of treatments to people who are experiencing symptoms of stress, anxiety, low mood, depression, panic, phobias or OCD. TalkPlus has care pathways to provide timely treatment for perinatal and military veteran referrals.
