Main duties of the job
As a valued member of the Operose Health team, you will provide care for presenting patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. Safe, clinical decision-making and expert care, including assessment and diagnosis skills for patients within the general practice, are crucial. Collaborative work with the general practice team to meet patients' needs, support policy delivery, and provide nurse leadership is expected. Ideal candidates should have experience in a Primary Care setting, be an NMC registered nurse with advanced clinical practice or working towards non-medical prescriber status, and possess a minor injury and illness qualification. Level 7 (MSc) Practitioners are preferred, though candidates with accreditation or a MSc course to ACP status may be considered.

Main Responsibilities:
As a valued member of the Operose Health team, your key duties will include:
- As a member of the practice team, you will work closely with our clinicians to help and improve patient care and make cost effective proscribing decisions.
- You will run daily telephone triage clinics and call in patients for assessment and management where clinically indicated, as well as review and triage pathology results, contacting patients. Provide practice-based review clinical to those with LYCs or complex health needs.
- Review and action incoming hospital correspondence, particularly with regards to medicine reconciliation or initiation, liaising with local pharmacies to improve patient outcomes.

Main Responsibilities:
As a valued member of the Operose Health team, your key duties will include:
- To deliver high quality clinical care to the patients registered and attending the GP surgery and to work closely with the clinical leads and local practice team to support the delivery of the surgeries operational plan.
- You will strive to maintain qualify within the practice ensuring participation in clinical governance activity and contribute to the improvement in qualify health outcomes through audit and the Quality and Outcomes Framework and contributing to the practice achieving other qualify standards to include CQC standards.

Southover Medical Practice are looking for a Practice Nurse for 21.5 hours per week.
Main Responsibilities:Key duties will include:
- You will assist the registered manager with duties related to CQC outcomes, ensuring that compliance is maintained. As well as working collaboratively with the general practice team to meet the needs of the patients, ensuring practice policies and procedures are followed.
- Providing care for the presenting patient, treatment, chronic disease management and evaluation of care.
