As a Healthcare Assistant, youll be a vital part of a multidisciplinary team delivering holistic, person-centred end of life care. Working under the guidance of registered professionals, youll support patients and those important to them with personal care, symptom management, and emotional support.

Youll contribute to care planning, assist with daily living activities, and help maintain a safe and dignified environment. This is a compassionate and hands-on role where youll make a meaningful difference in peoples lives during their final stages, while also supporting their loved ones through grief and bereavement.

• NVQ Level 3 in Health and Social Care OR an equivalent alternative Level 3 Qualification
• Recent experience in caring for and supporting patients at the end of life.
• A passion, knowledge and understanding of how to deliver person centred care
• A clear, appropriate and sensitive communicator who can make informed decisions in a timely manner
• Someone who understands the importance of prioritising a patients care, comfort and privacy
• Experience working within a Multidisciplinary team, assisting in curating individual assessments and advanced care plans.
• Knowledge of IT in particular Microsoft Office and electronic health record systems.
