Main duties of the job
To be responsible for undertaking a wide range of administrative duties and the provision of general support to the multidisciplinary team. Duties can include, but are not limited to, the processing of information and dealing with telephone calls and assisting patients as required. A more details list is included below.
To act as the central point of contact for patients, the distribution of information, messages and enquires for the clinical team, liaising with multidisciplinary team members and external agencies such as secondary care and community service providers.

Secondary Responsibilities

This position offers a unique opportunity to work as an autonomous practitioner within a supportive Home Visiting Team, delivering care where it is often needed most.
