Job summary
An exciting opportunity has arisen for experienced Practitioners (RMN, RNLD, RGN or Social Workers/ Allied Health Professionals) with a criminal justice interest, to work in the Black Country Health & Justice Outreach/Reconnect Service. The post holder would be expected to work in the community within Black Country in the Outreach element of the team but may also incorporate other areas of Health & Justice, such as the Reconnect Service, Police custody or court. This will include providing high quality assessments of people attending and being released from Police Custody/Prison and supporting people under the care of the Outreach/Reconnect Team. The service will be delivered to people of all ages including those under the age of 18yrs who present with any vulnerability including but not limited to; mental health, physical health, learning disability, head injury, unemployment, housing, educational needs etc.H&J is not itself a treatment service, but it is an identification, assessment and referral service. The role will involve assessing; accessing; and implementing a wide and, at times, complex range of care services for people over the age of 18 with increasingly complex needs.
The successful applicants must be experienced in risk assessment, highly motivated and someone who is looking for a new challenge and have the skills to make a difference.
Main duties of the job
1. Working in a Health and Justice (H&J) Outreach/Reconnect service within the Black Country, dependent on skills and service need.2. Delivering a high quality clinical service to patients and their families.3. Managing complex assessment in Custody, Court and the Outreach/Reconnect service. Responsible for clinical and caseload supervision of other team members where identified.4. Diverting and Liaising with multiple criminal justice, police and other partners.5. Identify, assess and refer people of all ages with a wide range of mental health, learning disability, substance misuse and/or social vulnerabilities when they first come into contact with the youth and adult criminal justice systems on suspicion of having committed a crime.6. When a person is assessed as having single or multiple vulnerabilities, they will be referred to the appropriate treatment or support service and an appropriate package of care and/or support will be instigated.7. Accurate, timely information on the person will be shared with police and the courts (with an individual's consent where necessary) to ensure that any charging, sentencing or disposal decisions are based upon an authoritative assessment of their mental health, any learning disability and whether they have a substance misuse issue.

The team in Lichfield is multidisciplinary and provides high-quality care for service users that experience mental health conditions. The team is part of the wider Integrated Mental Health Team covering Lichfield & Burntwood and links together for CPD events, supervision and partnership working.
We are seeking an experienced Mental Health Nurse who has a keen interest in working with this client group to provide a holistic overview and lead on the care and treatment of the person's journey through to recovery. The successful candidate will provide nursing leadership with the MDT, support junior colleagues in their development and facilitate student nurses in their training.
If appointed a Band 5-6 you will be supported to achieve a core set of competencies over a 12-month period. Upon successful completion you will progress to a Band 6.
Main duties of the job
- Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer and evaluation of individual treatment plans.
- Responsible for participating in the setting of quality standards, including the auditing, monitoring, and reviewing of practice in line with current clinical guidance practice and policy.
- Administer medication, complying with relevant Trust procedures and NMC Guidance.
- Assess patient's individual care needs, develop, implement, and evaluate programmes of care.

The post holder will support the delivery a high quality, comprehensive mental health service for children and young people with emotional health/mental health difficulties. The role involves managing a defined caseload with a focus on working with children and young people and their families who experience a complex range of needs and difficulties. Join our team as a Complex Trauma Cognitive Behavioural Therapist (CBT) within our established Ascend Service (Sexual Abuse and Assault Pathway) for 2 days a week, based in Cannock.
The Ascend Service is provided by Midlands Partnership NHS Foundation Trust, North Staffordshire Combined NHS Trust, VCSE (Survive and Woman's Aid) and Staffordshire Police Crime Commissioner.
The team comprises of a Consultant Clinical Psychologist, CBT Therapists, Assistant Psychologists and Psychological Wellbeing Practitioners to work with patients presenting with sexual abuse complex trauma with responsibility for assessing patient suitability and delivering evidenced based, trauma focused interventions and clinical supervision.
You will be a BABCP accredited CBT Therapist with a minimum of 2 years of demonstratable clinical experience of working with people experiencing trauma and will need clinical supervision training to deliver clinical supervision to the teams PWP's. Training in EMDR and/or experience of working in secondary care mental health services would be desirable.
Hours are 9-5. There may be an expectation to work flexibly between the hours of 8am-8pm Monday to Friday to meet service needs and in the future, on Saturday morningsApproximately 50% of the working week will be office based and 50% can be worked remotely across South Staffordshire.
Main duties of the job
Good communication skills and an informed clinical judgement are vital to this role as you will be liaising between primary and secondary sectors to facilitate referrals into the complex trauma pathway.
As part of this role, you will be expected to:
- Make decisions on the suitability of new referrals, identifying the most suitable service for the patient
- Formulate, implement and evaluate individual therapy programmes for patients that fall between Primary and Secondary services
- Attend Inter service meetings with regard to referrals
- Liaise with other health and social care staff from a range of agencies with a regard to referrals
- Promote and maintain links with services across the Trust to help co-ordinate provision of an effective Psychological Therapies Service
- Provide specialist advice to other professionals from the Trust and voluntary agencies in regard to our therapeutic service provision

The post holder will support in the coordination of care for service users and service providers across Staffordshire. Do you have lived experience of a recovery journey relating to mental ill health and accessing community mental health services? Would you like to support other people who are currently experiencing mental health issues? If so, we would like to hear from you as we have an exciting opportunity to join our Integrated Mental Health Team based in Codsall as a Peer Support Worker.
The Integrated Mental Health teams work as part of a 'neighbourhood' offering place-based provision of mental health services in partnership with other local organisations. Clinicians within the IMHT will offer interventions and treatments which are concordant with NICE guidelines, and allocation within the team will be based on the needs of the service user, matching clinician skills to service user needs.
Main duties of the job
A key feature of this post is to promote individual person-centred recovery by drawing on the peer support workers own personal lived experience of mental health challenges and of accessing mental health services.
As a pivotal and highly valued member of the team, peer support workers will have a case load of identified patients who they will work with on both an individual basis and by delivering group interventions, working in collaboration with the wider multi-disciplinary team.
This formalised peer support will allow the post holder to provide a safe space for individuals to talk and make sense of their experiences whilst sharing wisdom from their own recovery to inspire hope and belief that recovery is possible.
They will promote choice, self-determination and opportunities and assist the individual to set and achieve their own unique recovery goals and recognising that everyone's recovery is a distinctive and deeply personal process.
Their work may also involve providing practical assistance such as encouraging healthy lifestyles, helping individuals to build confidence, and offering information, advice and support to form connections within their local community resources.

Job summary
An exciting position has arisen within our Lichfield Mental Health Support Team. We are looking for an experienced & enthusiastic Education Mental Health Practitioner.
Youwill continue to build on the fantastic work carried out within your allocated schools. The role is varied & no two days are the same, you may be setting up and delivering group work & assemblies one day to delivering assessment & interventions the next, as well assupportingschools to develop their whole school approaches to mental health & wellbeing.
You will be skilled & experienced in conducting mental health assessments of children and young people (CYP) presenting with common mental health difficulties such as anxiety and low mood. You will conduct risk assessments & identify the right level of support for CYP you come into contact with.
Alongside the clinical aspect of your role, you will play an integral part in supporting schools with their whole school approaches. There is scope for creativity and opportunities to work alongside CYP & parents/carers to drive the whole school approach agenda.
This is an opportunity to be involved in an established team within the wider CAMHS structure, as a joint initiative between health & education. You will work across the Staffordshire locality as well as supporting other MHSTs across South Staffordshire. You will work deliver parent, staff well-being, 1:1 & group work sessions.
Main duties of the job
MPFT will offer a welcome bonus of 5% of first year salary- at 50% on commencement of employment and 50% following 12 months employment.
- Assess and deliver outcome focused, evidence-based interventions in educational settings for children and young people experiencing mild to moderate mental health difficulties, working at all times in collaboration with and giving respect to the education function of the setting in which the post-holder is deployed.

Pine ward is a 15 bedded mixed gender adult acute admissions ward providing treatment and care for a wide range of mental health illnesses to patients who are of working age.

Main duties of the job
- Delivering a high quality clinical service to patients and their families.
- Acting as named nurse to a group of patients under the direction / supervision of the person identified with continuing responsibility.
- Participating in therapeutic group work programmes.
- You will be responsible for the assessment of care needs and contributing to; the formulation of the person's needs and the development of a relevant plan based upon the person's negotiated goals and relevant evidence based interventions.

An exciting opportunity has arisen for a Occupational Therapist to be part of the Home First service in South Staffordshire.
This is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs within Midlands Partnership University NHS Foundation Trust (MPFT).
The Home First Team prides themselves on the delivery of quality care to community patients, many with complex needs.
The team also work to facilitate safe hospital discharges for patients leaving acute care and to offer them nursing support in their own home environment.
Thisis a dynamic post which will be evaluated and adjusted to reflect the needs of the service, local population and feedback from users and staff. Your feedback and input will be essential to the development and shaping of the role.
The Home First covers South Staffordshire, Burntwood, Lichfield, Tamworth, Burton, Uttoxeter and surrounding areas, providing clinical support for patients in the community to either avoid potential hospital admissions or to support people returning home through the Discharge to Access Pathway 1 following a period of acute care.
The role will require working 5 days over 7 including Bank Holidays.
Main duties of the job
The post holder will be part of the Allied Health Professions (AHP) service providing Occupational Therapy and will work in a variety of settings.
This is a dynamic post that requires the post holder to work closely with uni/multi-disciplinary teams/agencies in addressing a range of care/support issues across the spectrum of conditions, preventing unnecessary hospital admissions and improving the health and wellbeing of the population served. We are seeking a passionate and dedicated Assistant Therapy Practitioner to join the Home First in the North of Staffordshire. You will be responsible for delivering high quality rehabilitation to patients with a range of physical health conditions including MSK, Stroke, Parkinson's disease, Neurological conditions, Spinal Injuries, Cancer, Post Surgery, General Frailty, Amputees, plus a wide range of other conditions which provides you with a varied and exciting experience within our service.

- To complete secondary assessments following hospital discharge, including triage, subjective and objective assessments for appropriate patients. We are offering a rare opportunity to join our well-established Lighthouse MMHS in Shropshire Telford and Wrekin. We are a Psychology led, values driven team, providing a service for families (including partners) who have experienced birth trauma, pregnancy/neonatal loss, or moderate to severe distress related to fear of pregnancy/childbirth. The team hold a significant amount of expertise, with our clinical lead and several clinicians having also been involved in delivery of the Maternity Review Psychology Service for families who were part of the Ockenden Review.
We are looking for a compassionate, family focused Psychologist with a keen interest in maternal mental health, to work collaboratively with colleagues in providing a service to families experiencing moderate to severe distress related to birth trauma, loss, or fear of pregnancy/childbirth.
The post holder will work closely with specialist midwives, consultants and other mental health services, to offer a holistic service for families
Main duties of the job
- Provision of a Psychology service within the Lighthouse Maternal Mental Health Shropshire, Telford & Wrekin, including complex mental health needs. This is to involve providing specialist psychological assessments, leading to psychological formulations of distress; interpretation & opinion; and forming judgements to inform service provision & providing necessary psychological interventions.
- The post holder will work autonomously within professional guidelines and the overall framework of the service's policies and procedures. In so doing, they will be responsible for adhering to relevant Trust policies and professional ethics & standards. The post holder will be accountable for both professional & clinical actions.
- As a senior clinician, the post holder will develop a field of expertise that is a basis for providing advice to the service within which they work and will contribute to service and project development across the wider Trust area. This to include leading or contributing to training, supervision and governance aspects of the specific models of service delivery.
- The post holder will supervise and support the psychological assessment and therapy provided by more junior Psychologists within Lighthouse and provide peer supervisory support to colleagues as appropriate.

Job summary
An exciting opportunity has arisen for a NMC Registered Band 5 Nurse to work within the Home First Team (Discharge to Assess Pathway 1 based in South Staffordshire).
The Home First Team provides clinical support for patients in the community to either avoid potential hospital admissions or to support people to return home through the Discharge to Assess Pathway 1 following a period of acute care in hospital.
We pride ourselves on the delivery of quality care to community patients, many with complex needs and we actively promote the philosophy of self-care and the promotion of independence whilst offering the facilities of rehabilitation and assessment. The ethos of the team is to facilitate safe hospital discharges for patients leaving acute care and to offer them nursing support in their own home environment so they can remain in their own home and avoid unnecessary hospital admissions.
As an efficient and effective team member you will work collaboratively in a multi-disciplinary team alongside the Operational Leadership to ensure high quality care delivery to service users and provide specialist assessment and care planning in conjunction with service users and carers.
Due to the nature of this role, you will be required to work shift patterns covering a 24/7 365-day period and also be required to travel independently over a large geographical area.
Main duties of the job
As a Band 5 Nurse you will provide holistic assessment, planning, implementation and evaluation of care as part of the D2A Pathway 1 process.

Main duties of the job
