We are looking for a motivated, compassionate, and enthusiastic Community Support Practitioner Worker to join our Urgent Community Response and Intermediate Care Team across Scarborough and Ryedale Community Services.
This is a rewarding opportunity to make a real difference to patients' lives, working alongside a multidisciplinary team of Physiotherapists, Occupational Therapists, and Nurses. You will play an important role in supporting rehabilitation programmes and functional activities, helping individuals regain confidence, maximise their recovery, and remain independent within their own communities.
You will work with people experiencing a wide range of medical conditions and pathologies, providing support during a period of rehabilitation aimed at improving mobility, functional ability, and independence.
In return, you will receive regular supervision, access to ongoing learning and development opportunities, and a comprehensive induction programme designed to help you feel supported and succeed in your role.
Main duties of the job
Day to day, the role includes the following care carried out in the patient's own home with supervision/delegation oftherapists/registered nurses:
- Patient care following a fall
- Moving & handling of patients
- Vital observations
- End of life care and urgent support for people living with frailty
- Recognise and escalate issues of concern and/or safety
- Assisting with activities of daily living, e.g., personal care tasks, meal preparation following injury/illness to prevent hospital admission- supporting and monitoring mobility, and rehabilitation programmes which include aspects of nursing, occupational Therapy and Physiotherapy.
- Urgent equipment provision, supporting and guiding the patient and carers with the safe use of this

An excellent opportunity has arisen for a an enthusiastic and dedicated HCPC registered Band 5 Occupational Therapist to join our established, friendly and supportive team within the Hull and East Riding of Yorkshire Community Team for Learning Disabilities.
As part of the CTLD you will work as a member of the multidisciplinary team alongside Speech and Language Therapists, Psychologists, Physiotherapists and Learning Disability Nurses. We also work closely with our social care colleagues.
We will support you with specialist training and supervision to reach your potential and excel in your chosen career path. Experience in equipment and adaptation, moving and handling and supporting people to engage in meaningful occupations would be an advantage.
Main duties of the job
Main duties of the job
To follow the Occupational Therapy Process (information gathering, assessment, treatment planning, intervention, evaluation and discharge planning) and contribute to care planning process as a member of the multi-disciplinary team.
To manage own caseload.
To assess capacity, gain valid consent and work within legal framework appropriate to service area.
To offer education, advice and training to informal and formal carers to best support client safety.
Ensure patient documentation is accurate, maintaining complete and contemporaneous records in line with HCPC/RCOT and Trust standards.
Promote the involvement and engagement of patients/service users and carers /relatives.
Ensure understanding of own and others level of accountability and responsibility when delegating tasks and duties to students and support staff.
Ensure that practice is within scope of own ability, knowledge, skills and experience. To ensure that practice is within the RCOT Code of Conduct and professional standards as well as Trust standards and policies.
Provide occupational therapy advice to other disciplines
Undertake the practice educators course to supervise OT students.
May participate in departmental research

Newbridges is looking to recruit a Band 5 Nurse, to join an established Multi-Disciplinary Team. Newbridges is an 18 bedded male inpatient unit offering service users intensive, high quality, person-centred interventions and assessment utilising a trauma informed model/approach.
This aids decisions to provide further support in the community or signpost to treatment units for further intervention.

Main duties of the job
- Reporting to the Charge Nurse, the post holder will work as part of a multi-disciplinary team, co-ordinating with psychiatrist, psychologist, occupational therapists, health care assistants, and allied professionals, to help individuals with specific symptoms of mental illness, and supporting them to overcome challenges.

The CAMHS Crisis and Intensive Home Treatment Team provides a 24/7 community crisis service for young people up to the age of 18 years. The team works long day, long twilight, and night shifts to support this service. The team also provides intensive intervention, home treatment, and in-reach support within the CAMHS inpatient and acute hospital settings.
The team includes a Clinical Team Leader, Clinical Leads, an Advanced Clinical Practitioner, a Consultant Psychologist, a Specialty Doctor, Specialist Nurses/Practitioners, Registered Nurses, Nursing Associates, and Healthcare Assistants.
Main duties of the job
The successful candidate will have strong engagement skills, excellent communication skills, and some experience of working collaboratively with other agencies and professions.
The successful candidate will work as part of a multidisciplinary team and undertake the crisis assessment of young people, develop care plans, and provide psychological interventions in relation to the young person's presenting need. They will also lead and supervise interventions provided by junior staff.
The CAMHS Crisis and Intensive Home Treatment Team works closely with our CAMHS Inspire Inpatient Service and will support young people whilst they are inpatients to facilitate early discharge. Therefore, the successful candidate will be provided with additional training with regard to inpatient working to support this joint working.
Experience of working with young people with emotional or mental health difficulties, or working in a mental health urgent care, crisis or intensive setting, is essential.

An exciting opportunity has arisen for a Band 7 Speech and Language Therapy Team Leader to work within the Expert at Hand model. The role is for 22.5 hours per week, and support will be provided by a team of colleagues who are committed to delivering timely, high-quality and innovative care.

This role will mainly be Monday to Friday, 9.00am to 5.00pm. Some flexibility in working days and hours is negotiable, while maintaining a balance between service requirements and your work-life balance.
The successful candidate will be based on the Townend Court inpatient unit; however, they will also work across the Intensive Support Team (IST). The Townend Court Inpatient Unit and the IST work collaboratively, and the successful candidate will be pivotal in strengthening these working relationships.
This role provides the opportunity to develop skills and experience across two closely linked services while ensuring continuity of care for patients as they move through the Learning Disability Pathway. The role will also provide the opportunity to gain a wider understanding of the patient and family/carer journey.
Main duties of the job
- Manage a caseload of inpatient service users with complex needs, using evidence-based, client-centred principles.
- Assess, plan, implement, and evaluate interventions to enhance the quality of life for patients. Interventions will include psychosocial and physical health/equipment interventions, as well as more specialist interventions, such as the use of sensory-based approaches.
- Use standardised and non-standardised assessments to identify patients' care needs and inform intervention planning.
- Assess for and prescribe appropriate equipment and provide advice on moving and handling.
- Contribute to wider MDT working and interventions, including Positive Behaviour Support.
- Participate in the planning, development, and evaluation of Occupational Therapy services. Provide outcome measures for the service and service users using agreed tools and assessments.
- Contribute to the development and maintenance of Occupational Therapy services within the Trust. Ensure Occupational Therapy provision is delivered in accordance with HCPC and Royal College of Occupational Therapists (RCOT) requirements.

An exciting opportunity has arisen for an experienced and innovative pharmacist to join Humber Teaching NHS Foundation Trust as a Specialist Clinical Pharmacist & Independent Prescriber within Adult ADHD and Mental Health Services.
This role combines specialist clinical pharmacy practice in ADHD Services with support for Mental Health Services. Working within multidisciplinary teams, the post holder will provide medicines optimisation, independent prescribing and person-centred care.

The post holder will be responsible for delivering specialised clinical care to their own caseload and service within their speciality area. This role will predominantly involve assessing children and young people for Attention Deficit Hyperactivity Disorder (ADHD) in line with NICE guidance.
Please note that this role will primarily involve assessing young people virtually.
You will have the opportunity to support the Clinical Team Lead in developing the ADHD assessment pathway within the team, sharing specialist clinical knowledge, providing clinical supervision to peers, and developing strong links with our ADHD intervention pathway. Whilst this role mainly focuses on ADHD assessments, the position sits within the wider Children's Neurodevelopmental Team, and knowledge and expertise in autism are also desirable.
The post holder will work in accordance with professional and regulatory body codes, standards, and guidance, always ensuring that their practice is grounded in evidence-based theoretical and practical knowledge.
The team includes a Clinical Team Leader, Consultant Doctors, an Advanced Clinical Practitioner, a Speciality Doctor, Pharmacists, Specialist Nurses/Practitioners, Registered Nurses, Nursing Associates, and a Healthcare Assistant.
Main duties of the job
- Utilise specialist clinical skills to complete neurodevelopmental assessments (ADHD), support diagnostic decision-making, and provide feedback to young people and their families.
- Recognise the need for, initiate, and, where appropriate, interpret relevant diagnostic investigations.
- Recognise the need for, and where appropriate seek, advice from relevant specialisms.
- Liaise closely with other members of the MDT to ensure that a detailed and comprehensive diagnostic assessment is completed in line with NICE guidance.

An exciting opportunity has arisen for a dynamic individual to join the Virtual Frailty Ward as an Advanced Complex Care Practitioner, providing care across Scarborough, Whitby and Ryedale. We are looking for a skilled, organised, and dynamic Nurse - AHP- with community experience to recruit as an Advanced Complex Care Practitioner (ACCP). You will need to be hard-working, patient focused, passionate, driven, with a problem-solving approach to patient and team management.
You will work in an integrated multidisciplinary team, including community nursing, therapies, also working closely with acute care, social care, GPs and voluntary sector colleagues to deliver individualised, safe and effective, high-quality care at home.
The post holder will proactively identify service users' needs, be able to quickly identify a deteriorating patient, have knowledge and experience of caring for those with complex long-term conditions within their home environment understanding the experiences of patients and their families. With the overall aim of supporting patients to remain at home or in their normal place of residence, avoiding unnecessary hospital admissions. Also facilitating early discharge where patients have been hospitalised.
You will work as an autonomous practitioner and will be confident working collaboratively with colleagues across a wide range of professions and organisations.
Main duties of the job

There will be an excellent induction program and clinical supervision support. ACCPs manage a complex caseload of service users providing:
- Robust clinical triage identifying appropriate referrals from across multiple partner organisations and through agreed clinical diversionary pathways.
- Advanced holistic clinical assessments, person-centred care plans, treatment escalation plans, and complex treatment in the community setting.
- Coordination of the integrated multidisciplinary team and liaison with partners in wider health, social care and voluntary sector to support admission avoidance where clinically safe
- Assist the service user to navigate health and care systems to achieve successful outcomes
- Support technology focused patient care, where appropriate for patient care to be safely and effectively at home

We are seeking a motivated and compassionate Assistant Psychologist to join our multidisciplinary team. This is an excellent opportunity for a psychology graduate looking to develop clinical experience within dementia care, cognitive assessment, and older adult mental health.
About the Role
The post-holder will work under the supervision of a qualified Clinical Psychologist to support the assessment, diagnosis, and management of people experiencing memory difficulties, including those with suspected or diagnosed dementia. You will contribute to high-quality, person-centred care for patients and their families.
About You
A degree in Psychology (BPS-accredited) is essential.
Experience working with older adults, individuals with cognitive impairment, or in a mental health setting is desirable.
Strong communication, empathy, and organisational skills.
Ability to work flexibly within an MDT and manage sensitive information confidently.

Main duties of the job
The post holder will contribute to a range of key clinical and administrative activities, including but not limited to the following:
To support the delivery of high-quality psychological services within Maister Lodge inpatient ward by assisting with cognitive assessments, providing pre and post-diagnostic support, and contributing to service development under the supervision of a qualified Clinical Psychologist.
Provide feedback to patients and families following multidisciplinary meetings.
Support care planning and signpost to appropriate interventions and community resources.
Assist with audits, research projects, and service evaluations.

Job summary
We are looking to recruit a Registered Nursing Associate at our Older People's mental health inpatient ward for adults 65 and over admitted for assessment with an organic mental health diagnosis.

Working hours are shifts consisting of Early, Late, Long Day and Nights although currently, our nursing associates do not work night shifts.

- The registered Nursing Associate will work as part of the Nursing workforce supporting Registered Nurses to deliver high quality, person centred, compassionate care to patients.
- They will be part of the multi-disciplinary team delivering planned care to a group of patients working within the standards of proficiency outlined by the Nursing and Midwifery Council and in accordance with Trust policies and protocols.

An exciting opportunity has arisen for the role of a band 6 Specialist Practitioner to the Crisis Intervention Team for Older People (CITOP) due to the expansion of the team's current role. The CITOP team supports adults predominately aged over 65 in the Hull and East Riding area with both functional and organic illness.
The CITOP team provide 24-hour Crisis intervention and Home treatment for older adults across Hull and East Riding of Yorkshire. CITOP will be provide four Community Crisis Beds for older adults as an alternative to inpatient care and The Acute Community Service will be extended to provide a seven-day service.
Due to the development of the CITOP service we are looking for professionals who are keen to lead on innovation, developing robust care pathways and enhance the patient and carer experience.
We are seeking individuals who are motivated, enthusiastic and have experience of working with people who have complex needs and enduring mental illness. This role requires good organisational skills, being able to demonstrate an ability to prioritise and delegate work effectively, and an interest or specialism in urgent and emergency mental health care, providing expert clinical advice and support and provide leadership and guidance on shift.
Main duties of the job
- The main features of this role will include the utilisation of the specialist knowledge of Older Adult Mental Health care to provide crisis triage and assessments , planning and delivering a variety of Crisis interventions, whilst reviewing care and planning for discharge within the community setting across Hull and East Yorkshire.
- You will provide Home Treatment & Acute Community Service interventions as an alternative to hospital admission or support timely admission to and discharge from inpatient settings. You will be required to collect/collate information and attend formulations for patients working in collaboration with others to ensure the best care for service users. You will be working within a Multi-Disciplinary Team maintaining quality standards and patient safety, within a framework of clinical governance.
- You will have the opportunity to develop leadership skills, providing supervision to healthcare staff. You will lead and participate in the development of staff through appraisal and clinical supervision.
- The applicant will be expected to have knowledge in mental health conditions, including Dementia and experience in this area would be an advantage, additional training and support will be given.

There has been an exciting opportunity for a full time specialist occupational therapist to join the Humber teaching NHS foundation trust inpatient service based at Millview Court treatment ward.

Millview Court is a 15 bedded mixed-sex ward, based in Cottingham, and provides 24 hour care to adults experiencing a crisis in their mental health.
We are looking for a motivated, compassionate individual, the successful candidate will be required to have the specialist level of experience, knowledge, and clinical skills to conduct evidence-based OT Assessments and Treatment plans to promote the service user's occupational identity and performance skills to enable their recovery.
The successful applicant will work collaboratively with the multi-disciplinary team, provide clinical supervision to coordinate and develop the therapeutic activities offered across the service, as well as being a part of the OT inpatient Hub supporting and promoting positive changes across the services.
The Model of Human Occupation (MOHO) and relevant assessments and outcome measures are used to frame and guide clinical practice, therefore the successfully applicant would benefit from having previous knowledge and experience of the model and assessment tools.
Main duties of the job
- To provide specialist Occupational Therapy input into Millview Court.
- Service Development and Leadership, Lead on OT input within the team, contribute to service improvement initiatives, and support implementation of Trust and national policies.
- Training and Education, Plan and deliver specialist training sessions for staff and students, and support professional development across the team.

Working alongside the Team Leader and fellow Clinical Lead, you will play a key role in shaping the future direction of CITOP, supporting innovation, quality improvement and the continued delivery of safe, effective and compassionate care.
Main duties of the job
- Provide clinical and professional nursing leadership across CITOP and the Acute Community Service.
- Support staff managing complex mental health presentations and clinical risk.
- Promote excellence in clinical practice and professional standards.
- Contribute to service development, quality improvement and innovation.
- Support the ongoing development of crisis assessment, triage and intensive community intervention pathways.
- Support the delivery of effective alternatives to hospital admission where clinically appropriate.
- Provide supervision, support and development opportunities for staff.
- Work collaboratively with colleagues across community, inpatient and urgent care services.
- Support the development of pathways that improve patient experience, continuity of care and integrated working.

Job summary
An exciting opportunity has arisen to join the CAMHS Crisis and Intensive Home Treatment Team (CCIHT) as a Band 3 Healthcare Assistant within Children and Young People's Intensive Mental Health Services (CYPIMHS).
The post holder will support children and young people experiencing mental health crises, emotional distress, and complex mental health difficulties, providing practical, therapeutic, and recovery-focused interventions under the supervision of registered practitioners. The role includes supporting telephone enquiries, gathering information to assist clinical decision-making, delivering agreed therapeutic interventions, monitoring wellbeing, and maintaining accurate clinical records.
As part of a flexible CYPIMHS workforce, the post holder may be required to support service delivery across community and inpatient settings, including the CAMHS Crisis and Intensive Home Treatment Team, Inspire CAMHS Inpatient Services, and Family Day Services. Appropriate induction, training, supervision, and competency development will be provided. Opportunities for rotational development within inpatient services may also be available.

Main duties of the job
The Healthcare Assistant will support children, young people, and families experiencing a mental health crisis and emotional distress as part of a multidisciplinary team.
Key duties include delivering planned care and therapeutic interventions under the direction of registered practitioners; providing practical and emotional support; monitoring wellbeing and reporting concerns; gathering information to support clinical decision-making; responding to telephone enquiries and referrals; and maintaining accurate clinical records.
The post holder will contribute to handovers, team meetings, supervision, training, and service improvement activities, while working within safeguarding, confidentiality, and professional boundaries.
As part of a flexible CYP Intensive Mental Health Services workforce, the post holder may be required to provide support across community and inpatient services, including the CAMHS Crisis and Intensive Home Treatment Team, Inspire CAMHS Inpatient Services, and Family Day Services. Training, induction, and competency support will be provided, with opportunities for rotational development across services.
