Are you an Employment Specialist looking to make a difference in peoples lives? Do you wish to help people who are under our Secondary Mental Health Services get back into paid employment? If so you're in luck! Midlands Partnership University NHS Foundation Trust have an opportunity to join our Step On team for a 12-month fixed term contract,
You will champion of the Trust's Individual Placement and Support (IPS) Service via the Step On Team and effectively manage a caseload of 20-25 people in receipt of secondary mental health services, in order to facilitate individuals to gain and retain paid employment and/or engage in Higher Education.
A key to success will be enabling these clients to successfully make the transition into the labour market and reducing the number of service users who are not engaged in any form of vocational activity. You will work directly with mainstream employers to secure employment opportunities for service users whilst providing on-going support according to needs therefore to enable people who have experienced mental health problems to gain and retain education/employment.
You will be trained in Individual Placement and Support (IPS) or be willing to train to diploma level.
Main duties of the job
As an Employment Specialist you will effectively manage a caseload of people in receipt of secondary mental health services.
As parts of this role, you will be expected to:
- Receive complex and sensitive information in the form of referrals from Care Coordinators, to meet regularly with Case Managers to co-ordinate and integrate vocational services into mental health treatment.
- Communicate effectively with clients, giving empathy when disclosing sensitive or contentious information, providing reassurance, training and the motivation to move forward and to overcome any barriers to understanding.
- Understand a range of work procedures and practises following the Centre for Mental Health's intensive IPS Training course, Motivational Interviewing training and benefits advice programme.

We are offering an exciting and unique new role as a Band 4 Health Care Support Worker within our Integrated Mental Health Teams (IMHT) across South Staffordshire, supporting our new First Transitions Pathway.
The First Transitions Pathway is a new service to help support individuals with complex mental health as they move from the structured support of forensic services to the community-focused care offered by our Integrated Mental Health Teams. This transformation is underpinned by a central commitment to promoting sustained recovery, fostering meaningful integration within the community, and empowering service users to lead fulfilling lives. This specialised team will work in close collaboration and partnership with the six IMHTs spanning South Staffordshire.
Please be ware this role requires you to travel over a large geographical area, so access to suitable transport is essential.
Main duties of the job
You will be instrumental in providing practical and emotional support directly to service users, both during the immediate transition phase and in the crucial weeks and months that follow. Their focus will be on building rapport, implementing effective engagement strategies, and facilitating access to community resources.
As part of this role you will be expected to:
- Provide intensive and assertive outreach to service users at risk of disengagement, aiming to prevent relapse.
- Provide practical support within community settings, addressing immediate needs and implementing engagement strategies.
- Be curious and motivated to make a difference to this cohort of individuals.

You will support the provision of the dispensary, ward and community based services to enable the smooth, seamless and efficient supply of medicines across the Trust. We are looking for a highly motivated and autonomous Specialist Occupational Therapist to join our well established and evolving North Staffordshire Integrated Community Stroke Service (ICSS) giving you the opportunity to help drive developing and implementing high quality stroke rehabilitation care pathways and multidisciplinary training plans.
This is a 9 month contract for 30 hours to cover a secondment.
You will actively contribute to working in partnership with our patients, care partners and building a culture of research and development. Our service is transformational with the post holder requiring a level of NHS and stroke rehabilitation experience, high emotional intelligence and confidence to support with helping us drive development of our ICSS model.
We are based at the Haywood Hospital with our community service working across a 7 day shift pattern. Due to the nature of this role, you will be required to travel independently and efficiently over a large geographical area .
Main duties of the job
As the Specialist Occupational Therapist you will be responsible for the management of a complex stroke caseload, which includes specialist assessment and evaluation (within service specification), goal setting and care planning in conjunction with patient and care partner.
As part of this role you will be expected to,
Deliver a range of therapeutic interventions following specific occupational therapy and interdisciplinary assessments including goal planning and evaluation within a biopsychosocial approach to maximise an individual's abilities.
Develop effective communication and delegation to team members and wider professionals within service specified processes to meet the needs of the individual.
Complete and contribute to robust risk assessments & management plans promoting positive risk taking to improve quality of life and maximise occupational and functional performance/independence.
Maintain effective service and clinical governance reporting systems including contemporaneous paperwork, safeguarding and incident reporting mechanisms.
Support in the provision of formal and informal training of pre/post registration students, staff members, carers and service users.
Responsible for participating in quality driven processes, research and service development opportunities.

An exciting opportunity has arisen for a Band 6Community Mental Health Nurse in our Adult Community Mental Health Team based in Tamworth.
The team in Tamworth is multidisciplinary and provides high-quality care for service users that experience mental health conditions. The team is part of the wider Integrated Mental Health Team covering Lichfield & Burntwood and links together for CPD events, supervision and partnership working.

The South Staffordshire Perinatal and Maternal Mental Service is a specialist service delivering person centred care to expectant and new mothers who are experiencing moderate to severe and complex mental health problems within the perinatal period, or families who have mental health needs associated with loss and trauma occurring in the maternity and neonatal context.
Due to recent staff changes an opportunity has arisen to join our service on a fixed term basis. We are looking for a dynamic, passionate, and creative psychological professional who wants to make a difference to the lives of mothers, babies, and their wider families. You will have a passion for supporting both Adult and Infant Mental Health and have an interest in improving maternity systems and supporting those who have mental health needs associated with maternity or neonatal trauma or loss.
The successful candidate will be joining a supportive, engaged team of Perinatal and Maternal MH psychological professionals, MDT and wider service. The post holder will receive regular supervision and CPD as well as informal support. Across MPFT Perinatal Services there is a strong structure in place comprising of qualified clinical psychologists, psychological therapists and assistant psychologists supported by a Lead Psychologist for Perinatal and Maternal MH.
Main duties of the job
The service is part of MPFT Perinatal Services which includes Brockington Parent and Baby Unit and Shropshire Community Perinatal Mental Health Team. Psychological Professionals play an integrative role in the MDT and are valued and well supported by colleagues. While this post will be primarily focused on delivering psychological support to individual families there will be scope for delivering training and consultation to partner agencies and service development.
The team offers a range of individual and group interventions from a range of theoretical models. Clinical supervision structures are in place to support clinicians to access model specific supervision for specific interventions. We have an established CPD programme within the team and you will be supported to develop your psychological skills in this specialty. The psychology team have close connections with other psychologists within the Children and Families care group, the Maternal Mental Health Service within the Shropshire care group, and perinatal services in North Staffordshire, alongside the local DClinPsy courses offering regular trainee placements, research opportunities and perinatal course teaching.
The services covers the geographical region of South Staffordshire. There is an expectation for the successful post holder to be able to travel to meet the service need. We welcome applications for flexible and hybrid working.

The Mental Health Support Teams sit at the 'getting help' aspect of the Thrive model and work closely with the different pathways as part of the Children's Mental Health Function.
In the role you will be expected to contribute to undertake planned interventions with children, young people and their families as directed by qualified practitioners within MHST and/or Supervising Practitioners. This post is within the East Staffordshire team and covers Burton-on-Trent and Uttoxeter schools.
You will be responsible for working with colleagues in enabling schools to develop and embed their mental health and wellbeing strategies. This includes liaising with teaching staff and associated roles within the education setting, as well as with partner services and agencies. They will be involved in early identification and intervention of mental health need as well as delivering emotional health and wellbeingpromotion activities.
Main duties of the job
- To prepare, plan and carry out individual or group therapeutic interventions with children, young people and parents as indicated within the child / young person's care plan, with supervision and guidance from qualified practitioners.
- Complete planned reviews of care as requiredunder appropriate supervision.
- To be responsible for documentation and ensure that standards of record keeping are adhered to.
- To participate in and facilitate parent support and parenting skills groups.
- To participate in completing and reviewing risk assessments of the child/young person in terms of behaviour or mental health in relation to their circumstances, with guidance and supervision from qualified practitioners.

An exciting opportunity has arisen within our ESR & Pay Services Team for a Payroll Officer to join our friendly and customer-focused team.
We are looking for a motivated individual with payroll experience to support the delivery of an accurate and timely payroll service. You will manage your own allocated section of the payroll, provide advice and guidance on payroll-related matters, and work closely with managers, employees and external organisations to resolve queries.
This role offers hybrid working, combining home and office-based working, with attendance required for training, meetings and service delivery needs.
The successful candidate will have previous payroll experience, excellent communication and customer service skills, and the ability to work accurately under pressure to meet deadlines. Our community nursing teams deliver holistic, complex care to housebound patients within their own home environments. The Portering Supervisor / Working Porter is responsible for the delivery and day-to-day supervision of portering services across designated sites. The post holder will lead by example through active participation in portering duties while supervising the portering team to ensure safe, timely and efficient movement of patients, equipment, materials, waste, linen, mail and supplies. The post holder will contribute to service resilience by coordinating portering resources during periods of increased demand, unplanned events and emergency situations.
The post holder will be expected to work as part of the wider Facilities team, providing an integrated and flexible service, and as such may sometimes be required to undertake duties within other areas of the Facilities Services Department should the needs of the service demand it. This will include providing supervisory support and advice to teams within other disciplines should the need arise.
Main duties of the job

2. Undertake additional reasonable duties consistent with the role to support service continuity, including emergency response and incidental support to service users.

Job summary
An exciting opportunity has arisen within the Health and Justice service for the role of Reconnect Navigator based within the Black Country to support the strengthening of the female pathway within the service.
Reconnect supports people leaving prison and immigration centres. We will work with people up to 12 weeks prior to release and up to 6 months after release unless their health needs are met before.
The post will be based in the community however forging strong links with the female prisons will be vital to support the transition for those females being released into the Black Country.
It is expected that the post holder will form a strong working relationship with the female pathway leads from local Reconnect services and with the female pathway champions across Health and Justice within MPFT to develop the pathway and support the development of others within the team.
Reconnect is part of an integrated service with Liaison and Diversion and Mental Health Treatment Requirement services as part of the Health and Justice Service.
This is an 'all age' service and so you will be required to work with people from the age of 10 years upwards.
Prison vetting will be applied for as part of this post however please discuss any concerns regarding this as we also welcome applicants with lived experience.

Main duties of the job
o Independently undertake face to face and electronic screening to support the identification of multiple vulnerabilities of people who have been in contact with the criminal justice system or immigration centre, such as police custody, courts, and prison and community settings. Bromley Ward is an acute inpatient facility for individuals over 65 years presenting with a functional mental health condition or early stages of dementia, requiring admission for assessment and treatment with a planned discharge into the community.
We provide inpatient facilities for older adults with mental health problems and we are looking for caring, compassionate individual to work within our older adult service who are committed to providing high quality care and treatment to our patients. Working in an older adult environment can be challenging, but with the right team, positive attitude and resilience we are rising to the challenge and reaping the reward of being able to support people during their most difficult times.
Please note you must be able to work night shifts on rotation.
Main duties of the job
- Delivering a high quality clinical service to patients and their families.
- Acting as named nurse to a group of patients under the direction / supervision of the person identified with continuing responsibility.
- Participating in therapeutic group work programmes.
- Taking charge of the ward in the absence of the ward manager and deputy ward manager.
- You will be responsible for the assessment of care needs and contributing to; the formulation of the person's needs and the development of a relevant plan based upon the person's negotiated goals and relevant evidence based interventions.

As a band 4 Recovery Support Worker you will be delivering evidence based interventions in a group and 1:1 setting with our service users.

An exciting opportunity has arisen for experienced practitioners with a criminal justice interest, to work in the Sussex Health & Justice Liaison and Diversion Service. The post holder would be expected to work within the Outreach Team across the Sussex area. Many people would be unable to live at home without the support they receive from the Community Nursing teams and community services are admitting patients with a more complex acuity than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. Our Community Nursing teams assess, plan and deliver nursing care to patients within their own homes, in line with evidence-based practice.

This role includes daytime working, including weekends and cover of early morning shifts starting from 7am when required.
Due to the nature of community working, ability to travel across the patch to provide care to patients is essential.
Main duties of the job
As a highly skilled NMC Registered nurse you will work in collaboration with the muti-disciplinary team and colleagues to:
Promote independence and support patient centred care planning.
Provide care directly to patients on the caseload with a range of nursing needs (palliative & end of life care, long term condition management, medication administration, complex wound care and bladder & bowel management)
Work autonomously, in line with your scope of practice, competence and policies.
Handover patient information to the wider team, document patient information effectively.
Escalation of patients' needs as appropriate.
Lone working with the support from the wider team.

- Understand the scope of the community nursing service and the role of other MPFT and external services within each Primary Care Network. To ensure referrals are directed to the most appropriate service (including directing to emergency services where needed).
- Occasional delivery of care to patients in a community setting in order to maintain up to date clinical experience.
- Whilst not providing regular face to face patient contact the post holder will need to maintain an up-to-date knowledge of the clinical aspects of community nursing to resolve clinical queries over the phone and to enable effective remote assessment and clinical triage. This includes palliative care (particularly symptom management and support to families in the last few days of life); wound care; medication management; diabetes management; catheter and bowel care.
- Have supervisory responsibilities in the absence of the triage lead nurse; including coordinating the team; allocating work to team members; resolving and escalating issues; coordinating the daily huddle and group chat and representing the triage team on the community nursing capacity calls.
- Occasional supervision of students and new nurses joining the community nursing team.

North Shropshire Community Mental Health Service provide assessment and evidence based interventions for people who presenting with complex mental health difficulties that are significantly impacting on their daily life alongside our partner organisations.
We are looking for a practitioner to the join our team to work alongside our primary & secondary care practitioners and wider partners to support the triage and assessment of those being referred into our service.

Job summary
The Assistant Psychologist will work primarily within the Community Eating Difficulties Service (CEDS), while also providing psychological support to the ARFID pathway.
A key part of the role will be to contribute to multidisciplinary team (MDT) consultations, bringing a psychological perspective to discussions about children and young people presenting with eating difficulties. Within the ARFID pathway, the Assistant Psychologist will have a specific role in developing and delivering an 8-week parent and carer workshop programme. The workshop will provide parents and carers with psychological understanding, psychoeducation and practical strategies to support children and young people experiencing ARFID.
