Job summary
We are seeking an enthusiastic and dedicated GP to work alongside our clinical team to play an important role complementing and enhancing the Rise Park Surgery Team. This is a 2-4 session a week role; days of work by negotiation. We are open from 0700h on Wednesdays and Thursdays and every fourth Saturday morning and you may be required to cover these sessions.
We have recently undertaken an extension of our building and we are now looking for a like-minded team member to help us care for our patients. Successful applicants will be working in a brand-new clinical space which has been constructed and modified to our requirements.
We welcome applications from both experienced and newly qualified/early career GPs. We would support the successful applicant wishing to join the Phoenix program or who are on the GP retainer scheme.
Annual leave entitlement will be 30 days plus statutory bank holidays and 5 days study leave (pro rata). Additionally, your medical defence indemnity expenses will be covered by the practice for work undertaken at Rise Park Surgery.

Main duties of the job

Rise Park Surgery is a vibrant, forward thinking, team orientated Nottingham City General Practice serving 9700 patients. We have a great work ethic; caring for our patients, our community and the team around us is paramount to all that we do.
We are currently looking to add to our multi-disciplinary clinical team which is made up of GP Partners, Salaried GPs, Pharmacist, Advanced Clinical Practitioner, Practice Nurses, Healthcare Assistants, Prescribing Technicians and Phlebotomists. We also have a wide range of ARRS staff working with us at Rise Park Surgery including First Contact Practitioners, Social Prescriber, Health & Wellbeing Coach, Dementia Nurse and a Mental Health Worker.
Rise Park Surgery is also a training practice welcoming GP Registrars and Senior and Junior Primary Care Students. We are also a research accredited practice and encourage our clinicians to get involved.
The clinical team are supported by an excellent, friendly, proactive and efficient admin team. There is a full program of educational meetings and support and mentorship is available.
We hold a CQC 'Good' rating, we use 'systmone' and we are a leading practice within the Bulwell and Top Valley PCN.

We are seeking an experienced Practice Nurse to work alongside our clinical team to play an important role complementing and enhancing the Rise Park Surgery team. This is a part time role of 20 hours per week.

JOB PURPOSE
The post holder is responsible for the delivery of practice nursing services care to the practice population. Supported by the Lead Prescribing Nurse and General Practitioners within the practice, they will deliver care within the boundaries of their scope of practice, focusing upon supporting patients to be healthy, monitoring and management of long-term conditions, health prevention, wound care and screening activities that may include cervical screening, baby immunisations and travel health. Job summary
Bulwell and Top Valley PCN (Lead Employer: Rise Park Surgery) is recruiting an enthusiastic and highly organised Extended Access & PCN Services Coordinator to support the delivery and ongoing development of services across our network.
This is an exciting opportunity for an individual with excellent administrative, organisational and communication skills to play a key role in coordinating our Extended Access Hub and supporting the effective delivery of PCN services. Working closely with GP practices, clinicians, PCN staff and external stakeholders, the successful candidate will ensure services are delivered efficiently, appointments are managed effectively, and performance data and reporting requirements are maintained to a high standard.
The role requires a proactive and flexible approach, with the ability to manage multiple priorities in a fast-paced primary care environment. Previous experience within healthcare, general practice, NHS administration or service coordination would be advantageous.
We are looking for someone who is motivated, detail-focused and committed to delivering high-quality patient services across Bulwell and Top Valley Primary Care Network.

Main duties of the job
The Extended Access & PCN Services Coordinator will support the delivery of high-quality services across Bulwell and Top Valley Primary Care Network. The role will be responsible for coordinating the day-to-day operation of the Enhanced Access Hub, managing clinical rotas and appointment schedules, supporting service delivery, and liaising with practices and clinicians to ensure clinics run effectively.
The post-holder will also provide administrative support to the PCN leadership team, including coordinating meetings, preparing agendas and minutes, maintaining records, producing reports, and supporting PCN projects and service improvement initiatives.
Working closely with member practices and stakeholders, the successful candidate will help monitor activity, support reporting requirements, manage competing priorities, and contribute to the efficient, patient-focused delivery of PCN services.
