Job summary
A opportunity has arisen to join a well-established team within the District Nurse Triage Team at Oxford Health NHS Foundation Trust.
Using sound clinical knowledge, professional judgement and risk assessment skills, clinicians will assess referrals, clearly identify urgent and non-urgent care needs, and coordinate the most appropriate response.
This may include arranging urgent, same-day or routine visits from District Nurse clinicians, signposting patients to alternative services, or providing clinical advice where appropriate.
The team plays a key role in coordinating timely, responsive and patient-centred care, ensuring patients receive the right support in the right place and at the right time within the community setting.
The successful candidate will:o Clinically triage referrals received by telephone and email.o Use clinical knowledge, professional judgement and risk assessment skills to identify the appropriate level of care and urgency.o Arrange and coordinate urgent, same-day and routine visits for patients in the community.o Provide clinical advice and support where a face-to-face visit is not necessary.o Work closely with District Nurse teams and wider community services to ensure safe and effective care coordination.o Contribute to the ongoing development of the District Nurse Triage Desk and the future delivery of community care.
Main duties of the job
The services currently operates 7 days a week, 365 days a year, with core hours of 08.00-18.30.
Shift patterns are 08.00-16.00, 10.30-18.30 or 08.00-18.30.
What will we give you?
Once you have started with Oxford Health you will be on the road to develop and hone your clinical skills. We offer each person a bespoke induction in your team with a period of being supernumerary , which is usually up to 4 weeks. We will ensure you can attend the District Nursing Induction programme, this is tailored to district nursing staff, encompassing as facets of Oxford Health and the lived experience of what you will do, giving you an insight into the organisation. The preceptorship programme is another opportunity for you and you will be able to undertake a service development project. We are Oxford Health pride ourselves in supporting and developing our staff to achieve their potential and to fly in their career.
This role is a Triage based office job and requires to you to be on site.

Job summary
Oxfordshire's Mental Health Urgent Care pathway is now established and comprises the Adult Crisis Resolution and Home Treatment Team, CAMHS Crisis Resolution and Home Treatment, emergency department liaison services, and the mental health helpline.
The function of the CAMHS Crisis and Home Treatment Team is to provide patient-centred intensive care and treatment to patients and their families at home as an alternative to hospital admission. Crisis care is a highly specialist intervention and involves supporting our most vulnerable patients and their families in the least restrictive setting possible by providing a variety of recovery-focused psychosocial interventions that enable the person to regain or maintain independence.
The CAMHS CRHTT operates 24 hours a day, with core hours 08:00-20:00, and works closely with CAMHS community services, inpatient wards, children's social care, schools, local acute hospitals, and other emergency services (ambulance & police).
Successful post holders will be expected to actively contribute to the ongoing evaluation and development of interventions. Main duties of the job
As a Clinical/Counselling Psychologist in the CAMHS Crisis Resolution and Home Treatment Team, you will:
- Provide specialist psychological assessment, formulation, risk assessment and evidence-based interventions for children and young people experiencing severe or complex mental health crises, working closely with their families and professional networks to support safe care at home wherever possible.
- Work autonomously as part of a multidisciplinary, countywide service, offering consultation, advice, training and supervision to colleagues, trainees and assistant psychologists, while maintaining high-quality clinical records and contributing to care planning and review.
The role also involves travelling independently between community settings across Oxfordshire and contributing to audit, research, service evaluation and the continuing development of responsive, accessible crisis care; it will therefore suit an experienced, compassionate and resilient practitioner who communicates well, manages competing demands and is comfortable working flexibly with at times highly emotive unpredictable situations.

We are recruiting for a fixed term role to cover maternity leave as a Personal Assistant at Oxford Health NHS supporting the Director of Psychological Professions and Associate Director of Social Work and Social Care. We are recruiting for a Head Housekeeper / Cleaner to join the Cherwell Ward at the Fulbrook Centre in Oxford to manage the housekeeping team and ensure all service standards are met and maintained.
This role is 37.5 hours per week (full time) working various shifts including alternate weekends.

We're recruiting for an Mental Health Nurse to join the Crisis Team as Mental Health Crisis Team Clinician.
The Crisis Response and Home Treatment Team is a key part of the Urgent Care Pathway meeting the demands of the Ten-Year Plan to provide Crisis Response and Home Treatment Services by 2021. Clinicians employed into the team are part of an exciting and developing service aimed at supporting our most vulnerable patients and their families in the least restrictive setting possible by providing a variety of highly intensive interventions at home.
Main duties of the job
We require an RMN/clinician who can bring their own professional expertise to a fast-paced team, and who is confident in managing risk in the least restrictive way. By its nature, crisis work requires professionals who have a broad range of skills including being skilled mental health assessors with a proven ability to signpost service users towards the most effective treatment pathways.
Crisis can manifest with a range of needs as you have a background in mental health nursing, we want you to use your skills as part of a team to develop individualised care packages for our service users and their families.
Service Users access a crisis response in a variety of ways, ranging from referrals from their GP and attendance at an emergency department, to when they need care in the community and are seen by the police and our street triage teams. The aim of the Urgent Care Pathway is to bring together all those access points under one team, including:
- Psychiatric Liaison
- Overnight Response Team
- Assessment Teams
- Health Based Places of Safet

We are now recruiting to this Women's Transitional Mental Health Lead within theThames Valley Offender Personality Disorder Pathway, developing and leading a pilot service supporting women with complex mental health needs and significant offending histories to successfully move on from custody. This transition is often a time of increased anxiety, especially for women who have experienced significant trauma and adversity.
To be successful in this role, you should be a Psychologist (newly qualified or experienced), Social Worker or Mental Health Nurse.
You will provide liaison and advocacy to support women to access and engage with relevant services and will carry a small caseload of women to support directly. You will build strong working relationships with professionals from a variety of settings including prison, probation and community mental health and social care.
You will join a specialist and highly supportive team with regular CPD and excellent opportunities for career development.
This post will be fixed-term for 24 months.

Main duties of the job

We are hiring 2xFull-Time Senior Assistant Psychologistswho will each work in a split role based on the Littlemore Mental Health Centre site:
- 1x in a male medium secure service and in amale low secure service.

Main duties of the job
The post holder will provide highly specialist assessment, formulation and evidence-based interventions for children and young people with complex emotional and mental health needs within CAMHS. Working autonomously, they will manage a clinical caseload, provide consultation, supervision and specialist advice to colleagues and partner agencies, and contribute to multidisciplinary decision-making.
The role includes supporting carers through psychoeducation, promoting secure caregiver-child relationships, convening and influencing complex professional networks, and advocating for children and young people to achieve the best outcomes.

We're looking for a Psychologist to join our team, either as a Band 8a or in a developmental role, as a Band 7 to Band 8A. This full time role within our Inpatient Medium Secure forensic male wards - Oxford Clinic.
In our inpatient services, you will develop skills in risk assessment, MDT working and service development, alongside developing therapeutic expertise in supporting patients to maximise their potential and work towards successful and safe transition into the community.
This post attracts an annual RSU allowance payment of £1446 per annum.

Main duties of the job
In this role, you will provide highly specialist psychological advice, consultancy, guidance and where appropriate supervision to other members of the MDT (e.g. nursing staff) who are involved in client assessment and who provide direct intervention in individual cases and with groups of clients.
You will be supporting the development of other members of the MDT in the appropriate identification and use of psychologically informed assessment methodologies and treatment interventions as part of a client's agreed care plan. You'll contribute to the development, evaluation and monitoring of the team's operational policies and services, by attendance at meetings at which service developments are planned and discussed and through the deployment of professional skills in research, service evaluation and audit.

The Crisis Response and Home Treatment Team in Buckinghamshire Mental Health Services is a key part of the Urgent Care Pathway meeting the demands of the Ten-Year Plan to provide Crisis Response and Home Treatment Services to the county of Buckinghamshire.
Clinicians employed into the team are part of an exciting and developing service aimed at supporting our most vulnerable patients and their families in the least restrictive setting possible, by providing a variety of highly intensive interventions at home.
Main duties of the job
A Crisis can manifest with a range of needs and whether you have a background in mental health nursing, social care or occupational therapy we want you to use your skills as part of a team to develop individualised care packages for our service users and their families.
We deliver home treatment interventions, as well as an online acute treatment programme and clinics at Saffron House, with support from psychiatrists, psychologist and the other clinicians. Training in brief interventions is encouraged and fully supported, as well as any other training related to Crisis Intervention and Home treatment.
Service Users access a crisis response in a variety of ways, ranging from referrals from their GP and attendance at an emergency department, to when they need care in the community and are seen by the police. The aim of the Urgent Care Pathway is to bring together all those access points under one team, including:
- Psychiatric Liaison
- Overnight Response Team
- Assessment Teams

We are looking for a Mental Health Nurse, Occupational Therapist or Social Worker to join our Older Adults Community Mental Health Team (CMHT) based in the South of Bucks. We are expanding the permanent posts within our duty team and are recruiting into a full time post into this dedicated team, working alongside our Duty team and Clinical Lead Nurses.
We are a friendly, supportive and proactive team with a culture of high standards of care and any successful applicant will need to be committed to maintaining this. The team works closely with a wide range of internal and external partner's including GPs, district nursing, therapy services, ambulatory care, social care and physical and mental health inpatient providers. We aim to deliver well-coordinated patient care and to share our respective skills and knowledge with one another.
Our team covers a wide geographical area bordering Berkshire, Oxfordshire, Slough, Amersham and Marlow and so is ideally placed for staff commuting from Oxford, Milton Keynes, Reading and Aylesbury.
Our main office is in High Wycombe and our inpatient beds are based in Aylesbury.
Core working hours are Monday-Friday 09:00-17:00. Out-of-hours support services are managed via a team rota. Staff will be expected to work on this rota which includes occasional weekend 09:00-17:00 shifts.
Main duties of the job
Working as part of our MDT you will be responsible for supporting the coordination of our duty team, triaging referrals, planning assessments and liaison with the bed flow team, and local Psych wards. You will be expected to respond to urgent and emergency referrals, providing crisis support for people with dementia, and brief interventions to our older adult patients with mental health needs. You will also work closely with the crisis team who provide support for older adults with functional disorders.
The MDT is made up of Medical, RMN's, OT, SW and Psychology colleagues and we highly regard one another's professional background and contribution to the care of our patients. We view supervision as fundamental to the functioning of the team both clinically and in terms of the well being of our staff.
The responsibilities of the role include, but are not limited to:
- Work as an autonomous practitioner as an active participant of the multi-disciplinary team undertaking specialist assessments in collaboration with the patient and their families/carers and presenting back to the wider MDT.
- Work in partnership with service users and their families/carers to ensure the development of a treatment plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community. This may include working closely with partner agencies.

This key role within our urgent community response service supports provision of high quality care for people experiencing immediate health and care crisis and needing intervention at home to prevent hospital admission (where safe to do so).
The role provides the successful clinician with an opportunity to work within our community same day care services and help test, inform and finesse their continued development within the community.
Our same day care services currently operate on a shift basis working over seven (7) days a week 8am to 8pm, so flexible working into the evenings, over some weekends and bank holidays will be required. We are open to flexible and part time hours and the post is available part-time (minimum 30 hours per week) or full-time (37.5 hours per week).

Working with a high degree of autonomy to:

We are seeking to recruit a full time Activity Coordinator to join one of our Mental Health Wards based at the Warneford Hospital.
Allen Ward is a 18 bedded female acute working age mental health ward, supporting patients who are in an acute crisis of their mental health diagnosis.
The activity co-ordinator post forms part of the multi-disciplinary team, including Nurses, Doctors, Occupational Therapists, Psychologists, Health Care Assistants, Physiotherapist, Physio Technician, and Dietician.
Each of the wards at the Warneford Hospital have an activity co-ordinator post, providing lots of opportunities to link up with others and receive regular peer supervision.
Ward staff are required undertake PEACE training. This 5-day training course covers the curriculum relating to the prevention and application of Restrictive Practice and Interventions as detailed in the Restraint Reduction Network Training Standards 2019. Therefore, applicants need to have a good range of mobility and fitness to learn the physical restraint techniques taught on the course.

- Plan and co-ordinate a programme of activity under the supervision and direction of members of the multi-disciplinary team.

Main duties of the job
The role will involve being the allocated key worker (formerly known as Care Coordinator) for a caseload of patients with severe and enduring mental illness. In these instances, you will be playing an integral role as part of a multi-disciplinary team in that person's recovery, which would include the assessment, collaborative care planning and review of the person's needs.
You will also be expected to undertake bio-psycho-social assessments for patients referred to the team, and this may include offering brief evidence based interventions for up to eight weeks, ensuring to work in partnership with service users and their families/carers to ensure the development of a treatment plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community.

An exciting opportunity has arisen for a highly motivated and experienced administrator to join the Single Point of Access service in Oxfordshire.

- Previous experience in an administrative role.
- Ability to minute meetings.
- Organisational skills/managing diary systems and tracking information.
- Experience of managing information on a database
- experience of using computer programs including Excel, Word, and Outlook.
Desirable
- Working knowledge of Electronic Patient Record system.
- Experience in office management and supervisory/management role for staff.
- Experience of arranging and conducting interviews and of inducting new staff.
- An understanding of information governance.
- Data analysis and interpretation skills. Knowledge of project management or information analysis.
